ABSTRACTObjective: Caffeine consumption by adolescent population has increased dramatically over the last decade through increased coffee consumption andenergy drinks. However, caffeine causes many adverse effects, which demands people’s attention. The purpose of this study was to determine the mostpopular caffeinated product among adolescents and the knowledge and belief of adolescents about caffeine in their day to day life.Methods: An online survey was created and about 260 responses were obtained. Only 248 were counted as the remaining was removed by exclusioncriteria.Results: The most popular caffeine intake by adolescents was coffee. Students ingested caffeine at levels that could cause negative health effects andseemed unaware of the total amount of caffeine consumed.Conclusion: More information about caffeine should be incorporated into health education at all levels, so students can identify and avoid negativeeffects along with caffeine withdrawal and addiction.Keywords: Caffeine, Coffee, Adolescents, Addiction, Habits.

Introduction: A variety of research studies have shown that caffeine usage is highly prevalent among university students and many of them have different perception of its effect on their mental and physical health. These perceptions can influence the intake of caffeine which may lead to its addiction, resulting in serious public health consequences. Aims & Objectives: The objective of the study is to determine the frequency of caffeine consumption and the perception of its effects among university students. Place and duration of study: This study was conducted within 6 months duration, from May 2018 to October 2019 among students of three private universities in Lahore, Pakistan. Material & Methods: This was a descriptive cross-sectional study. A sample size of 670 university students was taken and the data were collected from three private universities of Lahore through convenience sampling technique. The perception of effects of caffeine was determined through a pre-validated questionnaire, Caffeine Expectancy Questionnaire (CaffEQ) from consumers and nonconsumers of caffeine. Results: A total of 670 university students participated in the study, out of which, 278 (41.5%) were males and 392 (58.5%) were females. 506 (75.5%) participants were consumer of caffeine, while 164 (24.5%) were non consumer. The occurrence of caffeine consumption was more in males (79.9%) than in females (72.4%). Participants reported that they consume caffeine because it makes them feel more alert (83.4%), they experience caffeine withdrawal without caffeine (83%) and caffeine makes them feel more energetic (80.6%). The perceptions due to which participants don't consume caffeine were that they expect caffeine makes their heartbeat irregular (81.7%), they don't like the way caffeine makes them feel (80.5%) and caffeine makes them irritable (76.8%). Conclusion: The study concludes that male students had more tendencies towards caffeine consumption than females. Perceptions for consuming caffeine were feeling of alertness, experiencing caffeine withdrawal symptoms, feeling of energy and ability to work over long periods of time after having caffeine. The perceptions for not consuming caffeine were irregular heartbeat, bad feelings, irritability, and sleep disturbances.

Most previous studies of the neurophysiological effects of caffeine have focused on the effects of caffeine ingestion, and few studies have examined the effects of caffeine withdrawal. This open study evaluated the quantitative EEG (QEEG) changes occurring during a 4-day period of abstinence in subjects who habitually consume 300 mg or more of caffeine daily. Thirteen subjects underwent QEEG studies during their usual caffeine consumption (baseline) and on days 1,2, and 4 of a 4-day period of caffeine abstinence. Ten of the subjects underwent a second QEEG on day 4 that consisted of a period of recording after reinstitution of caffeine. A comprehensive analysis of multiple quantitative variables was performed for each study during the abstinence period and compared to the variables obtained at baseline for each subject. Changes occurring during caffeine abstinence included: 1) increases in theta absolute power over all cortical areas, 2) increases in delta absolute power over the frontal cortex, 3) decreases in the mean frequency of both the alpha and beta rhythm, 4) increase in theta relative power and decrease in beta relative power, and 5) significant changes in interhemispheric coherence. Most of these changes tended to return to pre-abstinence baseline levels rapidly after resumption of caffeine consumption. The caffeine withdrawal state affects a number of neurophysiological variables. Further investigation of the neurophysiological aspects of caffeine withdrawal using placebo controlled double blind assessment methods is warranted.

Caffeine consumption may cause headache, particularly migraine. Its withdrawal also produces headaches and may be related to weekend migraine attacks. Transcranial Doppler sonography (TCD) has shown changes in cerebral blood flow velocities, (BFV) during and between attacks of migraine. In order to examine whether headache and changes in BFV could develop from controlled caffeine alterations, 20 healthy volunteers, without a headache history, underwent clinical evaluation, TCD and serum caffeine measurements on four occasions, comparing conditions of regular caffeine intake, caffeine withdrawal and “re-caffeination”. After 24 h of complete caffeine abstinence, 10 suffered from moderate to severe headaches with complete recovery within 1 h after caffeine intake. The BFVs in both middle cerebral, both posterior cerebral and basilar arteries were higher following the withdrawal period, reaching statistical significance in the left middle cerebral, basilar and both posterior cerebral arteries. BFVs decreased significantly within half an hour after caffeine intake in all subjects, and were similar to baseline values after 2 h. Our results emphasize the relationship between caffeine withdrawal, the development of headache and alterations in cerebral blood flow velocities. Also, these findings indicate that accurate interpretation of TCD measurements should account for the influence of caffeine on BFVs.

Caffeine-related health incidents in New Zealand have escalated over the last two decades. In order to reduce the risk of substance-related harm, it is important to understand the consumers’ motivations for its use. This is especially true for tertiary students who are presumed to be at a higher risk due to seeking out caffeine’s well-known cognitive benefits as well as the targeted marketing of such products to young adults. This study examined the habits and motivations for caffeine consumption in tertiary students in New Zealand. A previously validated caffeine consumption-habits (CaffCo) questionnaire was administered online to 317 tertiary students (n = 169 females), aged ≥16 years. Of the 99.1% of participants who regularly consumed caffeine, coffee (76.3%) tea (71.6%) and chocolate (81.7%) consumption were the most prevalent. Motivations for caffeinated-product consumption differed according to caffeine source. Tea was consumed for the warmth and taste, coffee was consumed to stay awake and for warmth, and chocolate, for the taste and as a treat. Marketing was not identified by participants as influencing their consumption of caffeinated products. Knowledge of motivations for caffeine consumption may assist in identifying strategies to reduce caffeine intake in those New Zealand tertiary students who regularly consume amounts of caffeine that exceed safe level.

Adverse effects associated with excessive caffeine consumption combined with increasing numbers and availability of caffeine-containing products are causes for concern. Tertiary students may be at increased risk of consuming excessive amounts of caffeine due to seeking caffeinated products with well-known wakefulness effects and cognitive benefits. This study explored caffeine consumption habits of New Zealand tertiary students (317; ≥16-years) using a previously validated caffeine consumption habits (CaffCo) questionnaire. Most (99.1%) regularly consumed caffeinated products, especially chocolate, coffee and tea, with coffee, tea and energy drinks contributing most to total caffeine intake. Median estimated caffeine intake was 146.73 mg·day−1, or 2.25 mg·kgbw−1·day−1. Maximum and minimum intakes were 1988.14 mg·day−1 (23.51 mg·kgbw−1·day−1) and 0.07 mg·day−1 (0.02 mg·kgbw−1·day−1), respectively. One-third (34.4%) of caffeine consumers ingested caffeine above the adverse effect level (3 mg·kgbw−1·day−1) and 14.3% above the safe limit (400 mg·day−1). Most caffeine consumers (84.7%), reported experiencing at least one ‘adverse symptom’ post-caffeine consumption, of which 25.7% reported effects leading to distress or negatively impacting their life. Experiencing ‘adverse symptoms’ did not, however, curtail consumption in the majority of symptomatic participants (~77%). Public health initiatives directed at tertiary students may be important to reduce potential caffeine-related harm.
