Background: Ankle dorsiflexion range of motion has been measured in weightbearing and nonweightbearing conditions. The different measurement conditions may contribute to inconsistent conclusions regarding the role of ankle dorsiflexion in several pathologic conditions. The purpose of this study was to examine the relationship between ankle dorsiflexion range of motion as measured in weightbearing and nonweightbearing conditions. Methods: We compared ankle dorsiflexion range of motion as measured in a weightbearing versus a nonweightbearing position in 43 healthy volunteers. Measurements were taken separately by two examiners. Results: Weightbearing and nonweightbearing ankle dorsiflexion measurements produced significantly different results (P < .0001). The two measurements correlated moderately (r = 0.6 and r = 0.64 for examiners 1 and 2, respectively; P < .001). Conclusions: Weightbearing and nonweightbearing ankle dorsiflexion measurements produce significantly different results and only a moderate correlation, suggesting that these two measurements should not be used interchangeably as measures of ankle dorsiflexion range of motion. Background: Measurement of weightbearing ankle dorsiflexion (DF) passive range of motion (PROM) has been suggested as a way to estimate ankle kinematics during gait; however, no previous study has demonstrated the relationship between ankle DF during gait and ankle DF PROM with knee extension. We examine the relationship between maximum ankle DF during gait and nonweightbearing and weightbearing ankle DF PROM with knee extension. Methods: Forty physically active individuals (mean ± SD age, 21.63 ± 1.73 years) participated in this study. Ankle DF PROM with knee extension was measured in the nonweightbearing and weightbearing conditions; maximum ankle DF during gait was assessed using a three-dimensional motion analysis system. The relationship between each variable was calculated using the Pearson product moment correlation coefficient, and the difference in ankle DF PROM between the nonweightbearing and weightbearing conditions was analyzed using a paired t test. Results: The weightbearing measurement (r = 0.521; P < .001) for ankle DF PROM showed a greater correlation with maximum ankle DF during gait than did the nonweightbearing measurement (r = 0.245; P = .029). Ankle DF PROM was significantly greater in the weightbearing than in the nonweightbearing condition (P < .001) despite a significant correlation between the two measurements (r = 0.402; P < .001). Conclusions: These findings indicate that nonweightbearing and weightbearing measurements of ankle DF PROM with knee extension should not be used interchangeably and that weightbearing ankle DF PROM with the knee extended is more appropriate for estimating ankle DF during gait.

The purpose of this study was to examine the relationship among personality traits, self-esteem and collective self-esteem of Latvian Arts therapists and specifically to examine a potentially mediating role of self-esteem on the correlation between personality traits and collective self-esteem. The sample of the study consisted of 81 Arts therapists of whom data for 74 AT were analysed. The data were collected by Latvian Personality Inventory (LPI-v3), Collective Self-Esteem Scale (CSES), Rosenberg Self-Esteem Scale (RSES) including the demographic data. Pearson correlations indicated significant negative moderate correlation between self-esteem and LPI scale Neuroticism, positive moderate correlation between self-esteem and CSES scale Membership self-esteem and between LPI scale Neuroticism and CSES scale Membership self-esteem, therefore mediation analysis was applied to examine the effect of self-esteem on a relationship between Neuroticism and Membership Self-Esteem. Sobel test was used to test the significance of a mediation effect. Results indicated Self-esteem as a significant mediator in the relationship between Neuroticism and Membership self-esteem. The findings of this research suggest that emotional stability and self-esteem influence Latvian Arts therapist’s sense of identifying themselves as members with their professional group. These results are important for further development of Latvian Arts therapist profession to strengthen Art therapist’s professional identity.

