A rare case of primary rectal tuberculosis presented as a rectal mass, indistinguishable clinically and radiologically from advanced rectal carcinoma . Histology failed at first to identify granulomas, acid fast bacilli, or malignancy, but subsequent biopsies showed multiple epithelioid granulomas with caseating necrosis and Langhans giant cells. Given these findings and the high regional incidence of tuberculosis, presumptive diagnosis was given and anti-tubercular drugs were initiated followed by resolution of the mass and symptoms.

Endometrial stromal sarcoma (ESS) is a rare uterine neoplasm infrequently arising in extra-genital sites. Herein, we report an extremely rare case of primary extra-genital ESS of transverse mesocolon occurring in a 51-year-old female presenting with gradually increasing abdominal mass. The clinical diagnosis considered was a gastrointestinal stromal tumor. Intra-operatively, the mass was confined exclusively to the transverse mesocolon. Microscopy revealed a cellular tumor composed of oval to elongate neoplastic cells with hyperchromatic nuclei, inconspicuous nucleoli and were immunoreactive for CD10, progesterone receptor (PR), estrogen receptor (ER), and PAX8; negative for KIT, CD34, SMA, S100, synaptophysin, chromogranin, WT-1, and calretinin. A distinct arborizing network of arterioles along with foci of endometriosis was also seen. We present this case for its extreme rarity and the challenges entailed in its diagnosis.

Ano-rectal melanoma is extremely rare and has dismal prognosis when compared to cutaneous melanomas. It presents as ano-rectal mass and patients usually complain of passing blood in stools. Being, a highly aggressive neoplasm, widespread metastasis is common, which can be seen even at the time of diagnosis. Here, we present a case report of ano-rectal melanoma in a 40-year old male, who presented with complaints of anal growth since one month. Initially, a clinical diagnosis of rectal carcinoma was suspected. However, biopsy of the growth showed histomorphological features of melanoma, which were confirmed on immunohistochemistry.

Electron microscopy and energy dispersive x-ray microanalysis (EDXA) are found to be very useful tools for identification of etiologic agents in pneumoconiosis or interstitial pulmonary disorders. Pulmonary interstitial fibrosis and granulomatosis are frequently associated with occupational and environmental pollution. Numerous reports of pneumoconiosis in various occupations such as coal and gold miners are presented in the literature. However, there is no known documented case of pulmonary changes in workers in the sandpaper industry. This study reports a rare case of pulmonary granulomatosis containing deposits from abrasives of sandpaper diagnosed by using EDXA.
