AbstractObjective:To summarise the applications and appropriate use of Dietary Reference Intakes (DRIs) as guidance for nutrition and health research professionals in the dietary assessment of groups and individuals.Design:Key points from the Institute of Medicine report, Dietary Reference Intakes: Applications in Dietary Assessment, are summarised in this paper. The different approaches for using DRIs to evaluate the intakes of groups vs. the intakes of individuals are highlighted.Results:Each of the new DRIs is defined and its role in the dietary assessment of groups and individuals is described. Two methods of group assessment and a new method for quantitative assessment of individuals are described. Illustrations are provided on appropriate use of the Estimated Average Requirement (EAR), the Adequate Intake (AI) and the Tolerable Upper Intake Level (UL) in dietary assessment.Conclusions:Dietary assessment of groups or individuals must be based on estimates of usual (long-term) intake. The EAR is the appropriate DRI to use in assessing groups and individuals. The AI is of limited value in assessing nutrient adequacy, and cannot be used to assess the prevalence of inadequacy. The UL is the appropriate DRI to use in assessing the proportion of a group at risk of adverse health effects. It is inappropriate to use the Recommended Dietary Allowance (RDA) or a group mean intake to assess the nutrient adequacy of groups.

Dietary Reference Intakes (DRIs) are used for assessing and planning diets of individuals and groups. Assessing individual intakes is complicated by the fact that neither the individual's usual nutrient intake nor their individual requirement is known. However, the degree of confidence that intakes are adequate or excessive can be estimated. Assessing diets of groups requires information on the group's usual nutrient intake distribution, which can be obtained by statistically adjusting 1 d intake distributions to remove within-person variability. For most nutrients with an Estimated Average Requirement (EAR), the group prevalence of inadequate intakes can be approximated by the percent whose usual intakes are less than the EAR. However, the prevalence of inadequacy cannot be determined for nutrients with an Adequate Intake (AI). The goals of planning are a low risk (for individuals) or low prevalence (for groups) of inadequate or excessive nutrient intakes. For individuals, these goals are met by planning intakes that meet the Recommended Dietary Allowance (RDA) or AI, are below the Tolerable Upper Intake Level (UL), and fall within the Acceptable Macronutrient Distribution Ranges (AMDRs). For groups, planning involves estimating a "target" usual intake distribution with an acceptably low prevalence less than the EAR and greater than the UL, planning menus to achieve the target distribution, and assessing the results.Key words: nutrition assessment, dietary planning, nutrient adequacy.

As avaliações de dietas e o planejamento de consumo são atividades tradicionalmente realizadas por meio da comparação de médias de ingestão contra valores de referência de energia e nutrientes, seja para indivíduos ou grupos. Limitações de ordem técnica devem ser levadas em conta, sem as quais se pode chegar a conclusões equivocadas quanto ao atendimento das necessidades nutricionais. As Recomendações Nutricionais propostas pelo Institute of Medicine dos Estados Unidos, em conjunto com a agência Health Canada, a partir de 1997, conhecidas como Dietary Reference Intakes, representam um novo paradigma para o estabelecimento de indicadores nutricionais de consumo, ao aperfeiçoarem o uso do conceito de risco na avaliação de dietas. Fontes de erro intra ou interindividuais, devidas à variabilidade de padrão de consumo e decorrentes da distribuição das necessidades na população, aliadas a um pequeno número de dias de observação, têm grande impacto sobre a confiabilidade da análise. Por esta razão devem orientar a utilização dos valores, que foram organizados em tabelas com as quatro categorias de nutrientes, publicadas entre 1997 e 2005. O presente trabalho teve por objetivo destacar algumas características de aplicação e consolidar os valores diários de Tolerable Upper Intake Level, Adequate Intake e Recommended Dietary Allowance, facilitando a consulta por parte de profissionais e estudantes da área de nutrição.

Prevalence of micronutrient malnutrition is high in individuals living in long-term care (LTC) homes with many individuals consuming low levels of vitamins B6, D, and E; folate; calcium; magnesium; and zinc. The focus of this research was to identify strategies and challenges encountered during development of micronutrient-dense menus for use in Ontario LTC homes and to examine costs associated with development of a menu with acceptable micronutrients. Semi-structured open-ended interviews were conducted with 13 menu planners (7 dietitians, 6 nutrition managers) in diverse LTC homes in Ontario. Data were thematically analyzed. A 7-day hypothetical menu meeting all nutrient requirements was developed and costed. Analysis of the interview data showed that menus are planned according to the Canada’s Food Guide (2007) and focus placed on Dietary Reference Intakes of protein, fibre, calcium, and sodium. Little focus is placed on micronutrients. Flexibility in foods offered was important to accommodate the small volume of food consumed. Resident preferences were balanced against nutritional requirements. Challenges included planning for diverse populations, managing portion sizes, and balancing the budget. A hypothetical menu planned to contain adequate levels of all micronutrients is 49% higher in food costs than the amount currently provided to Ontario LTC homes.

