Speech Perception by Four Single-Channel Cochlear Implant Users

Four profoundly deaf adults, each a recent recipient of a scala tympani implant, underwent auditory and auditory-visual speech comprehension evaluations. Two subjects had multiple-electrode auditory prostheses, and 2 had single-electrode implants. All subjects were tested preoperatively with a high-power hearing aid, and postoperatively with a single-channel wearable sound processor. Reported here are the results of the first formal speech recognition tests which were conducted during the 8 months after the sound processor fitting. Three subjects had used the single-channel processor on a daily basis for up to 8 months at the time of postoperative testing. The 4th subject was a nonuser. On listening tests, a comparison between pre- and post-implant scores revealed little difference for any subject. On postoperative speechreading tasks, all subjects identified medial consonant phonemes and 2-digit numerals better with stimulation than without. The 3 frequent users of the device experienced significant improvement on connected-discourse tracking, and their speechreading of videotaped and live voice CID Everyday Sentences (Davis & Silverman, 1978) was enhanced with the addition of stimulation. The nonuser was a very proficient speechreader at the outset and exhibited no significant difference on connected-discourse tracking with and without stimulation. Moreover her ability to speechread Everyday Sentences was hampered slightly by the addition of stimulation. This single-channel sound processor functioned as a sensory supplement for the 3 frequent users, but no subject was able to use the processor as a sensory substitute.

The insertion of an intrascalar electrode array during cochlear implantation causes immediate damage to the inner ear and may result in delayed onset of additional damage that may interfere with neuronal stimulation. To date, there have been reports on fewer than 50 temporal bone specimens from patients who had undergone implantation during life. The majority of these were single-channel implants, whereas the majority of implants inserted today are multichannel systems. This report presents the histopathologic findings in temporal bones from 8 individuals who in life had undergone multichannel cochlear implantation, with particular attention to the type and location of trauma and to long-term changes within the cochlea. The effect of these changes on spiral ganglion cell counts and the correlation between speech comprehension and spiral ganglion cell counts were calculated. In 4 of the 8 cases, the opposite, unimplanted ear was available for comparison. In 3 of the 4 cases, there was no significant difference between the spiral ganglion cell counts on the implanted and unimplanted sides. In addition, in this series of 8 cases, there was an apparent negative correlation between residual spiral ganglion cell count and hearing performance during life as measured by single-syllable word recognition. This finding suggests that abnormalities in the central auditory pathways are at least as important as spiral ganglion cell loss in limiting the performance of implant users.

This study evaluated 20 deaf adults' ability to understand connected discourse under two conditions: (a) lipreading alone (LA), and (b) lipreading plus electrical stimulation by a single-channel cochlear implant (LI). Performance was evaluated using the speech-tracking procedure, which required the deaf receiver to repeat verbatim textual material read by another individual. The reception of ongoing speech was measured in terms of the number of words repeated correctly per minute. The results revealed, on the average, a significantly higher tracking rate in the LI condition (28.6 wpm) than the LA condition (16.4 wpm). There was a significant learning effect across sessions for both conditions. Subjects who were previous hearing aid users achieved a significantly higher tracking rate in the LI condition than did subjects who had no previous experience with amplification. Significant subject differences were present as a function of condition. Sixteen of the 20 subjects achieved higher tracking rates in the LI than in the LA condition. However, 4 of these 16 subjects showed similar relative gains in both conditions and 3 showed a decrease in performance in the LA condition over time. The remaining 4 subjects showed nearly equivalent tracking rates in both conditions over time. The data suggest that the majority of subjects were able to learn to make use of the cues provided by electrical stimulation by a single-channel cochlear implant to enhance their lipreading skills, at least for highly redundant material in a communicative situation.

