Objective: Prospectively validate two prognostic scores, pre-hospitalisation (SOARS) and hospitalised mortality prediction (4C Mortality Score), derived from the coronavirus disease 2019 (COVID-19) first wave, in the evolving second wave with prevalent B.1.1.7 and parent D614 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variants, in two large United Kingdom (UK) cohorts. Design: Prospective observational cohort study of SOARS and 4C Mortality Score in PREDICT (single site) and multi-site ISARIC (International Severe Acute Respiratory and Emerging Infections Consortium) cohorts. Setting: Protocol-based data collection in UK COVID-19 second wave, between October 2020 and January 2021, from PREDICT and ISARIC cohorts. Participants: 1383 from single site PREDICT cohort and 20,595 from multi-site ISARIC cohort. Main outcome measures: Relevance of SOARS and 4C Mortality Score derived from the COVID-19 first wave, determining in-hospital mortality and safe discharge in the UK COVID-19 second wave. Results: Data from 1383 patients (median age 67y, IQR 52-82; mortality 24.7%) in the PREDICT and 20,595 patients from the ISARIC (mortality 19.4%) cohorts showed both SOARS and 4C Mortality Score remained relevant despite the B.1.1.7 variant and treatment advances. SOARS had AUC of 0.8 and 0.74, while 4C Mortality Score had an AUC of 0.83 and 0.91 for hospital mortality, in the PREDICT and ISARIC cohorts respectively, therefore effective in evaluating both safe discharge and in-hospital mortality. 19.3% (231/1195, PREDICT cohort) and 16.7% (2550/14992, ISARIC cohort) with a SOARS of 0-1 were potential candidates for home discharge to a virtual hospital (VH) model. SOARS score implementation resulted in low re-admission rates, 11.8% (27/229), and low mortality, 0.9% (2/229), in the VH pathway. Use is still suboptimal to prevent admission, as 8.1% in the PREDICT cohort and 9.5% in the ISARIC cohort were admitted despite SOARS score of 0-1. Conclusion: SOARS and 4C Mortality Score remains valid, providing accurate prognostication despite evolving viral subtype and treatment advances, which have altered mortality. Both scores are easily implemented within urgent care pathways with a scope for admission avoidance. They remain safe and relevant to their purpose, transforming complex clinical presentations into tangible numbers, aiding objective decision making. Trial registration: NHS HRA registration and REC approval (20/HRA/2344, IRAS ID 283888).

<div align="center"><table width="645" border="1" cellspacing="0" cellpadding="0"><tbody><tr><td valign="top" width="439"><p><strong>Abstract: </strong>The purpose of this study is to describe the implementation of the school litersion movement in the Tanjung Redeb Elementary School. The schools selected in this study are SDN 002 Tanjung Redeb, SDN 004 Tanjung Redeb, and SDIT Ash-Shohwa because these elementary schools have implemented a school literacy movement policy. This research uses a qualitative research method of multi-site research. Data collected related to the implementation of the school literacy movement in the habituation stage. The data analysis technique starts with a single site data analysis followed by cross site data analysis. A single site analysis procedure is performed by presenting data, reducing data, and withdrawing conclusions. Cross-site analysis procedures are carried out by looking for temporary cross-site findings. Research results can be reviewed from the focus of the study. Each site engages in literacy habituation by providing literacy facilities and infrastructure in the form of library facilities, reading corners in the school environment and in the classrooms and reading areas. At the stage of habituation, the selection of appropriate reading books for literacy is carried out, carrying out literacy before learning with reading and writing activities, making the school environment rich in text through pictures and posters, and involving the public in the implementation of school literacy.</p><p class="Abstract"><strong>Abstrak:</strong> Tujuan penelitian ini untuk mendeskripsikan impementasi Gerakan Literasi Sekolah di Sekolah Dasar Tanjung Redeb. Sekolah yang dipilih dalam penelitian ini yaitu, SDN 002 Tanjung Redeb, SDN 004 Tanjung Redeb, dan SDIT Ash-Shohwa karena SD tersebut telah menerapkan kebijakan gerakan literasi sekolah. Penelitian ini menggunakan metode penelitian kualitatif jenis penelitian studi multisitus. Data yang dikumpulkan terkait implementasi gerakan literasi sekolah dalam tahap pembiasaan. Teknik analisis data dimulai dengan analisis data situs tunggal dilanjutkan dengan analisis data lintas situs. Prosedur analisis situs tunggal dilakukan dengan penyajian data, reduksi data, dan penarikkan kesimpulan. Prosedur analisis lintas situs dilakukan dengan mencari temuan sementara lintas situs. Hasil penelitian dapat ditinjau dari fokus penelitian. Setiap situs melakukan keggiatan pembiasaan literasi dengan dengan menyediakan sarana dan prasarana literasi berupa fasilitas perpustakaan, pojok baca di lingkungan sekolah dan di dalam kelas serta area baca. Pada tahap pembiasaan dilakukan kegiatan pemilihan buku bacaan yang tepat untuk literasi, melaksanakan literasi sebelum pembelajaran dengan kegiatan membaca dan menulis, membuat lingkungan sekolah kaya akan teks melalui gambar dan poster, dan melibatkan publik dalam pelaksanaan literasi sekolah.</p></td></tr></tbody></table></div>

Abstract The UK standard for estimating flood frequencies is outlined by the flood estimation handbook (FEH) and associated updates. Estimates inevitably come with uncertainty due to sampling error as well as model and measurement error. Using resampling approaches adapted to the FEH methods, this paper quantifies the sampling uncertainty for single site, pooled (ungauged), enhanced single site (gauged pooling) and across catchment types. This study builds upon previous progress regarding easily applicable quantifications of FEH-based uncertainty estimation (Kjeldsen 2015, 2021; Dixon 2017). Where these previous studies have provided simple analytical expressions for quantifying uncertainty for single site and ungauged design flow estimates, this study provides an easy-to-use method for quantifying uncertainty for enhanced single site estimates.

Vascular surgery has recently been recognised as a full surgical specialty in the UK. Major vascular interventions are centralised into hub units where sufficient volume of cases exists to achieve good outcomes and support training. 1 If no consolidation onto a single site has occurred, clinical networks have formed to provide emergency vascular cover for their catchment area. In local district hospitals, visiting vascular surgeons can run clinics, perform day-case minor elective surgery and offer a vascular opinion. 1 In 2013 the first round of trainee recruitment at ST3 level in vascular surgery, separate from general surgery, was completed. Vascular trainees will do less general surgery and as consultants they will work on a pure vascular rota. 2