Objective. The NOCTURNIN gene links nutrient absorption and metabolism to the circadian clock. Shift workers are at a heightened risk of overweight and of developing obesity and metabolic syndrome. This study investigates the diurnal variation of NOCTURNIN in healthy volunteers and its expression levels in rotational shift and daytime workers. Methods. NOCTURNIN expression levels were evaluated in peripheral blood lymphocytes from 15 healthy volunteers at 4-hour intervals for 24 h. Metabolic parameters and NOCTURNIN expression were measured in workers engaged in shift and daytime work. Results. In the group of volunteers NOCTURNIN expression showed diurnal variation, with a peak at 8:00 AM. NOCTURNIN expression was higher in shift workers than in daytime workers. Multivariate analysis confirmed the role of shift work as an independent factor affecting NOCTURNIN expression. Notably, its level correlated directly with body mass index and inversely with total energy expenditure. Conclusions. Measuring NOCTURNIN expression levels in human peripheral blood lymphocytes can improve investigations on the relationship between changes in circadian rhythm and metabolic disorders. Shift workers show higher NOCTURNIN levels than daytime workers.

Background: The glenoid track concept has been widely used to assess the risk of instability caused by a bipolar lesion. The mean glenoid track width is reported to be 83% of the glenoid width. However, this width seems to be affected by the range of motion of the shoulder. By clarifying the relationship between the range of shoulder motion and the glenoid track width, a more precise determination of the glenoid track width for each individual could be possible. Purpose: To determine the relationship between the glenoid track width and the range of motion of healthy volunteers. Study Design: Descriptive laboratory study. Methods: Magnetic resonance imaging was taken in 41 shoulders of 21 healthy volunteers (mean age, 32 years) with the arm in maximum horizontal extension, with the arm kept in 90° of abduction and 90° of external rotation. Three-dimensional surface bone models of the glenoid and the humerus were created with image analysis software. The distance from the anterior rim of the glenoid to the medial margin of the footprint of the rotator cuff tendon was defined as the glenoid track width. Active and passive ranges of shoulder motion were measured in the supine and sitting positions. The correlations between the glenoid track width and the ranges of shoulder motion were investigated with Pearson correlation coefficients. Intra- and interobserver reliabilities based on the intraclass correlation coefficient were also analyzed to assess the reliability of the glenoid track measurement. Results: The intra- and interobserver reliabilities for the glenoid track measurement were excellent (0.988 and 0.988, respectively). Among all the measurements, the glenoid track width and the active range of motion in horizontal extension in the sitting position showed the greatest correlation coefficient ( r = −0.623, P < .0001). A correlation between the glenoid track width and this angle was expressed as Y = −0.49 X + 90, where X is the horizontal extension angle (degrees) and Y is the glenoid track width (percentage of glenoid width). Conclusion: The present data demonstrate that the greater the horizontal extension angle in abduction and external rotation, the smaller the glenoid track width. An individualized glenoid track width can be obtained by measuring the active horizontal extension angle with the arm in abduction and external rotation in the sitting position. Clinical Relevance: An individualized glenoid track width enables selection of a more precise surgical option by the on-track/off-track concept.

Abstract Context: Gastrocnemius stretching exercises often are prescribed as part of the treatment program for patients with overuse injuries associated with limited ankle dorsiflexion. However, little is known about how the position of the subtalar joint during gastrocnemius stretching affects ankle dorsiflexion range of motion (ROM). Objective: To determine the effect of subtalar joint position during gastrocnemius stretching on ankle dorsiflexion ROM. Design: This study was a 3-way mixed-model design. The 3 factors were subtalar joint position (supinated, pronated), lower extremity (experimental, control), and time (pretest, posttest). Lower extremity and time were the repeated measures. Setting: University research laboratory. Patients or Other Participants: Thirty-three healthy volunteers (29 women, 4 men). Intervention(s): Participants performed a gastrocnemius stretching exercise 2 times daily for 3 weeks with the subtalar joint of the randomly assigned experimental side (dominant or nondominant) in the randomly assigned position (supination or pronation). The contralateral lower extremity served as the control. Main Outcome Measure(s): Before and after the 3-week gastrocnemius stretching program, we used goniometers to measure ankle dorsiflexion ROM in weight-bearing and non–weight-bearing positions with the subtalar joint positioned in anatomic 0°. Results: Ankle dorsiflexion ROM measured in weight-bearing and non–weight-bearing positions increased after the gastrocnemius stretching program (P  =  .034 and .003, respectively), but the increase in ROM did not differ based on subtalar joint position (P  =  .775 and .831, respectively). Conclusions: Subtalar joint position did not appear to influence gains in ankle dorsiflexion ROM after a gastrocnemius stretching program in healthy volunteers.