ABSTRACT Introduction Recently, there has been increasing concern about the adverse health effects of long-term occupational exposure to low-level blast in military personnel. Occupational blast exposure occurs routinely in garrison through use of armaments and controlled blast detonations. In the current study, we focused on a population of breaching instructors and range staff. Breaching is a tactical technique that is used to gain entry into closed spaces, often through the use of explosives. Materials and Methods Initial measurements of blast overpressure collected during breaching courses found that up to 10% of the blasts for range staff and up to 32% of the blasts for instructors exceeded the recommended 3 psi exposure limit. Using a cross-sectional design, we used tests of balance, ataxia, and hearing to compare a sample of breachers (n = 19) to age-and sex-matched military controls (n = 19). Results There were no significant differences between the two groups on the balance and ataxia tests, although the average scores of both groups were lower than would be expected in a normative population. The prevalence of hearing loss was low in the breacher group (4 of 19), and hearing thresholds were not significantly different from the controls. However, the prevalence of self-reported tinnitus was significantly higher in the breacher group (12 of 19) compared with the controls (4 of 19), and all breachers who were identified as having hearing loss also reported tinnitus. Conclusions Our results suggest that basic tests of balance, ataxia, and hearing on their own were not sensitive to the effects of long-term occupational exposure to low-level blast. Some of the blast exposure levels exceeded limits, and there was a significant association of exposure with tinnitus. Future studies should supplement with additional information including exposure history and functional hearing assessments. These findings should be considered in the design of future acute and longitudinal studies of low-level blast exposure.

In the field of nanotechnology, the use of multi-walled carbon nanotubes (MWCNTs) is growing. Pulmonary exposure during their production, use, and handling is raising concerns about their potential adverse health effects. The purpose of this study is to assess how the physical characteristics of MWCNTs, such as diameter and/or length, can play a role in cellular toxicity. Our experimental design is based on the treatment of human bronchial epithelial cells (BEAS-2B) for six weeks with low concentrations (0.125–1 µg/cm2) of MWCNTs having opposite characteristics: NM-403 and Mitsui-7. Following treatment with both MWCNTs, we observed an increase in mitotic abnormalities and micronucleus-positive cells. The cytotoxic effect was delayed in cells treated with NM-403 compared to Mitsui-7. After 4–6 weeks of treatment, a clear cellular morphological change from epithelial to fibroblast-like phenotype was noted, together with a change in the cell population composition. BEAS-2B cells underwent a conversion from the epithelial to mesenchymal state as we observed a decrease in the epithelial marker E-cadherin and an increased expression of mesenchymal markers N-cadherin, Vimentin, and Fibronectin. After four weeks of recovery, we showed that the induced epithelial-mesenchymal transition is reversible, and that the degree of reversibility depends on the MWCNT.

The implications of long-term high calcium (Ca) intake are well documented in growing dogs and in adult dogs of large breed size, however, the consequences on other breeds and breed sizes are yet to be determined. Eighteen neutered adult beagles, nine males and nine females aged 1.4–4.4 years, were randomized to control or test diets providing in g∙4184 kJ−1 (1000 kcal−1): 1.44 and 7.19 total Ca balanced with 1.05 and 4.25 total phosphorus, respectively, for 40 weeks. Health parameters, ultrasound scans, radiographs, glomerular filtration rate, and mineral balance were measured at eight-week intervals. All dogs remained healthy with no measured evidence of orthopedic, urinary, or renal disease. The test diet resulted in a 5.2 fold increase in fecal Ca excretion. Apparent Ca digestibility (%) and Ca balance (g/d) did not significantly (p > 0.05) change from baseline in the test diet group, although dogs displayed a positive Ca balance (maximum at week 8, 1.11 g/d with 95% CI (0.41, 1.80)) before a neutral Ca balance was restored at week 32. Despite an initial positive Ca balance, we can conclude that no measurable adverse health effects were observed as a result of the test diet fed in this study in beagles over a period of 40 weeks.

Key Points BDR is a chemotherapy-free, non-stem-cell–toxic regimen associated with high response rates and long-term remissions. The long-term safety profile of BDR is favorable, with high probability of response to reintroduction of rituximab-based regimens at relapse.
