A large primary cardiac glomus tumour: a surgical dilemma

The human respiratory tract has a complex airflow pattern. If any obstruction is present in the airways, it will change the airflow pattern and deposit particles inside the airways. This is the concern of breath quality (inspired air), and it is decreasing due to the unplanned production of material goods. This is a primary cause of respiratory illness (asthma, cancer, etc.). Therefore, it is important to identify the flow characteristics in the human airways and airways with a glomus tumour with particle deposition. A numerical diagnosis is presented with an asymmetric unsteady-state light breathing condition (10 l/min). An in vitro human respiratory tract model has been reconstructed using computed tomography scan techniques and an artificial glomus tumour developed 2 cm above a carina on the posterior wall of the trachea. The transient flow characteristics are numerically simulated with a realizable (low Reynolds number) k–ɛ turbulence model. The flow disturbance is captured around the tumour, which influenced the upstream and downstream of the flow. The flow velocity pattern, wall shear stress and probable area of inflammation (hotspot) due to suspended particle deposition are determined, which may assist doctors more effectively in aerosol therapy and prosthetics of human airways illness.

The requirements on gratings and coatings for astronomical use differ from the general industrial requirements primarily in the scale of the components to be fabricated. Telescopes have large primary mirrors which require large coating plants to handle the components. Dispersive elements are driven by the requirement to be efficient in the presence of large working apertures, and usually optimize to large size in order to efficiently use the incoming radiation. Beyond this, there is a “new” technology of direct electronic sensors that places specific limits upon the image scale that can be used at the output of a telescope system, whether direct imagery or spectrally divided imagery is to be examined. This paper will examine the state of the art in these areas and suggest some actions and decisions that will be required in order to apply current technology to the predicted range of large new telescopes.

Glomus tumour, also referred to as Barré–Masson syndrome is an enigmatic, rare, painful tumour that is that represents a proliferation of the normal capsular-neural glomus apparatus. These are rare hamartomas that arise from the traditional glomus apparatus, located in subcutaneous tissue These are benign soft tissue neoplasms presenting usually within the second to fourth decade of life, originating from the glomus body. It accounts for 1-5% of all upper limb soft tissue tumors . It's a component of the dermis layer of the [1] skin, involved in thermoregulation. It structurally consists of an arterio-venous shunt which is surrounded by a connective tissue capsule and is found in increased amounts in the ngers and toes.

Purpose:Glomus tumours are characteristically benign solitary tumours. At our knowledge, about 23 reports are present in literature regarding the malignant counterpart, but only a minority developed metastases. We describe a locally aggressive glomus tumour with lymphnode metastasis.Patient:The patient was a 40 year-old man presenting a 1.5-cm lesion on the right wrist incompletely excised and a recurrent tumour, 4 × 2 cm in size, removed after 9 months, for which he received radiotherapy. After 2 years he developed an axillary lymphnode metastasis.Results:Histologically, both tumours (primary and metastasis) were similar. There were sheets and nests of uniform small cells with scant eosinophilic cytoplasm and round to polygonal nuclei; there was some degree of pleomorphism and the mitotic index was high (up to 18 m/10 HPF). The tumour cells were positive for vimentin and smooth muscle actin, but negative for desmin, NSE, Factor VIII, chromogranin, cytokeratin. Remarkably, in the primary, the cells strongly expressed p53 (70%) and MIB-1 (35%).Discussions:In many reported malignant cases, the histology of the tumour cells suggested that they were malignant, yet the clinical course has been benign. Carefully reviewing the literature, it seems that actually we have enough histological criteria to identify the cases with biological adverse outcome. Those unfortunate cases behave as high grade sarcomas and therefore may deserve an aggressive therapeutic treatment.
