We read with interest the study of Vain and colleagues.1 Eight years ago we also observed a similar favorable effect of exchange transfusions upon the evolution of severe neonatal sepsis associated with sclerema neonatorum.2,3 We studied 27 newborns with this problem; eight of these survived. In all eight neonates exchange transfusions were repeated at 12- to 15-hour intervals, until clinical and hematologic improvement was noted. When exchange transfusion was not used in newborns with sepsis, none survived.
