The use of miniscrews for orthodontic anchorage combined to periodontal treatment in aggressive periodontitis: case report and review of the literature. Introduction: Miniscrews (MSs) have gained rapid popularity among orthodontic specialists over the past decade. Subsequently, as general dental practitioners continue to provide general care for patients undergoing orthodontic treatment they are likely to encounter MSs. Observation: A 60-year-old woman concerned about generalized agressive periodontitis (generalized periodontitis stage-III) with complaints of diastema and spacing in the upper arch, received for ten years nonsurgical therapy via scaling and root planning combined with systemic antibiotics. Maxillary periodontal flaps achieved this primary periodontal treatment. As she complained of protrusion of her maxillary incisors she was referred for orthodontic treatment with palatinal MSs to retract and align the maxillary anterior segment. A 26 months follow-up showed esthetic and functional stability of the result. Discussion: This case report demonstrates efficacity of combined periodontal and orthodontic treatments using palatinal MSs in order to successfully contribute to treatment of stage-III periodontitis.

BACKGROUND: Aggressive periodontitis (AgP) is associated with an extensive and rapid destruction of periodontal tissues. Unpredictable treatment outcomes of the disease are consequences of various cross-linked factors. Antimicrobial adjunctive treatment is routinely used as a part of non-surgical periodontal treatment (NSPT) of AgP. AIM: Therefore, the aim of this study was to compare the effects of active periodontal treatment (APT) combined with systemic antibiotics with mechanical debridement alone, in patients with generalized AgP (GAgP). METHODS: Two groups, consisting of 10 participants each, were randomly formed. Both groups were clinically and microbiologically tested and monitored for 3 months. Test group received APT with antimicrobial supplementation (amoxicillin and metronidazole [AMX-MET]). Patients from the control group were treated by APT only. Clinical and microbiological parameters were recorded at baseline and 3 months following the NSPT. Clinical measurements involved probing depth, clinical attachment level, bleeding on probing, and plaque index. Samples of subgingival crevicular fluid were analyzed by qualitative polymerase chain reaction. RESULTS: Significant improvement of all clinical parameters was observed 3 months following the NSPT (p < 0.05), but significant difference between groups was not found (p > 0.05). Periodontal pathogens’ detection between baseline and 3 months follow-up was also not statistically significant in both examined groups (p > 0.05). CONCLUSION: Similarity of results obtained in both treatment groups underlined the key role of APT in the treatment of AgP. The AMX-MET supplementation did not improve clinical and microbiological outcomes, when compared to APT alone. Longer follow-up period, with larger sample, could provide a more comprehensive insight into this issue.

ABSTRACT Hypodontia is the most prevalent craniofacial malformation in mankind. It may present a wide variety of manifestations and, depending on the number and location of missing teeth, it may affect the esthetics, mastication, speech and occlusal balance. This paper discusses the therapeutic approaches to solve this condition, describing a case report with hypodontia of one mandibular lateral incisor, which treatment option included space closure at the region of hypodontia associated with composite resin restorations in the mandibular central incisors. The three-year follow-up after treatment revealed occlusal stability, adequate intercuspation in Class I relationship and excellent micro and macroesthetics.

Transmigrated mandibular canines increase treatment complexity in terms of both anchorage and biomechanical planning, posing challenges for both Orthodontists and Oral Surgeons. This case report presents the orthodontic treatment in a 12-year-old girl patient, with transmigrated and impacted mandibular right canine positioned horizontally below the apices of the mandibular incisors. The mandibular deciduous right canine was extracted followed by the traction of the transmigrated permanent canine. Eruption was properly guided, and the correct position of the tooth was orthodontically guided into its normal position in the arch. The patient’s occlusion was significantly improved with good aesthetic outcome, functional occlusion, and a stable result after 1.5 years of follow-up.
