ObjectivesTo investigate the impact of standardisation of the perioperative protocol based on the Joint Commission International (JCI) accreditation guidelines for operating time in cataract surgery.DesignRetrospective observational study.SettingSingle centre in Japan.ParticipantsBetween March 2014 and June 2016, 3127 patients underwent cataract surgery under topical anaesthesia including 2581 and 546 patients before and after JCI accreditation, respectively.Primary and secondary outcomesWe compared three time periods, comprising the preprocedure/surgery time (pre-PT), PT and post-PT, and total PT (TPT) of cataract surgery between patients before and after JCI accreditation, by regression analysis adjusted for age, sex and cataract surgery-associated confounders.ResultsThe main outcomes were pre-PT, PT, post-PT and TPT. Pre-PT (19.8±10.5 vs 13.9±8.5 min, p<0.001) and post-PT (3.5±4.6 vs 2.6±2.1 min, p<0.001) significantly decreased after JCI accreditation, while PT did not significantly change (16.8±6.7 vs 16.2±6.3 min, p=0.065). Consequently, TPT decreased on average by 7.3 min per person after JCI accreditation (40.1±13.4 vs 32.8±10.9 min, p<0.001). After adjusting for confounders, pre-PT (β=−5.82 min, 95% CI −6.75 to −4.88), PT (β=−0.76 min, 95% CI −1.34 to −1.71), post-PT (β=−0.85 min, 95% CI −1.24 to −0.45) and TPT (β=−7.43 min, 95% CI −8.61 to −6.24) were significantly shortened after JCI accreditation.ConclusionPerioperative protocol standardisation, based on JCI accreditation, shortened TPT in cataract surgery under local anaesthesia.

Pembangunan kesehatan telah berhasil meningkatkan pelayanan kesehatan secara lebih merata, namun peningkatan mutu baik mutu pelayanan kesehatan itu sendiri maupun mutu sumber daya manusia masih perlu senantiasa diupayakan dengan lebih mengembangkan dan menerapkan ilmu pengetahuan dan teknologi. Dengan semakin meningkatkan pendidikan dan status sosial ekonomi, masyarakat cenderung menuntut pelayanan kesehatan lebih baik dan lebih bermutu.Rumah Sakit sebagai salah satu institusi pelayanan kesehatan memiliki fungsi penting dalam meningkatkan derajat kesehatan masyarakat sehingga dituntut untuk selalu meningkatkan mutu pelayanan yang diberikan. Dalam hal ini semua pihak di dalam rumah sakit saling berkaitan satu sama lain, mulai dari Yayasan pemilik, direksi, para dokter, perawat, dan profesional lainnya serta staf pada umumnya.Kualitas rumah sakit tidak hanya terlihat dari bangunan megah, dokter-dokter yang berpengalaman, obat-obatan yang lengkap, dan peralatan medis yang serba canggih. Rumah Sakit dituntut untuk memberikan pelayanan kesehatan terbaik dan lebih terbuka pada masyarakat. Kepercayaan masyarakat terhadap mutu pelayanan kesehatan dapat ditingkatkan dengan adanya status akreditasi karena standar standar yang ditetapkan dalam akreditasi dibuat untuk memenuhi hak-hak pasien.Berdasarkan UU No. 44 tahun 2009 tentang Rumah Sakit mewajibkan rumah sakit menjalani akreditasi, Peraturan Menteri Kesehatan No. 659 tahun 2009 tentang Rumah Sakit Indonesia kelas Dunia, SK Menteri Kesehatan No. 1195 tahun 2010 tentang Lembaga Akreditasi Rumah Sakit Bertaraf Internasional, dan Keputusan Direktur Jendral Bina Upaya Kesehatan Nomor HK.02.04/1/2 90/11 Tentang Standart Akreditasi Rumah Sakit menunjukkan bahwa pemerintah Tengah melakukan penyempurnaan akreditasi Rumah Sakit menuju akreditasi internasional yaitu JCI (Joint Commission International). JCI adalah suatu organisasi yang independen, nonprofit, dan bukan lembaga pemerintahan yang berpusat di Amerika Serikat dan merupakan divisi dari Joint Commission Resources (JCR) cabang dari The Joint Commission. Fokus dan akreditasi JCI adalah keselamatan pasien (Pantient safety yang tertuang dalam standar 6 sasaran keselamatan pasien (Pambudi dkk, 2018).

The Staff Qualifications and Education (SQE) is a chapter in the Joint Commission International (JCI) Accreditation (Fifth Edition)[i] which describes the standards of Medical Staff & Staffing Management Standards in DET NORSKE VERITAS (DNV) Standard, Interpretive Guidelines and Surveyor Guidance for Hospitals, May 2013, Version 3.0.[ii] JCI Accreditation is a USA based international healthcare accrediting organization, whereas DNV is a Norway based international accreditation organization. The DNV International Accreditation Standard for Hospitals  are based on the National Integrated Accreditation for Healthcare Organizations (NIAHO) standard platform developed in the United States (US), and has been adopted by hospitals around the world as a new standard of excellence. These standards integrate clinical and patient safety requirements with proven quality principles into one seamless program. It’s a highly developed management system uniquely formulated to promote excellence within hospitals. However, both these standards are accredited by Ireland based International Society for Quality in Health Care (ISQua), which is the only accrediting organization who “accredit the accreditors' in the world.Methods: This is a comparison study (normative comparison) in which the researcher has critically analyzed and compared the Staff Qualifications and Education (SQE) standards in the Joint Commission International (JCI) Accreditation (Fifth Edition) and Medical Staff & Staffing Management Standards in DNV Standards for Hospitals, May 2013, Version 3.0.Data Collection: Primary data are collected from the JCI Accreditation Standards for hospitals, fifth edition, and DNV Standards for hospitals, Version 3.0. Secondary data are collected from relevant published journals, articles, research papers, academic literature and web portals.Objectives of the Study: The aim of this study is to analyze critically SQE Standards in JCI Accreditation and Medical Staff & Staffing Management Standards in DNV Standards to point out the best among both these international standards.Conclusion: This critical analysis of SQE, MS and SM standards in JCI and DNV Accreditation for hospitals clearly shows that the SQE Standards in JCI  Accreditation are very comprehensive, to the point and are much better than the DNV Accreditation.
