OBJECTIVE: To report a patient with lung cancer and idiopathic myelofibrosis with myeloid metaplasia who developed purpura and acute renal failure while receiving levofloxacin, and review the existing literature on quinolone nephrotoxicity. CASE SUMMARY: A 73-year-old white man, with a medical history of non-small-cell lung cancer and idiopathic myelofibrosis with myeloid metaplasia, was prescribed levofloxacin because of a lower urinary tract infection. Three days later, he presented with palpable purpura and erythematous skin lesions over the lower limbs and trunk, with a markedly reduced urinary output. Serum creatinine and urea nitrogen were 6.4 and 190 mg/dL, respectively. Levofloxacin was discontinued, and prednisone, furosemide, and intravenous fluids were given. The patient fully recovered over the ensuing 4 weeks. CONCLUSIONS: Nephrotoxicity associated with levofloxacin is uncommon. Allergic interstitial nephritis or vasculitis is believed to be the underlying pathologic process. Definitive diagnosis requires performance of renal biopsy, although this is not always feasible. In this case, a return of renal function to normal, with the disappearance of purpura following the discontinuation of levofloxacin and corticosteroid treatment, supports the presumptive diagnosis of a hypersensitivity reaction to levofloxacin.

Τόσο η IL-8 όσο και η OPG εμπλέκονται στην παθογένεση και τις κλινικές εκδηλώσεις της Μυελοΐνωσης που αποτελεί το Ph-αρνητικό ΜΥΝ με τη βαρύτερη εικόνα. Έχει αναφερθεί ότι η IL-8 και οι υποδοχείς της προάγουν την παθολογική αύξηση των μεγακαρυοκυττάρων (ΜΓΚ) (Emadi et al. IL-8 and its CXCR1 and CXCR2 receptors participate in the control of megakaryocytic proliferation, differentiation, and ploidy in myeloid metaplasia with myelofibrosis. Blood. 2005;105(2):464-73) ενώ είναι γνωστό ότι η OPG βρέθηκε αυξημένη σε ενδοθηλιακά κύτταρα (Bock et al. Osteosclerosis in advanced chronic idiopathic myelofibrosis is associated with endothelial overexpression of osteoprotegerin. Br J Haematol. 2005;130(1):76-82). Σκοπός αυτής της διατριβής, είναι η μελέτη της έκφρασης και έντασης ανοσοιστοχημικών χρώσεων IL-8 και OPG σε ΟΜΒ ασθενών με ΜΥΝ κατά τη διάγνωσή τους καθώς και των πιθανών συσχετισμών των παραπάνω με κλινικά χαρακτηριστικά και πρόγνωση. Μελετήθηκαν αναδρομικά 78 ασθενείς με ΜΥΣ που διαγνώστηκαν μεταξύ 1984 και 2012, με διάμεσο χρόνο παρακολούθησης 55 μηνών. Η διάμεση ηλικία τους ήταν 65 έτη, 49 εξ΄αυτών ήταν άνδρες. Το 22% είχαν διάγνωση Αληθούς Πολυκυτταραιμίας (ΑΠ), 42% είχαν Ιδιοπαθή Θρομβοκυττάρωση (ΙΘ) και 36% είχαν Μυελοΐνωση (ΜΙ). Στο σύνολο των ασθενών, 58% παρουσίαζαν σπληνομεγαλία, 8% είχαν οστικά άλγη, 81% παρουσίαζαν κόπωση, 9% είχαν εφιδρώσεις, 12% εμφάνισαν κάποιο θρομβωτικό επεισόδιο, 5% ανέφεραν απώλεια βάρους ενώ 77% ήταν θετικοί για τη μετάλλαξη JAK2. Ο καρυότυπος ήταν παθολογικός στο 32%, ενώ το 7%παρουσίαζε καρυοτυπικά ευρήματα δυσμενούς πρόγνωσης. Οι ανοσοιστοχημικές χρώσεις για την IL-8 και την OPG, πραγματοποιήθηκαν σε τομές παραφίνης 4μm των οστεομυελικών βιοψιών της διάγνωσης με τη μέθοδο δύο φάσεων συζευγμένων πολυμερών υπεροξειδάσης. Η ένταση και η θετικότητα της έκφρασης της IL-8 και της OPG μετρήθηκαν σε κλίμακα βαθμονομημένη από 0 έως 3. H στατιστική ανάλυση έγινε με το πρόγραμμα SPSS και στατιστικά σημαντικές θεωρήθηκαν οι τιμές του p < 0,05. Δεν παρατηρήθηκε σημαντική διαφορά στο βαθμό της έκφρασης των δύο χρώσεων ανάμεσα στα διάφορα ΜΥΣ. Στο σύνολο των ασθενών υψηλή ένταση της έκφρασης της IL-8 στα ΜΓΚ συσχετίσθηκε με πολυμορφοπυρήνωση (p=0,029), λεμφοπενία (p=0,026) και αυξημένη αιμοσφαιρίνη (p=0,02). Η OPG ήταν παρούσα στα κύτταρα της μυελικής σειράς και η υψηλή της ένταση παρατηρήθηκε σε ασθενείς με θρομβοκυττάρωση (p=0,015). Υψηλή ένταση της έκφρασης της OPG, στα ΜΓΚ, συσχετίσθηκε με πολυμορφοπυρήνωση (p=0,045), θρομβοκυττάρωση (p=0,02) και καρυότυπο μη δυσμενούς πρόγνωσης (p=0,006). H χαμηλή ένταση της OPG στα MΓΚ, συσχετίστηκε με πτωχή επιβίωση (p=0,012, Εικ 1). Δε βρέθηκε συσχετισμός μεταξύ OPG και παρουσίας οστικών αλγών. Συνοψίζοντας, το σημαντικότερο εύρημα αυτής της μελέτης ήταν ότι η υψηλή ένταση της έκφρασης της OPG στα MΓΚ συσχετίστηκε με αυξημένη επιβίωση και ευνοικό καρυότυπο σε ασθενείς με ΜΥΝ. Το παραπάνω αποτελεί νέο εύρημα και χρήζει επιβεβαίωσης σε μεγαλύτερη σειρά ασθενών.
