BACKGROUND With the development of the Internet, online medical community can help patient access to medical information and relevant decisions more conveniently, and meet the needs of patients for their own healthcare management. Mining these Q&A (Question and Answer) data, we can help doctors give more targeted feedback which improve the efficiency of question-and-answer, and patient satisfaction. OBJECTIVE This study aimed to (1) analysis frequency and position of diabetes related diseases or symptoms in Q&A website and (2) find out the differences of disease terms in gender and age using in the questions. METHODS We collected 5766 Q&A diabetes related data on the website of Chunyuyisheng from June 2012 to April 2020. In 38176 combined sentences, a vocabulary contains 3 categories of 3851 word and 2094 ICD (International Classification of Diseases) matching terms were obtained by calculating the similarity using word vectors. Proportion of the frequency of words and Mann-Whitney U test on word position were used to quantify the difference in patient’s gender and age group. RESULTS The vocabulary of the disease category accounts for 70%. We analyzed the word frequency and position in questions for different gender and age group. For gender, women participate in question answering more, accounting for 53% of total questions. They pay more attention to pregnancy, sleep and thyroid gland related vocabulary compared to men. Men focus more on circulation system, kidney failure related vocabulary. For different age group, pregnancy, glucose regulation, digestive and respiratory system related vocabulary have a higher proportion for patients under 40 years old. Patients over 40 years old pay more attention on kidney failure, cerebral ischaemia, infectious and circulation system. CONCLUSIONS This study provides a new insight into frequency and position of diabetes related diseases or symptoms in online medical services. It can show patients’ different attention by comparing disease or symptom categories for gender and age with ICD disease codes. The frequency and position of disease category words in patients’ conversation can be used for further risk evaluation for chronic diseases research.

Nowadays, cities are intended to change to a smart city. According to recent studies, the use of data from contributors and physical objects in many cities play a key element in the transformation towards a smart city. The ‘smart city’ standard is characterized by omnipresent computing resources for the observing and critical control of such city’s framework, healthcare management, environment, transportation, and utilities. Mist computing is considered a computing prototype that performs IoT applications at the edge of the network. To maintain the Quality of Service (QoS), it is impressive to employ context-aware computing as well as fog computing simultaneously. In this article, the author implements an optimization strategy applying a dynamic resource allocation method based upon genetic algorithm and reinforcement learning in combination with a load balancing procedure. The proposed model comprises four layers i.e. IoT layer, Mist layer, Fog layer, and Cloud layer. Authors have proposed a load balancing technique called M2F balancer which regulates the traffic in the network incessantly, accumulates the information about each server load, transfer the incoming query, and disseminate them among accessible servers equally using dynamic resources allocation method. To validate the efficacy of the proposed algorithm makespan, resource utilization, and the degree of imbalance (DOI) are considered as the scheduling parameter. The proposed method is being compared with the Least count, Round Robin, and Weighted Round Robin. In the end, the results demonstrate that the solutions enhance QoS in the mist assisted cloud environment concerning maximization resource utilization and minimizing the makespan. Therefore, M2FBalancer is an effective method to utilize the resources efficiently by ensuring uninterrupted service. Consequently, it improves performance even at peak times.

Abstract Background The coronavirus disease 2019 (COVID-19) pandemic has severely impacted the Italian healthcare system, underscoring a dramatic shortage of specialized doctors in many disciplines. The situation affected the activity of the residents in neurology, who were also offered the possibility of being formally hired before their training completion. Aims (1) To showcase examples of clinical and research activity of residents in neurology during the COVID-19 pandemic in Italy and (2) to illustrate the point of view of Italian residents in neurology about the possibility of being hired before the completion of their residency program. Results Real-life reports from several areas in Lombardia—one of the Italian regions more affected by COVID-19—show that residents in neurology gave an outstanding demonstration of generosity, collaboration, reliability, and adaptation to the changing environment, while continuing their clinical training and research activities. A very small minority of the residents participated in the dedicated selections for being hired before completion of their training program. The large majority of them prioritized their training over the option of earlier employment. Conclusions Italian residents in neurology generously contributed to the healthcare management of the COVID-19 pandemic in many ways, while remaining determined to pursue their training. Neurology is a rapidly evolving clinical field due to continuous diagnostic and therapeutic progress. Stakeholders need to listen to the strong message conveyed by our residents in neurology and endeavor to provide them with the most adequate training, to ensure high quality of care and excellence in research in the future.

MiRNAs have recently become a subject of great interest within cancers and especially colorectal cancers in diagnosis, prognosis, and therapy decisions; herein we review the current literature focusing on miRNAs in colorectal cancers, and we discuss future challenges to use this tool on a daily clinical basis. In liquid biopsies, miRNAs seem easily accessible and can give important information toward each step of the management of colorectal cancers. However, it is now necessary to highlight the most sensitive and specific miRNAs for each goal thanks to multicentric prospective studies. Conclusions: by their diversity and the feasibility of their use, miRNAs are getting part of the armamentarium of healthcare management of colorectal cancers.

AbstractCurrently, rapidly developing digital technological innovations affect and change the integrated information management processes of all sectors. The high efficiency of these innovations has inevitably pushed the health sector into a digital transformation process to optimize the technologies and methodologies used to optimize healthcare management systems. In this transformation, the Internet of Things (IoT) technology plays an important role, which enables many devices to connect and work together. IoT allows systems to work together using sensors, connection methods, internet protocols, databases, cloud computing, and analytic as infrastructure. In this respect, it is necessary to establish the necessary technical infrastructure and a suitable environment for the development of smart hospitals. This study points out the optimization factors, challenges, available technologies, and opportunities, as well as the system architecture that come about by employing IoT technology in smart hospital environments. In order to do that, the required technical infrastructure is divided into five layers and the system infrastructure, constraints, and methods needed in each layer are specified, which also includes the smart hospital’s dimensions and extent of intelligent computing and real-time big data analytic. As a result of the study, the deficiencies that may arise in each layer for the smart hospital design model and the factors that should be taken into account to eliminate them are explained. It is expected to provide a road map to managers, system developers, and researchers interested in optimization of the design of the smart hospital system.

The relationship between healthcare organizational accreditation and their leaders’ professional certification in healthcare management is of specific interest to institutions of higher education and individuals in the healthcare management field. Since academic program accreditation is one piece of evidence of high-quality education, and since professional certification is an attestation to the knowledge, skills, and abilities of those who are certified, we expect alumni who graduated from accredited programs and obtained professional certification to have a positive impact on the organizations that they lead, compared with alumni who did not graduate from accredited programs and who did not obtain professional certification. The authors’ analysis examined the impact of hiring graduates from higher education programs that held external accreditation from the Commission on Accreditation of Healthcare Management Education (CAHME). Graduates’ affiliation with the American College of Healthcare Executives (ACHE) professional healthcare leadership organization was also assessed as an independent variable. Study outcomes focused on these graduates’ respective healthcare organization’s performance measures (cost, quality, and access) to assess the researchers’ inquiry into the perceived value of a CAHME-accredited graduate degree in healthcare administration and a professional ACHE affiliation. The results from this study found no effect of CAHME accreditation or ACHE affiliation on healthcare organization performance outcomes. The study findings support the need for future research surrounding healthcare administration professional graduate degree program characteristics and leader development affiliations, as perceived by various industry stakeholders.
