La Oxigenación con Membrana Extracorpórea (ECMO en inglés) es un tipo de soporte artificial cardiopulmonar usado para tratar el fallo respiratorio severo ya que permite la oxigenación extracorpórea. Además, en pacientes con fallo cardíaco severo o que han sufrido un infarto, esta técnica provee circulación sistémica. Sin embargo, la ECMO conlleva algunos riesgos, por ejemplo, coagulopatías. El enfermero tiene un papel clave en esta situación.Objetivo: Dejar clara la función científica de los enfermeros en relación con el paciente crítico en tratamiento con ECMO.Método: Llevamos a cabo un estudio usando las bases de datos de la plataforma EBSCO host y siguiendo la metodología propuesta por el Instituto Joanna Briggs. Con base en los descriptores de la MeSH, hemos seleccionado estudios publicados entre el 2009 y 2019, escritos en portugués e inglés. Resultados: Se han incluido 5 estudios en esta revisión. El análisis de los artículos nos permitió verificar que la labor del enfermero se basa en la monitorización, supervisión, dirección y coordinación del cuidado de los pacientes críticos sometidos a ECMO. También se apreció que el uso de protocolos adecuados, la formación de equipos multidisciplinarios y la comunicación adecuada entre los componentes del equipo contribuye a un abordaje eficaz, seguro y de calidad para tratar a los pacientes sometidos a ECMO.Conclusión: El papel del enfermero es fundamental para la calidad y seguridad de los cuidados prestados a los pacientes críticos sometidos a ECMO. Extracorporeal Membrane Oxygenation (ECMO) corresponds to a form of mechanical cardiopulmonary support used to treat severe respiratory failure, since it allows extracorporeal gas exchange. Additionally, in patients with severe heart failure, or experiencing cardiorespiratory arrest, this technique also provides systemic circulation. Nevertheless, ECMO carries some risks, such as clot formation. It is in this context that the nurse has a key role.Objective: To map the available scientific evidence about the nurses’ approach to the critically ill person subjected to ECMO.Method: We conducted a scoping review, using databases accessed through the EBSCOhost platform and following the methodology proposed by the Joanna Briggs Institute. Through the combination of appropriate MesH descriptors, we selected articles published between 2009 and 2019, written in Portuguese or English.Results: Five studies were included in the review. Their analysis allowed us to verify that the nurse’s approach focuses on the monitoring, surveillance, management and coordination of the care provided to critically ill patients subjected to ECMO. It was also found that the adoption of adequate protocols, the training of the multidisciplinary teams and an efficient communication between team members contributed to an effective, safe and high-quality performance when facing patients subjected to ECMO.Conclusion: An appropriate nursing approach is essential to ensure the provision of care with quality, and safety, to the patient subjected to ECMO.

