Nuclear protein in testis (NUT) carcinoma represents a highly aggressive, poorly differentiated carcinoma that is genetically defined by rearrangement of NUT gene. The histomorphological appearance ranges from entirely undifferentiated carcinoma to carcinoma with prominent squamous differentiation. NUT carcinoma can display neuroendocrine features. Although it is typically distributed along the midline axis, it may manifest in nonmidline locations. The majority of patients develop rapidly disseminated disease. We illustrate 2 cases of NUT carcinoma, one located in the lung, which closely resembled a neuroendocrine carcinoma, and the other one with assumed lung origin demonstrating metastatic dissemination with diffuse bone involvement, which was clinically first suspected to be a hematological malignancy. Due to its undifferentiated nature, NUT carcinoma may be confused with many entities. NUT immunohistochemistry is considered to be sufficient for the diagnosis. Fluorescence in-situ hybridization analysis and next-generation sequencing are currently used to confirm the diagnosis.

Replacement of Ringer's chloride by a variety of nonpenetrating anions results in prolonged electrical and mechanical responses of muscle to stimulation. The ‘negative after-potential’ is characterized by a slowly increasing secondary depolarization which reaches a stable plateau lasting as long as 2 minutes. After-discharge frequently occurs during early depolarization. In most fibers repolarization is relatively abrupt, but in some, slow oscillations resembling relaxation oscillations arise following the plateau, grow gradually in amplitude and, only when they are of sufficient amplitude, does the membrane repolarize. Prolonged depolarization can still be produced when the spike has failed. At times, fibers may respond with short-duration action potentials, but may be primed to give prolonged responses by previous stimuli or by increase of external potassium. Addition of chloride has no effect below a critical concentration. Reduction of sodium to 25% of normal does not change plateau level or duration. Duration of the plateau phase is decreased by potassium.

NUT carcinoma (NC) is a type of aggressive cancer driven by chromosome translocations. Fusion genes between a DNA-binding protein, such as bromodomain and extraterminal domain (BET) proteins, and the testis-specific protein NUTM1 generated by these translocations drive the formation of NC. NC can develop in very young children without significant accumulation of somatic mutations, presenting a relatively clean model to study the genetic etiology of oncogenesis. However, after 20 years of research, a few challenging questions still remain for understanding the mechanism and developing therapeutics for NC. In this short review, we first briefly summarize the current knowledge regarding the molecular mechanism and targeted therapy development of NC. We then raise three challenging questions: (1) What is the cell of origin of NC? (2) How does the germline analogous epigenetic reprogramming process driven by the BET-NUTM1 fusion proteins cause NC? and (3) How will BET-NUTM1 targeted therapies be developed? We propose that with the unprecedented technological advancements in genome editing, animal models, stem cell biology, organoids, and chemical biology, we have unique opportunities to address these challenges.