Purpose The aims of this study were to characterize hearing symptoms, habits, and sound pressure levels (SPLs) of personal audio system (PAS) used by young adults; estimate the risk of developing hearing loss and assess whether instructions given to users led to behavioral changes; and propose recommendations for PAS users. Method A cross-sectional study was performed in 50 subjects with normal hearing. Procedures included questionnaire and measurement of PAS SPLs (real ear and manikin) through the users' own headphones and devices while they listened to four songs. After 1 year, 30 subjects answered questions about their usage habits. For the statistical analysis, one-way analysis of variance, Tukey's post hoc test, Lin and Spearman coefficients, the chi-square test, and logistic regression were used. Results Most subjects listened to music every day, usually in noisy environments. Sixty percent of the subjects reported hearing symptoms after using a PAS. Substantial variability in the equivalent music listening level (Leq) was noted ( M = 84.7 dBA; min = 65.1 dBA, max = 97.5 dBA). A significant difference was found only in the 4-kHz band when comparing the real-ear and manikin techniques. Based on the Leq, 38% of the individuals exceeded the maximum daily time allowance. Comparison of the subjects according to the maximum allowed daily exposure time revealed a higher number of hearing complaints from people with greater exposure. After 1 year, 43% of the subjects reduced their usage time, and 70% reduced the volume. A volume not exceeding 80% was recommended, and at this volume, the maximum usage time should be 160 min. Conclusions The habit of listening to music at high intensities on a daily basis seems to cause hearing symptoms, even in individuals with normal hearing. The real-ear and manikin techniques produced similar results. Providing instructions on this topic combined with measuring PAS SPLs may be an appropriate strategy for raising the awareness of people who are at risk. A tongue pressure unit for measurement of lingual strength and patterns of tongue pressure is described. It consists of a force displacement transducer, a single channel, direct writing recording system, and a specially designed tongue pressure disk, head stabilizer, and pressure unit holder. Calibration with known weights indicated an essentially linear and consistent response. An evaluation of subject reliability in which 17 young adults were tested on two occasions revealed no significant difference in maximum pressure exerted during the two test trials. Suggestions for clinical and research use of the instrumentation are noted.

Microorganisms that have been identified in dental unit waterlines (DUWLs) are of concern because they can cause infections, especially in immunocompromised patients. This study aimed to assess the incidence of microbial contamination in DUWLs before and after intervention to reduce contamination, and to investigate the presence of coliforms, Escherichia coli and Pseudomonas aeruginosa. Water samples were collected aseptically from the waterlines. The high-speed hand-piece and dental chair units were served by one distillation apparatus, which was fed by the potable tap water of four dental clinics. Different interventions were used: chlorination, flushing before clinics and between patients, draining at the end of the day, and freshly distilled water on a daily basis. There was a significant difference between the level of contamination in the high-speed hand-piece (1.5–2.7 log CFU/ml) and dental chair unit water (2.0–3.5 log CFU/ml). Coliforms (0.9%) E. coli (0.9%) and Pseudomonas (1.8%) were detected during 2008. This study indicates the need to monitor water quality regularly and prevent stagnation in DUWLs to reduce the number of viable bacteria to <100 CFU/ml. We recommend flushing the DUWL for 2 min before the first patient and for 10–20 s between patients, flushing the dental unit at the end of the day and draining it overnight to reduce the development of biofilms, and chlorination of the DUWLs.

Abstract Objective: The aim of this study was to investigate the dietary patterns and sedentary behaviors among university students in Bahrain. Materials and methods: A cross-sectional study was carried out with students of the College of Health Sciences in Bahrain using a self-reported questionnaire. All the students enrolled in this college were included in this study (642 students; 90 males and 552 females). The mean age of the sample was 20.1±2.0 years. A pre-tested questionnaire was used to collect information on the students’ breakfast intake, snacking, food frequency intake, and sedentary habits. Results: More than 50% of the students did not consume breakfast on a daily basis. A statistically significant difference (p<0.003) was found between males (19%) and females (35%) in relation to the intake of an afternoon snack. There were no significant gender differences regarding frequency of food intake, except for carbonated beverages (p<0.001) and nuts (p<0.047) consumption. Females were more likely than males to spend time watching television, use the Internet, use a mobile phone and sleep; however, the only significant difference found was for Internet use (p<0.003). Being physically active on a daily basis was more prevalent among males (41.6%) than females (16%) (p<0.001). Females (42.9%) were more prone to sleep for less time (<7 h) than males (34.4%)(p<0.08). Conclusion: The findings indicated that a high percentage of health science students in Bahrain had unhealthy dietary habits and sedentary behaviors. Thus, an intervention program to promote healthy dietary patterns and lifestyle habits among university students is highly recommended.