El sueño desempeña un papel fundamental en el crecimiento y desarrollo de los recién nacidos, su privación presenta numerosos efectos negativos. El ambiente de las unidades de cuidados intensivos neonatales (UCIN) se presenta como perturbador y nocivo del sueño neonatal.Objetivo: Mapear la evidencia científica existente en cuanto a las estrategias promotoras del sueño de los recién nacidos en contexto de UCIN. Metodología: Scoping review basada en la metodología propuesta por el Joanna Briggs Institute, utilizando los siguientes motores de búsqueda y bases de datos: MEDLINE via PubMed, CINHAL Plus with Full Text e Nursing & Allied Health Collection: Comprehensive via EBSCO-HOST, Academic Search Complete, ScienceDirect e Directory of Open Access Journals via B-ON, LILACS, RCAAP e SciELO. Definido como limitadores artículos publicados en los últimos 5 años (hasta febrero de 2019), disponibles en portugués o inglés y en texto completo.Resultados: Se localizaron 186 registros, y la muestra final incluyó 9 estudios que respondieron al objetivo de la revisión sobre la base de los criterios de inclusión definidos. Las estrategias promotoras del sueño identificadas remiten a la gestión del ambiente en la UCIN, concentración de los cuidados de enfermería respetando el ciclo de sueño / vigilia de los recién nacidos, posicionamiento adecuado, contención, succión no nutritiva, técnica de Yakson, toque humano suave, canguro materno, masaje, estímulos auditivos tranquilos y colchón modelador. Conclusión: Dada la importancia del sueño en el desarrollo de los recién nacidos, es fundamental el papel del Enfermero a través de la adopción de estrategias protectoras y promotoras de éste, especialmente en contexto de UCIN. Sleep plays a fundamental role in the growth and development of newborns, with their deprivation having many negative effects. The environment of the neonatal intensive care units (NICU) presents itself as disturbing and harmful for the neonatal sleep.Objective: Map the existing scientific evidence regarding the strategies promoting newborn’s sleep in the NICU setting.Methodology: Scoping review based on the methodology proposed by the Joanna Briggs Institute, using the following search engines and databases: MEDLINE via PubMed, CINHAL Plus with Full Text and Nursing & Allied Health Collection: Comprehensive via EBSCO-HOST, Academic Search Complete, ScienceDirect and Directory of Open Access Journals via B-ON, LILACS, RCAAP and SciELO. As search limiters, there were defined articles published in the last 5 years (until February 2019), available in Portuguese or in English and in full text.Results: A total of 186 results were listed, and the final sample included 9 studies that responded to the review objective based on the selected inclusion criteria. The sleep-promoting strategies identified refer to the management of the environment in the NICU, the concentration of Nursing care respecting the newborn’s sleep-wake cycle, and non-pharmacological interventions such as proper positioning, physical restraint, non-nutritive sucking, Yakson technique, gentle human touch, kangaroo mother care, massage, calm auditory stimuli and remolding mattress.Conclusion: Given the importance of sleep in the newborn’s development, the nurse’s role is crucial through the implementation of protective and promoting sleep strategies, especially in the NICU. O sono tem um papel fundamental no crescimento e desenvolvimento dos recém-nascidos, com a sua privação a apresentar inúmeros efeitos negativos. O ambiente das unidades de cuidados intensivos neonatais (UCIN) apresenta-se como perturbador e nocivo do sono neonatal.Objetivo: Mapear a evidência científica existente quanto às estratégias promotoras do sono do recém-nascido em contexto de UCIN. As estratégias promotoras do sono identificadas remetem para a gestão do ambiente na UCIN, concentração dos cuidados de Enfermagem respeitando o ciclo de sono-vigília dos recém-nascidos, posicionamento adequado, contenção, sucção não-nutritiva, técnica de Yakson, toque humano suave, canguru materno, massagem, estímulos auditivos calmos e colchão modelador.Conclusão: Dada a importância do sono no desenvolvimento dos recém-nascidos, é fulcral o papel do Enfermeiro através da adoção de estratégias protetoras e promotoras deste, especialmente em contexto de UCIN.

