The 1918 influenza pandemic stands out because of the unusual age pattern of high mortality. In the United States, another feature merits scientific scrutiny: against a historical backdrop of extreme racial health inequality, the pandemic produced strikingly small ratios of nonwhite to white influenza and pneumonia mortality. We provide the most complete account to date of these racial disparities in 1918, showing that, across U.S. cities, they were almost uniformly small. We examine four potential explanations for this unexpected result, including [1] socio-demographic factors like segregation, [2] city-level implementation of non-pharmaceutical interventions (NPIs), [3] exposure to the milder spring 1918 “herald wave,” and [4] early-life exposures to other influenza strains resulting in differential immunological vulnerability to the 1918 flu. While we find little evidence for 1-3, we offer suggestive evidence that racial variation in early-life exposure to the 1889-1892 influenza pandemic shrunk racial disparities during the 1918 pandemic. We also raise the possibility that differential behavioral responses to the herald wave may have protected nonwhite urban populations. By providing a comprehensive description and careful examination of the potential drivers of racial inequality in mortality during the 1918 pandemic, our study provides a framework to consider interactions between the natural history of particular microbial agents and the social histories of the populations they infect.

ABSTRACT During World War I, the 1918 influenza pandemic struck the fatigued combat troops serving on the Western Front. Medical treatment options were limited; thus, skilled military nursing care was the primary therapy and the best indicator of patient outcomes. This article examines the military nursing’s role in the care of the soldiers during the 1918 flu pandemic and compares this to the 2019 coronavirus pandemic.

Objectives. To test whether distortions in the age structure of mortality during the 1918 influenza pandemic in Michigan tracked the severity of the pandemic. Methods. We calculated monthly excess deaths during the period of 1918 to 1920 by using monthly data on all-cause deaths for the period of 1912 to 1920 in Michigan. Next, we measured distortions in the age distribution of deaths by using the Kuiper goodness-of-fit test statistic comparing the monthly distribution of deaths by age in 1918 to 1920 with the baseline distribution for the corresponding month for 1912 to 1917. Results. Monthly distortions in the age distribution of deaths were correlated with excess deaths for the period of 1918 to 1920 in Michigan (r = 0.83; P < .001). Conclusions. Distortions in the age distribution of deaths tracked variations in the severity of the 1918 influenza pandemic. Public Health Implications. It may be possible to track the severity of pandemic activity with age-at-death data by identifying distortions in the age distribution of deaths. Public health authorities should explore the application of this approach to tracking the COVID-19 pandemic in the absence of complete data coverage or accurate cause-of-death data.

Abstract Osteopathic physicians played a pivotal role in treating patients suffering from the H1N1 influenza A virus of the 1918 Influenza Pandemic. This article focuses on case reports and questionnaire answers from the Journal of the American Osteopathic Association (JAOA), now the Journal of Osteopathic Medicine (JOM), and Osteopathic Physician concerning the modalities, techniques, and efficacy of osteopathic treatments of the 1918 pandemic. There are 19,565 patients who are represented in this analysis. The results are compared to the often-cited 110,120 patient cases reported by the JOM in 1920. Several different approaches, including lymphatic and visceral techniques, were widely used at the time, and their historic incorporation into patient treatment is explored. There is a discussion of the geographic location and characteristics of the practices. Statistical breakdown of mortality rate, the most commonly used approaches, somatic dysfunctions commonly treated, physician anecdotes, and other common remedies used by osteopathic physicians, are noted additionally. A comparison is done of the literature regarding the osteopathic approach for COVID-19. The newly analyzed case reports in this article demonstrate a similar mortality rate as in the 1920 JAOA article and illustrate the geographical distribution, treatment approaches, and personal stories of osteopaths during the pandemic.
