Philip Maloney Lecture: The Role of Oral and Maxillofacial Surgery in the Trauma Care Center: Trauma Surgeons Perspective

Additive manufacturing technology or 3-dimensional printing has been used since ages in various fields including medical. Their addition to dentistry is recent and has tried to revolutionize the field. It is being used in various fields of dentistry like endodontics, prosthodontics, orthodontics, oral and maxillofacial surgery and recently in periodontics and Implantology. With introduction of recent “layer-by-layer” additive technology, their use in periodontal field has changed its treatment planning. It is use in preparation of customized scaffold with or without stem cell therapy, ridge augmentation, sinus lift and guided implant surgery, implant fixtures, education models, drug technology and many more. This review has tried to explain the various applications of the additive manufacturing in the field of periodontics with recent evidences. This paper highlights the role of 3-dimensional printing which can change the future of periodontal management.

The pathology of the oro-maxilo-facial territory comprises a vast chapter of diseases with primary or secondary infectious etiology, the mouth being the natural cavity of the organism with the most varied and variable flora of bacterial contamination. By the continuity solutions created by the mechanical act of mastication, decubitus of defective prosthetic works, local dento-periodontal infectious processes, but especially of dental or small-scale interventions, open gates and possibilities of penetration into the blood circulation of the microorganisms, which cause the transient bacteria. The study included 243 randomly selected patients, with the address of the Oral and Maxillofacial Surgery Clinic in the period 2016-2018. The inoculation of microorganisms in the oral cavity in circulating blood as a result of oral surgery or dental care procedures is much more common than in other areas of the body. Aggressive dental maneuvers that create solutions of continuity of oral mucosa, gingiva, periodontal surgery, periodontal surgery techniques can cause bacteria.

ABSTRACT Background: As the morbidity and mortality due to trauma are ever increasing, there is proportionally growing need of trauma care facilities across the country. In the context of expanding designated trauma care facilities, the role of plastic and reconstructive surgeon needs to be analysed and defined at least at a Level 1 trauma centre. Materials and Methods: We included the patients who were operated under the department of plastic, reconstructive & burns surgery at a Level 1 urban trauma centre between January 2016 and December 2017. We analysed the demographic data and categorised operative data according to anatomical areas and interacting specialties. Results: A total of 1539 procedures were performed under the division of plastic reconstructive and burn surgery. Amongst them, 81% were male, and 19% were female. Mean age was 27.3 years (range: 3–90 years). The anatomical locations treated were upper limb (49%), lower limb (35%), head and neck (8%) and trunk (8%). Interdepartmental cases were 600 and majority of them were in collaboration with orthopaedics (n = 298), general surgery (n = 163), neurosurgery (79) and maxillofacial surgery (60). Conclusion: There is a significant role of plastic surgeon at a Level 1 trauma centre in India. The plastic surgeon's interventions are limb saving and sometimes lifesaving, many at times morbidity of post-traumatic sequelae are either prevented or treated. Along with other core specialties involved in the management of trauma, plastic surgeons play an integral role in a Level 1 trauma centre. The policymakers should take note to augment the number of plastic surgeons at a Level 1 apex trauma centre on par with other specialties, as the workload is heavy and is steadily on an increasing trend.

Objective: During pregnancy, changes occur in the oral environment with gingivitis predominating. The development of odontogenic infections within the period of pregnancy may endanger the life of the mother as well as that of her unborn baby. Materials and methods: A retrospective observational study of cases of cervicofacial infection in women during pregnancy was conducted at the oral and maxillofacial surgery clinic of a northern Nigerian tertiary health care center from January 2006 to June 2018. Results: Seventy women were managed for cervicofacial infection during the period reviewed, out of which 20 women (28.6%) presented during pregnancy. Their mean age was 33.8 ± 9.35 years with a range of 20 to 55 years. The 30-39 years age bracket had the highest frequency (40%) and the mean duration of pregnancy at presentation was 24.9 ± 11.12 weeks with a range between 10 to 36 weeks. Majority (n=15, 75.0%) presented in the 3rd trimester. At presentation, the frequently involved fascial space was unilateral submandibular space (n=10; 50.0%), All the patients had incision/drainage/decompression on the dental chair under local anesthesia (2% lidocaine with 1:80,000 adrenaline). The mean length of hospital stay was 13.9 ± 6.2 days with a range of 6 to 26 days. The mortality rate was 15% (n=3 cases). Conclusion: There is a need for oral health evaluation in pregnant women during ante-natal visits to prevent these complications. Oral health education should also form part of teachings received by women both in the ante-natal and postnatal clinics.