Transportar a un paciente crítico conlleva una serie de riesgos que pueden poner en peligro su seguridad. Conocer los riesgos asociados con el transporte intrahospitalario es esencial para mejorar su seguridad. Elegimos estudiar la lista de verificación de transporte intrahospitalario, ya que la literatura los describe como una forma práctica y simple de aumentar la seguridad.Objetivo: Mapear la envidencia científica existente sobre los aspectos del check list o lista de verificación garantizando la seguridad del paciente crítico en el transporte intrahospitalario. Material y método: Se realizó un scoping review através de la metodologia The Joanna Briggs Institute, utilizando los motores de búsqueda EBSCOhost y B-on. La investigación fue realizada en portugués, inglés y español. Fueron seleccionados artículos con texto libre, sin límite de tiempo. Los criterios incluídos: el paciente crítico adulto/anciano, transporte intrahospitalario, la lista de verificación y la seguridad del paciente.Resultados: Se incluyeron 7 artículos. La mayoría de los aspectos de la lista de verificación se enfoca en la preparación del transporte, el aspecto que más se menciona es la monitorización del paciente. La parte después del transporte, aspectos de la lista de verificación solo es mencionada en 3 de los 7 artículos.Conclusión: Todos los estudios constatan aspectos que pueden incluírse en una lista de verificación y que mejoran la seguridad de los pacientes críticos en el transporte intrahospitalario. No hay unanimidad en cuanto a qué aspectos incluir en la lista de verificación. Transporting critically ill patients is has a set of risks that may jeopardize their safety. Knowing the risks associated with intrahospitalar transport of critically ill patients is essential to improving patient safety. For the sake of improving patient safety, was chosen to approach it as an intrahospitalar transport checklist, as the literature describes it as a practical and simple way to increase safety.Objective: Map available scientific evidence regarding aspects of a checklist that ensure the safety of critically ill patients in intrahospitalar transport.Material and Method: A scoping review was performed following the methodology proposed by The Joanna Briggs Institute in databases using the EBSCOhost and B-on search engines. Conducted research in Portuguese, English, and Spanish. Selected free full text articles, with no time limit. Inclusion criteria: adult/elderly critically ill patient, intrahospital transport, a checklist and patient safety.Results: Included 7 articles for analysis. Most verification requests that ensure the safety of critical patients on intrahospitalar transport concern the transport preparation phase, in which patient is monitored was the most mentioned aspect. From the post-transportation phase, checklist aspects are only available in 3 of 7 articles Conclusion: All studies address aspects that improve the safety of critically ill patients in intrahospitalar transport and are likely to be included in the checklist. There is no unanimity as to which aspects to include in the checklist. O transporte do doente crítico acarreta um conjunto de riscos para a sua segurança. Conhecer os riscos associados ao transporte intra-hospitalar do doente crítico é essencial para melhorar a segurança do mesmo. Optou-se por abordar a checklist de transporte intra-hospitalar, uma vez que a literatura a descreve como uma forma prática e simples de aumentar a segurança.Objetivo: Mapear a evidência científica disponível referente aos aspetos de uma checklist que garantem a segurança do doente crítico no transporte intra-hospitalar.Material e Método: Realizou-se uma scoping review recorrendo à metodologia do The Joanna Briggs Institute, nas bases de dados utilizando os motores de busca EBSCOhost e B-on. Realizada pesquisa em Português, Inglês e Espanhol. Selecionados artigos free full text, sem limite temporal. Critérios de inclusão: o doente crítico adulto/idoso, o transporte intra-hospitalar, a checklist e a segurança do doente.Resultados: Foram incluídos 7 artigos. A maioria dos aspetos da checklist que garantem a segurança do doente crítico no transporte intra-hospitalar, dizem respeito à fase da preparação do transporte, na qual o aspeto mais mencionado é a monitorização do doente. Da fase posterior ao transporte, aspetos da checklist apenas são mencionados em 3 dos 7 artigos.Conclusão: Todos os estudos abordam aspetos que são passíveis de incluir em checklist e melhoram a segurança do doente. Não existe unanimidade quanto aos aspetos a incluir na checklist do transporte intra-hospitalar.

This study aimed to evaluate the efficacy of Benson’s relaxation technique on anxiety and depression among patients undergoing hemodialysis. A systematic review of randomized controlled trials was conducted. Searching databases included EBSCO Host, PubMed, ProQuest, Science Direct, Sage Journals, Ovid, and Google Scholar from January 2000 to September 2020. Five randomized controlled trials were identified. Findings revealed a statistically significant decrease in anxiety score. Concerning depression; one RCT showed a statistically significant decrease and two RCTs revealed a non-significant difference in depression level. The overall quality of the RCTs was not high. Despite that the RCTs showed benefits of BRT in managing anxiety. However, this is not the same for depression. The lack of high-quality scientific evidence supporting its retinue use indicates that additional well designed randomized controlled trials in multiple countries are warranted to support the efficacy of Benson’s relaxation technique on anxiety and depression among patients undergoing hemodialysis.

AbstractThe number of critically-ill coronavirus disease 2019 (Covid-19) patients requiring mechanical ventilation is on the rise. Most guidelines suggest keeping the patient intubated and delay elective tracheostomy. Although the current literature does not support early tracheostomy, the number of patients undergoing it is increasing. During the pandemic, it is important that surgeons and anesthesiologists know the different aspects of tracheostomy in terms of indication, procedure, tube care and complications. A literature search was performed to identify different guidelines and available evidence on tracheostomy in Covid-19 patients. The purpose of the present article is to generate an essential scientific evidence for life-saving tracheostomy procedures.