In today’s generation, mathematics will be very useful in our daily lives. Using mathematics is a tool when meeting life-related circumstances involving computation; hence mathematics is particularly important on a daily basis. This research study is entitled “Mindset and Level of Anxiety in General Mathematics among Grade 11 students”. The study focused on the association between the mindset of the students if it is fixed or growth and their anxiety levels in General Mathematics. This research study is quantitative research; it is using a descriptive-correlation design. This study was answered by all Grade 11 students who are taking General Mathematics in the first semester in Jagobiao National High School. In gathering the data, the checklist survey questionnaire was being used. The result found out that students of Jagobiao National High School are more on growth mindset. Students with a growth mindset lead to a moderate level of anxiety in General Mathematics. By using the chi-square, it tells that mindset and level of anxiety has a significant difference. School heads, faculties, and staff can also have monthly activities to diminish the anxiety level of the students. They could also motivate or cheer up students to become growth-minded.

This study was designed to standardize the measurement of visual speech comprehension in order to facilitate the evaluation of a patient’s communication handicap, his basic capacity for lipreading and the benefit received through lipreading training as a clinical tool. To this end, a silent color film of four minutes' duration was used to obtain a standardized measurement of the lipreading ability of 1108 hearing-impaired subjects. The film depicts an everyday situation in which two persons, while having coffee together, speak nine sentences of varying difficulty. Item analysis reveals good consistency, and the score obtained by the individual patient corresponds well with the clinical evaluation of his lipreading ability.

Objective. To measure the audiological benefit of the Baha SoundArc, a recently introduced nonimplantable wearing option for bone conduction sound processor, and to compare it with the known softband wearing option in subjects with normal cochlear function and a purely conductive bilateral hearing loss.Methods. Both ears of 15 normal hearing subjects were occluded for the time of the measurement, yielding an average unaided threshold of 49 dB HL (0.5 – 4 kHz). Soundfield thresholds, speech understanding in quiet and in noise, and sound localization were measured in unaided conditions and with 1 or 2 Baha 5 sound processors mounted on either a softband or a SoundArc device.Results. Soundfield thresholds and speech reception thresholds were improved by 19.5 to 24.8 dB (p<.001), when compared to the unaided condition. Speech reception thresholds in noise were improved by 3.7 to 4.7 dB (p<.001). Using 2 sound processors rather than one improved speech understanding in noise for speech from the direction of the2nddevice and sound localization error by 23° to 28°. No statistically significant difference was found between the SoundArc and the softband wearing options in any of the tests.Conclusions. Bone conduction sound processor mounted on a SoundArc or on a softband resulted in considerable improvements in hearing and speech understanding in subjects with a simulated, purely conductive, and bilateral hearing loss. No significant difference between the 2 wearing options was found. Using 2 sound processors improves sound localization and speech understanding in noise in certain spatial settings.

A simulation model was developed for secondary tillage and sowing operations in autumn, using discrete event simulation technique in Arena<sup>®</sup> simulation software (Version 14). Eight machinery sets were evaluated on a 50-hectare farm. Total costs including fixed-costs, variable costs and timeliness costs were calculated for each machinery set. Timeliness costs were estimated for 21-years period on daily basis (Daily Work method) and compared with another method (Average Work method) based on the equation proposed by ASAE Standards, EP 496.3FEB2006. The Inputs of the model were machinery sets, field size, machines performances and daily soil workability state. The optimization criteria were the lowest costs and lowest standard deviation in daily work method plus the lowest costs based on average work method. The validity of the model was evaluated by comparing the output of the model with field observed data collected from various farms. Results revealed that there was no significant difference (P &gt; 0.01) between the observed and predicted finish day.
