The Impact

Background:One of the drugs used in the treatment of rheumatoid arthritis (RA) is rituximab. Given the fact that the RA affects not only the joints, but, in addition to other organs, and the heart, our interest has caused the likelihood of the impact of rituximab on non-articular manifestations of rheumatoid arthritis, in particular, related to the cardiovascular system.Objectives:to study the effect of rituximab on the development of diastolic dysfunction of the heart in patients with RA.Methods:98 patients with RA were examined, of which 28 patients took rituximab 1000 mg according to the scheme for 6-12 months and methotrexate up to 25 mg per week per os (1st group), and 70 patients – only methotrexate up to 25 mg per week at least 12 months (2nd group). They were held echocardiography with calculation of the E/a of mitral valve, E/a of tricuspid valve, end-diastolic size of left ventricle, electrocardiography with calculation of the dispersion of QT interval and vectorcardiography with determination of the areas of the P loop, QRS loop, T loop, and the maximum vector (MV), MV-azimuth and MV-lift.Results:the E/a of mitral and tricuspid valves in 1st group were higher than in 2nd group (p<0.05). In 1st group, a correlation was found between the E/a of both valves and the MV-lift; and also between the dispersion of QT interval and the maximum vector (p<0.05).Conclusion:in patients taking rituximab, there is a relationship between diastolic dysfunction, electrical instability of the ventricles and signs of electrophysiological remodeling. Background:The presence of SSA (anti-Ro) and SSB (anti-La) autoantibodies has important clinical implications for female patients with a wish to conceive. The association between maternal levels of these autoantibodies and complete congenital heart block and neonatal Lupus syndrome is well established. Currently guidelines advise to test for these antibodies in all Rheumatoid Arthritis (RA) patients with a wish to conceive (1).Objectives:The objective was to determine the prevalence and titers of SSA and SSB autoantibodies in female patients with rheumatoid arthritis and a wish to conceive or who are pregnant.Methods:Patients were derived from 2 large prospective cohorts on RA and pregnancy (PARA-cohort and PreCARA-cohort). The presence and titers of SSA and SSB were tested using Phadia, an automated system which uses fluorescence enzyme immunoassays (FEIA) (Thermo-Fisher Scientific).Results:We included a total of 647 patients with RA and a wish to conceive, 417 (64.5%) conceived during the follow-up period. A detailed description of the study population, stratified for the presence of SSA and SSB antibodies autoantibodies is presented in table 1. A total of 25 out of 647 patients had detectable SSA or SSB antibodies: SSA n = 25 (SSA-52 n = 17, SSA-60 n = 19), SSB n = 7. This corresponds with a prevalence of 3.9% for SSA antibodies and 1.1% for SSB antibodies. 13 (52% of the SSA positive patients, 2.0% of the total population) patients had a titer of >240 units/ml of SSA antibodies, and 3 (42.9% of the SSB positive patients, 0.46% of the total population) patients had a titers >340 units/ml of SSB antibodies. Moreover, about half of the patients positive for SSA or SSB had, based on literature, moderate or low titers of these antibodies (2). The paper discusses the article «Common language description of the term rheumatic and musculoskeletal diseases (RMDs) for use in communication with the lay public, healthcare providers and other stakeholders endorsed by the European League Against Rheumatism (EULAR) and the American College of Rheumatology (ACR)» that appeared in the on-line first journal Annals of the Rheumatic Diseases in March 2018. It is shown that, on the one hand, the development of the definition is positive, but, on the other, blurs the verges of rheumatology due to the introduction of nosological entities that are absolutely unrelated to our profession.

Introduction. Modern sports places high demands on the functional systems of the body at all stages of the training and competitive processes. High loads create signifi cant background for the occurrence of musculoskeletal diseases, among which the myofascial syndrome (MFS) holds the leading position. MFS negatively affects the functional readiness of the sportsman′s body and is a risk factor for injuries of the musculoskeletal system. In this regard, timely treatment of MFS is important. It creates the conditions for the improvement of sports achievements, for health maintenance and sports longevity. Treatment of patients with MFS should include methods that normalize trophism and muscle tone. Given the problem of doping, the use of non-drug methods, including osteopathy, is preferable. The goal of research — to develop a system of monitoring and osteopathic correction of myofascial disorders (MFD) and to introduce it into the practice of medical and biological support for sportsmen of different categories and levels during the annual training process.Materials and methods. 93 sportsmen (45 women and 48 men), mean age 22,1±1,1 years, representing diffe rent sports were examined. The essence of the proposed system is regular clinical and electroneurophysiological monitoring of the body condition and preventive correction of myofascial disorders in order to exclude their infl uence on the success of sports activities.Results. It was established that the proposed system of monitoring and osteopathic correction led to a signifi cant reduction in clinical and electroneurophysiological manifestations both in local and in extensive forms of myofascial disorders, which allowed to maintain the optimal functional readiness of sportsmen throughout the entire annual training cycle.Conclusion. The system of monitoring and correction of MFS with the use of osteopathic treatment can be one of the elements of medical and biological support for sportsmen during the annual training process.
