Post-operative delirium in elderly patients is associated with higher mortality and longer lengths of stay. This prospective observational study investigated the incidences of preoperative mild cognitive impairment (MCI) and post-operative delirium among elderly patients undergoing elective surgery at the authors’ institution, the relationship between MCI and delirium, and the impact of delirium on surgical morbidity and lengths of stay. The authors found that although MCI did not predict post-operative delirium, the high incidence of MCI in elective surgery patients warrants further study.

Age is not as important in predisposing to infections as are the associated problems peculiar to certain age groups. Factors such as the advanced age of the patients combined with the presence of chronic disease reduce their resistance to infection. This study comprises 212 elderly patients (aged 65–98 years) who were treated with 500–1000 mg/day ciprofloxacin for 1–18 days. Despite the high incidence of associated chronic diseases, microbiology showed that infections were eradicated in 88.5%. Clinical resolution occurred in 75.5% of patients and clinical failure occurred in 6.1%. Treatment was well tolerated, with clinical side-effects reported in only seven patients. Ciprofloxacin may be considered an effective and safe antimicrobial agent for the treatment of infections in the elderly.

New drugs targeting single mutations have been recently approved for Acute Myeloid Leukemia (AML) treatment, but allogeneic transplant still remains the only curative option in intermediate and unfavorable risk settings, because of the high incidence of relapse. Molecular analysis repertoire permits the identification of the target mutations and drives the choice of target drugs, but the etherogeneity of the disease reduces the curative potential of these agents. Primary and secondary AML resistance to new target agents is actually an intriguing issue and some of these mechanisms have already been explored and identified. Changes in mutations, release of microenvironment factors competing for the same therapeutic target or promoting the survival of blasts or of the leukemic stem cell, the upregulation of the target-downstream pathways and of proteins inhibiting the apoptosis, the inhibition of the cytochrome drug metabolism by other concomitant treatments are some of the recognized patterns of tumor escape. The knowledge of these topics might implement the model of the ‘AML umbrella trial’ study through the combinations or sequences of new target drugs, preemptively targeting known mechanisms of resistance, with the aim to improve the potential curative rates, expecially in elderly patients not eligible to transplant.

Abstract Background Delirium is an overlooked syndrome in Emergency Departments (ED) around the world. Previous studies demonstrated high incidence of Delirium in elderly patients with COVID-19, however, these studies did not compare Delirium incidences in patients admitted for SARS-CoV-2 infection or sepsis for other etiologies. Methods This is an observational study that analyzed elderly patients (> 65 years) from two cohort studies that recruited patients in the ED at Hospital Clínicas da Faculdade de Medicina da Universidade de São Paulo, an urban, academic department in São Paulo, Brazil. The first cohort studied patients with sepsis and was conducted between September 30, 2019, and March 17, 2020. The second cohort studied COVID-19 patients between April 8, 2021, and May 28, 2021. Our primary outcome was incidence of delirium at admission in ED, which we defined using the Confusion Assessment Method (CAM). Additional study information was collected from medical records. Results We included 141 patients in our final analysis, with a median age of 72 (IQR 68-79) years and a predominance of the male sex (58%). We identified delirium at admission in 31 participants (22%), 20 in septic group and 11 in COVID-19 group (p = 0.70). Conclusion This present study does not support that elderly patients with COVID-19 have higher risk for Delirium at admission in Emergency Department when compared to patients hospitalized with sepsis for other etiologies. In spite of sepsis for COVID-19 have had more than 28% mortality, COVID-19 was not associated with higher mortality in elderly than sepsis for other etiologies.

Objectives: Osteosarcoma is the most common malignant bone tumor in childhood. Although a poorer prognosis has been described in older patients, few reports have focused solely on primary osteosarcoma. We evaluated the clinical features of elderly patients with primary osteosarcoma. Materials and Methods: Ninety-four patients were included in this retrospective study, and we divided them into 2 groups (older patients and younger patients) based on a cut-off age of 40 years. The patients’ information, including age, tumor type, location, presence of metastasis, American Joint Committee on Cancer (AJCC) stage, treatment-related factors, local and distant relapse, and outcome, was collected. We compared the clinical courses between the 2 groups in all and only deceased patients. Results: In all patients, the frequency of chemotherapy in the older group was significantly lower than in the younger group (p < 0.001), and tumors were more frequent in axial bone in the older patients (p = 0.041). Only in patients with surgical treatment, histological effectiveness after chemotherapy in the older group was lower than in the younger group (p = 0.041). The older patients showed a poorer prognosis (p = 0.031). However, the 5-year overall survival rate in the older patients was more favorable than that in the younger patients only among deceased patients (p =0.032). Only the existence of metastasis affected the prognosis in older patients (p = 0.012). Conclusion: Primary osteosarcoma in elderly patients showed a high incidence of axial bone involvement, a low rate of chemotherapy, and resistance to chemotherapy. Although the final life prognosis is poor, survival may be relatively prolonged.

Bronchial asthma is a common problem with enormous medical and economics impacts. It is an inflammatory disease of the airways associated with intermittent episodes of bronchospasm. Asthma is not uncommon in the elderly patients. Prevalence of asthma is similar in older and younger adults. Asthma in the elderly patient is underdiagnosed because of false perceptions by both patient and physician. The high incidence of comorbid conditions in the elderly patient makes the diagnosis and management more difficult. Correct diagnosis is demonstrated with spirometry. The goals of asthma treatment are to achieve and maintain control of symptoms and to prevent development of irreversible airflow limitation. Asthma drugs are preferably inhaled because this route minimizes systemic absorption and, thus, improves the ratio of the therapeutic benefit to the potential side-effects in elderly patients.
