In the present environment of staggering technical innovations and increasing expectations of quality healthcare it is evident that we need to fine tune our diagnostic abilities in order to fulfil patients� demands for more efficient therapies and augmented quality of life. We are looking for current trends in clinical gynecology that make use of Liquid chromatography tandem mass spectroscopy, technology not yet employed in Romanian laboratories for the clinical practice but that is rapidly becoming the worldwide method of choice for accurate characterization of the hormonal milieu essential for the requirements of women healthcare.

Purpose. Sceletium plants have been used for its medicinal properties for centuries. However, there is a wide range of Sceletium plant species in which various alkaloidal components such as ∆7mesembrenone, mesembrenol, mesembranol, mesembrenone, mesembrine hydrochloride, epimesembranol and, sceletium A4 differ between species. Hence, to ensure the quality of Sceletium products used as a medicine, it is imperative to identify the appropriate species using both botanical and chemical methods. The chemical approach to identify and characterize the phytochemical composition of a particular species facilitates the choice of species that will provide the purported therapeutic outcome.  Hence, specific analytical methods to identify relevant constituents from complex matrices are necessary. Although HPLC-UV detection is commonly used to identify and estimate phytochemical content of medicinal plants, use of mass spectroscopy (MS) and tandem mass spectroscopy (MS/MS) can unequivocally confirm their presence/absence based on characteristic ions and fragmentation patterns. Methods. The various alkaloidal components were characterized by electrospray ionization (ESI) MS and MS/MS using an ionizing medium of 0.1% ammonium hydroxide in water mixed with acetonitrile. Compounds were purified and characterized for use as reference standards to identify the relevant alkaloidal constituents of several Sceletium plant species using HPLC with on-line UV-MS detection. Results. ESI-MS provided the [M+H]+ ions with respective m/z values that related to the respective molecular weights 287, 289, 291, 287, 289, 324 and 291 for the above mentioned alkaloids, whereas, ESI MS/MS provided the characteristic fragment ions to confirm the structural identity of the individual alkaloids and subsequently used to  confirm the presence and/or absence of specific alkaloids in various Sceletium plant samples. Conclusions. Whilst HPLC-UV detection has been a widely-used conventional analytical technique for both qualitative and quantitative analyses, the results highlight the necessity of ESI-MS detection to avoid erroneous identification of phytochemical components, particularly with mesembrine-type compounds which have closely related chemical structures. This article is open to POST-PUBLICATION REVIEW. Three oligostilbenes have been identified in stems extract of Dipterocarpus semivestitus through dereplication strategy. The stems extract was injected to liquid chromatography tandem mass spectroscopy. Two tetramer, hopeaphenol and hemsleyanol D, were identified by comparing their mass fragmentation patterns with in-house library. A trimer, a-viniferin was isolated by various separation techniques and its structure was confirmed by NMR analyses and comparison with published data.

Background The incidence of inborn errors of metabolism varies widely across countries. Very few studies have analyzed the incidence of these disorders in Mainland China. We aimed to estimate the overall and disease-specific incidences of inborn errors of metabolism in Chinese newborns and investigate the geographical distribution of these disorders. Methods A national cross-sectional survey was conducted to investigate newborn inborn errors of metabolism screening by tandem mass spectroscopy in Mainland China between 2016 and 2017. A total of 246 newborn screening centers were surveyed using a standardized questionnaire. We examined the cumulative and disease-specific incidences of inborn errors of metabolism in Mainland China as a whole and in different geographical locations. Results Over 7 million newborns were screened and 2747 were diagnosed with inborn errors of metabolism, yielding an overall incidence of 38.69 per 100,000 births (95% confidence interval: 37.27–40.17). The most common disorders were amino acid disorders (17.14 per 100,000 births, 95% confidence interval: 16.21–18.13), followed by organic acid disorders (12.39 per 100,000 births, 95% confidence interval: 11.60–13.24) and fatty acid oxidation disorders (9.16 per 100,000 births, 95% confidence interval: 8.48–9.89). The overall and disease-specific incidence rates differed significantly across geographical locations (P < 0.001). Conclusions The overall incidence of inborn errors of metabolism in Chinese newborns is relatively high. It is urgent to establish the recommended uniform screening panel for inborn errors of metabolism to guide the national and regional tandem mass spectroscopy newborn screening programs.
