Evidence‐supported interventions for children in care: Does Treatment Foster Care Oregon (TFCO) fit within the UK context?

BackgroundChildren in care often have poor outcomes. There is a lack of evaluative research into intervention options.AimsTo examine the efficacy of Multidimensional Treatment Foster Care for Adolescents (MTFC-A) compared with usual care for young people at risk in foster care in England.MethodA two-arm single (assessor) blinded randomised controlled trial (RCT) embedded within an observational quasi-experimental case–control study involving 219 young people aged 11–16 years (trial registration: ISRCTN 68038570). The primary outcome was the Child Global Assessment Scale (CGAS). Secondary outcomes were ratings of educational attendance, achievement and rate of offending.ResultsThe MTFC-A group showed a non-significant improvement in CGAS outcome in both the randomised cohort (n= 34, adjusted mean difference 1.3, 95% CI −7.1 to 9.7,P= 0.75) and in the trimmed observational cohort (n= 185, adjusted mean difference 0.95, 95% CI −2.38 to 4.29,P= 0.57). No significant effects were seen in secondary outcomes. There was a possible differential effect of the intervention according to antisocial behaviour.ConclusionsThere was no evidence that the use of MTFC-A resulted in better outcomes than usual care. The intervention may be more beneficial for young people with antisocial behaviour but less beneficial than usual treatment for those without.

This article presents findings from research into how young people growing up in foster care in the UK manage the relationships in their social networks and gain access to social capital. It is a concept that highlights the value of relationships and is relevant to young people in care as they have usually experienced disruptions to their social and family life. Qualitative methods were used and the findings show that despite experiencing disruption to their social networks, the young people demonstrated that they were able to maintain access to their social capital. They achieved this in two ways. Firstly, they preserved their relationships, often through what can be seen as ordinary practices but in the extraordinary context of being in foster care. Secondly, they engaged in creative practices of memorialisation to preserve relationships that had ended or had been significantly impaired due to their experience of separation and movement. The article highlights implications for policy and practice, including the need to recognise the value of young people’s personal possessions. Furthermore, it stresses the need to support them to maintain their relationships across their networks as this facilitates their access to social capital.

The Pyjama Foundation is an Australian charity working to improve the literacy and numeracy outcomes for children in foster care. The foundation delivers the Pyjama Foundation Love of Learning programme, a learning-based mentoring programme in which volunteer ‘Pyjama Angels’ visit children in care each week to read books, play games and engage in other learning-based activities.This study surveyed 121 Love of Learning mentors (‘Pyjama Angels’) to assess their perceptions of the relationships they had developed with the children they mentored and of the children's improvement in their literacy skills, a key aim of the programme.The statistical data analysis based on the structural equation modelling and multiple regression approach showed that several factors had a statistically significant impact on the mentors’ perceptions of the children's improvement in literacy skills: relationship with the child, child's engagement and tenure in the programme, and frequency of meetings. Age and gender of the mentors were not found to have a statistically significant impact on mentors’ perceptions of this improvement, while mentors’ perceptions of their relationship with the children was the most important factor influencing their perceptions of improvement in literacy skills. The study did not include objective measures of the children's literacy outcomes, so its results are limited to the mentors’ perceptions. However, this study offers valuable insights for mentoring programmes working with children living in foster care.

Children in care have higher rates of mental health problems than the general population and placement instability contributes to this. Children are both most vulnerable to the effects of poor quality care and most responsive to treatment in the early weeks and months of life yet, in the UK, permanency decisions are generally not in place until around the age of four. We aimed to understand the components of an innovative system for assessing and intervening with maltreated children and their families developed in New Orleans and to consider how it might be implemented in Glasgow, UK. During and after a visit to New Orleans by a team of Glasgow practitioners, eight key interviews and meetings with New Orleans and Glasgow staff were audio-recorded. Qualitative analysis of verbatim transcripts identified key themes. Themes highlighted shared aspects of the context and attitudes of the two teams, identified gaps in the Glasgow service and steps that would be needed to implement a version of the New Orleans model in Glasgow. Our discussions with the New Orleans team have highlighted concrete steps we can take, in Glasgow, to make better decision-making for vulnerable children a reality.
