SUMMARY A method is described for estimating oestriol in 2–10 ml samples of human pregnancy peripheral plasma. It incorporates acid hydrolysis, chemical purification, methylation, chromatography on alumina columns, formation of a derivative and quantitative determination by gas chromatography. A radioactive internal standard was added to correct for procedural losses. Plasma oestriol determinations in five normal patients throughout pregnancy and delivery are reported.

Abstract We describe an isothermal gas-liquid chromatographic procedure developed for measuring thiamylal and thiopental in plasma. The unchanged drug is extracted into ethyl acetate from acidified plasma, together with an internal standard. The solvent is removed, the residue methylated, aliquots, diluted with benzene, are injected into a 183-cm gas-liquid chromatographic column containing 3% OV-17. Sensitivity of detection is in the nanogram to picogram range.

Abstract A new column packing, SP-2510 DA (Supelco, Inc., Bellefonte, Pa. 16823), is an excellent stationary phase for the determination of a wide variety of anticonvulsant drugs by gas--liquid chromatography without derivatization. However, when uncomplicated extraction procedures are used, serum cholesterol interferes with the determination of primidone. By the simple expedient of adding a short "pre-column" containing another phase (SP-2250 DA) the problem is overcome.

Abstract We describe a case of fatal overdosage with primidone and methsuximide. During the early phase of the patient's hospital course we found concentrations of methsuximide, N-desmethylmethsuximide, and primidone in serum that far exceeded the usual therapeutic concentrations, as determined by gas-liquid chromatography. Determination of N-desmethylmethsuximide in peritoneal fluid demonstrated concentrations comparable to those in serum. This led to the therapeutic decision to manage the patient by dialysis. Subsequently, serum samples collected during the course of hospitalization were analyzed quantitatively by gas-liquid chromatography for methsuximide, N-desmethylmethsuximide, primidone, phenobarbital, and diphenylhydantoin. Selected serum specimens were also analyzed by gas chromatography-mass spectrometry, and N-methyl-2-hydroxymethyl-2-phenylsuccinimide, a metabolite of methsuximide not previously described in human serum, was identified by analysis of its mass spectrum.

Abstract Serum and cerebrospinal fluid of patients in coma owing to glutethimide overdose was assayed for glutethimide and associated compounds. The sample was extracted with chloroform and the extract assayed by gas-liquid chromatography on a column of "3% OV-17." Up to six constituents were found in serum in addition to glutethimide, only three of which were ever present in substantial quantity. The peaks seen on gas chromatography were numbered in order of elution. Glutethimide was peak No. 2. Peak No. 1 usually reached a maximum serum concentration in 10 or less hours, as did peak No. 4. Peak No. 3 reached a maximal serum concentration in 10 h or sooner also, but at 20 h the amount had changed little in most patients. Ambre and Fischer [Res. Commun. Pathol. Pharmacol. 4, 307 (1972)] speculate that peak No. 3 plays an important role in maintaining coma. Neither the pattern of change nor the relative concentrations in serum or cerebrospinal fluid on waking support their hypothesis.

Abstract The need for simultaneous determination of the concentration of diphenylhydantoin (Dilantin) and primidone (Mysoline) in serum is frequently expressed to the clinical chemistry laboratory. Isothermal gas-liquid chromatography (GLC) has been used to develop a rapid, specific method. The method involves a single extraction and no derivative formation. The procedure is simple enough to be used with large numbers of samples. Detection limits for the method are 0.3 µg diphenylhydantoin per ml and 0.1 µg primidone per ml. The mean precision of the method is 6.2% and 4.8%, expressed as the coefficient of variation, for diphenylhydantoin and primidone, respectively. Barbiturates and glutethimide added to serum samples did not interfere with the analysis. The method has been used for more than 500 patients without interferences from metabolites, and meets all criteria for routine and emergency use.