IntroductionSport-related repetitive subconcussive head impacts (RSHIs) are increasingly thought to be associated with adverse long-term outcomes. However, owing to potentially subtle effects, accurate assessment of harm to the brain as a consequence of RSHI is a major challenge and an unmet need. Several studies suggest that biofluid markers can be valuable objective tools to aid the diagnosis and injury characterisation and help in medical decision-making. Still, by and large, the results have been limited, heterogeneous and inconsistent. The main aims of this scoping review are therefore (1) to systematically examine the extent, nature and quality of available evidence from studies investigating effects of RSHI on fluid biomarkers and (2) to formulate guidelines and identify gaps with the aim to inform future clinical studies and the development of research priorities.Methods and analysesWe will use a comprehensive search strategy to retrieve all available and relevant articles in the literature. The following electronic databases will be systematically searched: MEDLINE (EBSCO host; from 1809 to 2020); Scopus (from 1788 to 2020); SPORTDiscus (from 1892 to 2020); CINAHL Complete (from 1937 to 2020); PsycINFO (from 1887 to 2020); Cochrane Library (to 2020); OpenGrey (to 2020); ClinicalTrials.gov (to 2020) and WHO International Clinical Trials Registry Platform (to 2020). We will consider primarily biomedical studies evaluating the biofluid markers following RSHI. Two independent reviewers will screen articles for inclusion using predefined eligibility criteria and extract data of retained articles. Disagreements will be resolved through consensus or arbitrated by a third reviewer if necessary. Data will be reported qualitatively given the heterogeneity of the included studies. In synthesising the evidence, we will structure results by markers, sample types, outcomes, sport and timepoints.Ethics and disseminationEthics approval is not required. We will submit results for peer-review publication, and present at relevant conferences.

Purpose – The purpose of this paper was to study how psychiatric doctors practise leadership in multidisciplinary healthcare teams. The paper seeks to answer the question: How do psychiatric doctors lead multidisciplinary teams during treatment conferences? Design/methodology/approach – Six psychiatric teams were studied at a university hospital. Each team was observed over a period of 18 months, and data were collected during four years (2008-2011). Data were collected through interviews with doctors (n = 19) and observations (n = 30) of doctors’ work in multidisciplinary psychiatric teams. Findings – Doctors in a multidisciplinary team use either self-imposed or involuntary leadership style. Oscillating between these two extremes was a strategy for handling the internal tensions of the team. Research limitations/implications – The study was a case study, performed during treatment conferences at psychiatric wards in a university hospital. This limitation means that there is cause for some caution in generalising the results. Practical implications – The results are useful for understanding leadership in multidisciplinary medical teams. By understanding the reversible logic of leadership, cooperation and knowledge sharing can be gained, which means that a situation of mere peaceful coexistence can be avoided. Understanding the importance of the informal contract makes it possible to switch leadership among team members. A reversible leadership with an informal contract makes the team less vulnerable. The team’s professionals can thus easily handle difficult situations and internal tensions, facilitating leadership and management of multidisciplinary teams. Originality/value – Doctors in multidisciplinary psychiatric teams use reversible leadership logic.

This chapter explores the concept and practice of teamwork and interprofessional collaboration in the support and treatment of clients with mental health problems. Mental health care provision is complex, ethically challenging, and frequently delivered via mental health care teams (MHCT) in both primary and secondary health care settings. We consider how such teams may work together optimally using values-based and client-centered approaches. We discuss the nature of and reasons for conflict arising in multidisciplinary MHCTs, focusing on ethical dilemmas that occur where there is diversity amongst team members in respect of personal, professional, and/or organizational values. The specific ethical issues discussed are: boundary issues; receiving gifts; confidentiality, and involuntary treatment and restraint. Three case studies are used to provide examples of values in action.

Introdução: A principal causa de mortalidade pelo novo coronavírus é a insuficiência respiratória. Assim, os cuidados intensivos devem ser prontamente prestados ao paciente crítico. Objetivo: Explorar as evidências acerca dos achados clínicos, tratamento e desfecho de pacientes infectados pelo Sars-CoV-2 internados em Unidades de Terapia Intensiva. Método: Revisão de escopo, realizada em abril de 2021, em oito fontes de dados nacionais e internacionais, conforme orientações do Instituto Joanna Briggs, seguindo o checklist Preferred Reporting Items for Systematic reviews and Meta-Analysesextension for Scoping Reviews, sem restrição de idioma ou temporal. Adotou-se a estratégia Population, Concept and Contextpara a elaboração da questão de pesquisa. Resultados: Incluíram-se 15 artigos científicos, com predominância de publicações na China, Estados Unidos da América e Canadá. Dos estudos, 80% foram com adultos e idosos em Unidade de Terapia Intensiva. Os principais achados clínicos foram febre, tosse, Síndrome do Desconforto Respiratório Agudo e lesão renal, tratamento com ventilação mecânica invasiva e não invasiva, oxigenoterapia de alto fluxo e corticoesteroides. Como principal desfecho, o óbito. Conclusão: A maioria dos pacientes apresentou febre, tosse e Síndrome do Desconforto Respiratório Agudo, recebendo cuidados como ventilação mecânica, oxigenoterapia de alto fluxo e corticoesteroides, com alto índice de óbitos.
