Viral infection is known to induce transient autoimmunity in humans. Acute cytomegalovirus (CMV) infection is implicated in occasional thrombosis formation. We here, for the first time, report a 19-year-old female who had an acute CMV infection, leading to a deep venous thrombosis and a pulmonary embolism along with transient appearance of lupus anticoagulant. The pathological role of antiphospholipid antibodies in CMV-mediated thrombosis is discussed.

Background: acute Cytomegalovirus (CMV) infection in the immunocompetent usually asymptomatic or may present as a mononucleosis syndrome, but sometimes it is present as colitis, meningitis, encephalitis, or as ocular involvement and others. Thrombosis is considered an extremely rare manifestation described only in a scarce number of case reports.   Case presentation: a 30-years-old immunocompetent patient present with extensive vascular thrombosis shortly after a diagnosis of CMV infectious mononucleosis was established. Other than morbid obesity, no other risk factors existed. Therapeutic anticoagulation was associated with clinical improvement.   Conclusion: CMV infection can be associated with acute venous thrombosis in immunocompetent patients. An active search for both underlying prothrombotic conditions and other organ involvement sounds reasonable and should be treated, if found, accordingly.

Cytomegalovirus infections are widely distributed with a seroprevalence of up to 100%. The majority of the cases take a silent course or deal with unspecific clinical symptoms. Complications in immunocompetent patients are rare but may affect the liver and lead up to an acute organ failure. In this case report, we describe a 35-year-old immunocompetent female with an acute cytomegalovirus infection presenting as acute hepatitis with ongoing upper right abdominal pain after cholecystectomy. Upper right abdominal pain is a common symptom with a wide range of differential diagnoses. If common reasons can be excluded, we want to sensitize for cytomegalovirus infection as a minor differential diagnosis even in immunocompetent patients.
