A patient was diagnosed with ulcerative colitis (UC) in 2010. In March 2015, she had abdominal pain, diarrhea, bloody stool, and UC has relapsed. In June 2015, pain and sensory disturbance of both lower limbs appeared. Blood flow at the distal femoral artery was not confirmed with magnetic resonance angiography, and it was diagnosed as bilateral acute inferior limb ischemia. Arterial thrombolectomy with Fogarty’s balloon catheter was performed and blood flow was improved. The severity of UC was moderate with Mayo score 8. Thrombosis is considered to be a complication with a high incidence in inflammatory bowel disease. Reports of arterial thrombosis are very rare. It is important to evaluate the risk of bleeding and thrombosis in active or severe cases in UC and need to do thrombotic prophylactic treatment simultaneously with UC treatment.

1. A rapid fall in plasma renin concentration occurs not only in colonectomized but also in hepatectomized dogs after binephrectomy. 2. In the anhepatic anephric dog no renin extraction can be demonstrated across lungs, splanchnic region, spleen, inferior limb and head. 3. No renin inactivation occurs in blood kept in vitro at 37°C for 3 h taken from anaesthetized dogs and from the anhepatic anephric dogs. 4. We suggest that the clearance of renin in the anhepatic anephric dog is due to extraction or inactivation of renin by tissues in general.

Ewing's Sarcoma is a rare, malignant osseous tumor arising from undifferentiated cells originating from the neural crest; however it may also develop in the soft tissue. The initial evaluation of a patient who is suspected to be suffering from Ewing's Sarcoma entails taking a thorough medical history along with performing a detailed physical examination. Our patient was diagnosed with Ewing's Sarcoma upon seeking medical attention due to the sudden appearance of pain in his left hip. The medical staff conducted a series of investigations including X-rays, MRI, CT and 3D-CT scans of the inferior limb and pelvis in order to yield the diagnosis of Ewing's Sarcoma; however, the definitive diagnosis was confirmed only by the biopsy performed before the surgery for modular endoprotesation. The patient received preoperative chemotherapy (in order to reduce the tumor dimension) according to the EuroEwing 2008 Protocol. After the seventh course of chemotherapy, surgery was performed, consisting of in-block resection of the proximal left femur and hip joint, followed by the insertion of a modular femoral endoprosthesis. Postoperative follow-up, however necessitated another six courses of chemotherapy.

Background:  It is suggested that surgery results in changes in kinetic profile of some medication. The aim of this study was to investigate possible alterations in pethidine’s half-life in postoperative pain management following orthopedic surgery of the inferior limb. Methods: Twelve patients, who were classified as class I patients according to the American society of anesthesiologists physical status classification, were enrolled. Following the surgery of the lower limb, 25 mg of pethidine was injected intravenously. After that, 5, 30, 60 and 180 minutes following the injection, blood samples were taken and concentration of pethidine in blood samples was measured by High Performance Liquid Chromatography method. Moreover, patients’ pain levels were assessed on visual analogue scales. Results: The average half-life of pethidine was measured to be 29.68 minutes. Thirty minutes after the injection, significant relationship between plasma levels of pethidine and pain scale was reported (p= 0.041, r= 0.595). Moreover, men were found to perceive more pain than women. Pain scale was considerably different between smokers and non-smokers (p= 0.006), although blood concentration of pethidine was not significantly different between these two groups (p=0.09). Conclusions: The average half-life of pethidine was shorter compared to the established half-life in literature. Orthopedic surgery most probably results in alterations in pharmacokinetic profile of pethidine. Moreover, gender and smoking status of the patients influence pain perception. Thus, pharmacokinetic alterations due to inferior limb orthopedic surgery, gender-related factors and smoking status of the patients should be considered in pain management in clinical settings. ABSTRACT Background: We describe here a very rare congenital deformity that involves the splitting of the right upper limb with the superior limb articulating with the shoulder joint, and the inferior limb laterally attached to the chest wall. Material and Methods: The child with this rare split limb was treated by transferring the inferior limb on an islanded pedicle to the superior one while creating the hand. Results: A unified limb was reconstructed while creating the hand without any compromise on the existing function and vascularity. The patient is on regular follow-up and further staged procedures have been planned to provide a better functional and aesthetic limb. Conclusion: The congenital deformity described here has not been mentioned in world literature so far and its embryological basis is a matter of discussion. Opinions regarding further management of this anomaly are invited from experts in the field.
