Die laparoskopische Antirefluxchirurgie garantiert bessere Kurz- und Langzeitresultate als die offenen Verfahren und gilt heute als Standardtechnik des Refluxleidens. Chirurgische Antirefluxplastiken sind nach optimaler präoperativer Abklärung mittels Gastroskopie, Biopsie sowie 24h-pH-Manometrie bei Versagen einer korrekten medikamentösen Langzeittherapie, bei Vorliegen von Komplikationen der Refluxkrankheit, bei fehlender Compliance und bei atypischen Symptomen (wie chronischer Husten, Asthma, etc.) indiziert. Der Eingriff umfasst die laparoskopische Magenreposition, die dorsale Crurorrhaphie und die Bildung einer lockeren schmalen Fundusmanschette von 360° (Nissen-Rossetti), resp. 270° (Toupet); letztere bei einer Dysmotilität des Ösophagus (tailored procedure). Die Resultate der Literatur und eigene Erfahrungen werden diskutiert.