Restricted gastrocnemius length may impair movement efficiency during functional movements. However, this is yet to be examined among tactical athletes. This study examined the relationship between gastrocnemius muscle length and movement compensations during a two-leg overhead squat among career firefighters. Bilateral ankle dorsiflexion passive range of motion data were collected from 50 firefighters, and movement compensations observed during a two-leg overhead squat were recorded. Firefighters with reduced average ankle dorsiflexion passive range of motion were more likely to demonstrate movement compensations during the overhead squat. Clinicians should utilize interventions that lengthen gastrocnemius musculature to improve the squat movement efficiency of firefighters.

Purpose The minimum masking level (MML) is the minimum intensity of a stimulus required to just totally mask the tinnitus. Treatments aimed at reducing the tinnitus itself should attempt to measure the magnitude of the tinnitus. The objective of this study was to evaluate the reliability of the MML. Method Sample consisted of 59 tinnitus patients who reported stable tinnitus. We obtained MML measures on two visits, separated by about 2–3 weeks. We used two noise types: speech-shaped noise and high-frequency emphasis noise. We also investigated the relationship between the MML and tinnitus loudness estimates and the Tinnitus Handicap Questionnaire (THQ). Results There were differences across the different noise types. The within-session standard deviation averaged across subjects varied between 1.3 and 1.8 dB. Across the two sessions, the Pearson correlation coefficients, range was r = .84. There was a weak relationship between the dB SL MML and loudness, and between the MML and the THQ. A moderate correlation ( r = .44) was found between the THQ and loudness estimates. Conclusions We conclude that the dB SL MML can be a reliable estimate of tinnitus magnitude, with expected standard deviations in trained subjects of about 1.5 dB. It appears that the dB SL MML and loudness estimates are not closely related.

Abstract. Background: Lowered eye blink rate may be a clinically useful indicator of acute, imminent, and severe suicide risk. Diminished eye blink rates are often seen among individuals engaged in heightened concentration on a specific task that requires careful planning and attention. Indeed, overcoming one’s biological instinct for survival through suicide necessitates premeditation and concentration; thus, a diminished eye blink rate may signal imminent suicidality. Aims: This article aims to spur research and clinical inquiry into the role of eye blinks as an indicator of acute suicide risk. Method: Literature relevant to the potential connection between eye blink rate and suicidality was reviewed and synthesized. Results: Anecdotal, cognitive, neurological, and conceptual support for the relationship between decreased blink rate and suicide risk is outlined. Conclusion: Given that eye blinks are a highly observable behavior, the potential clinical utility of using eye blink rate as a marker of suicide risk is immense. Research is warranted to explore the association between eye blink rate and acute suicide risk.

Parenting behaviors – specifically behaviors characterized by high control, intrusiveness, rejection, and overprotection – and effortful control have each been implicated in the development of anxiety pathology. However, little research has examined the protective role of effortful control in the relation between parenting and anxiety symptoms, specifically among adults. Thus, we sought to explore the unique and interactive effects of parenting and effortful control on anxiety among adults (N = 162). Results suggest that effortful control uniquely contributes to anxiety symptoms above and beyond that of any parenting behavior. Furthermore, effortful control acted as a moderator of the relationship between parental overprotection and anxiety, such that overprotection is associated with anxiety only in individuals with lower levels of effortful control. Implications for potential prevention and intervention efforts which specifically target effortful control are discussed. These findings underscore the importance of considering individual differences in self-regulatory abilities when examining associations between putative early-life risk factors, such as parenting, and anxiety symptoms.
