Purpose. To determine immunological and virological failure and associated factors among children infected with human immunodeficiency virus receiving antiretroviral treatments at Hawassa University Hospital, Southern Ethiopia. Methods. A hospital-based cross-sectional study was conducted among 273 HIV-infected children from July 1 to December 1, 2019. Data were collected using a structured questionnaire and review of patient records. Blood samples for viral load and CD4 count were collected. Data were analyzed using SPSS version 20. Significance group comparison was done by the Kaplan-Meier log-rank test. The Cox proportional hazard model was used to select significant factors of the variability between groups. Results. A total of 273 children, between the age ranges of 1 to 14 years, were included. Of these, 139 (50.9%) and 134 (49.1%) were males and females, respectively. Children from the rural area were almost five times more vulnerable for virological and immunological failure than those children from the urban area ( AOR = 4.912 , (1.276-8.815), P = 0.032 ). The overall viral load suppression was 196 (71.8%) with a good adherence of 226 (82.9%). Nonsuppressed HIV viral load was found to be 77 (28.2%) which had two times more viral load copies ( AOR = 2.01 , (1.21–2.66), P = 0.001 ) when compared to those who had suppressed viral load copies. The proportions of children who had immunological nonresponse were 45.6% (21 out of 46), 30.4% (14 out of 46), and 23.9% (11 out of 46) among children with baseline CD4 of <200, 201-500, and >500 cells/μl, respectively. Unimproved outcomes among females were noted for immunological and virological failure in this study ( AOR = 1.901 , (1.038-3.481), P = 0.038 ). Conclusion. In conclusion, the highly active antiretroviral treatment appeared highly effective in terms of immunological and virological long-term outcomes. However, viral suppression (71.8%) in our study was far apart from the UNAIDS target of 90% in 2020. For that reason, strengthening adherence counseling and early initiation of HAART is important.

Abstract Background Uterine rupture is dropped significantly in the developed world. However, it is still a major public health problem in developing countries including Ethiopia. The aim of this study was to assess management outcome of uterine rupture and its associated factors in Yirgalem General and teaching Hospital of southern Ethiopia. Methods An institution based cross-sectional study was conducted. All records of uterine rupture managed in Yirgalem general and teaching hospital between January 1, 2012, and December 30, 2017, were reviewed. Data were collected using a checklist. Descriptive statistics and logistic regression analyses were done. Results A total of 331 cases of mothers who managed for uterine rupture were included in the study. 235 (71%) of them had a poor management outcome. Wound site infection 131 (39.6%) and anemia 129 (39%) where the common post-operative complications. There were 13 (4%) maternal death and 320 (96.7%) neonatal death. Lack of antenatal care follow up (adjusted odds ratio (AOR) =2.2, 95% CI: 1.1-4.5), prolonged duration of labor more than 24hr (AOR=3.6; 95% CI: 1.7-7.4), patients presented with sepsis (AOR=2.9; 95% CI: 1.4-6.1), mothers who did not transfuse for blood during the case scenario (AOR=4; 95% CI: 2.1-7.9) and prolonged intra operative time (AOR=5.5; 95% CI: 2.8-10.8) were factors associated with poor management outcome of uterine rupture. Conclusion Poor management outcome of uterine rupture was high in the study area as compared to other studies. Lack of ANC follow up, longer duration of the surgical procedure, prolonged duration of labor, lack of blood transfusion and pre and postoperative complications are associated with poor management outcome. Therefore, proper ANC and early identification of the high-risk groups can prevent and decline the occurrence of uterine rupture and that enhance good outcome.

Coagulase-negative staphylococci (CoNS) are a group of commensal microorganisms residing on the skin and mucous membranes of both humans and animals. Until recently, they have been regarded as non-pathogenic to livestock and companion animals, but since then, their clinical importance in veterinary medicine has increased with the discovery of their potential pathogenic roles in animals causing skin and soft tissue infections together with spontaneous abortions and mastitis. Scientific data concerning the presence of CoNS in North Cyprus are very limited. Therefore, the purpose of the study reported herein was to investigate the presence and antimicrobial resistance patterns of CoNS species isolated from various animals presented at the Veterinary Teaching Hospital in North Cyprus between July 2018 and 2019. Staphylococci were isolated from 37.0% (87/235) of the samples submitted, within which 60.9% (53/87) and 39.1% (34/87) were identified as coagulase-positive staphylococci (CoPS) and coagulase-negative staphylococci (CoNS), respectively. Among the CoNS, S. chromogenes was the most predominantly isolated species (14/34, 41.2%), followed by S. capitis (5/34, 14.7%) and S. simulans (4/34, 11.8%). Of these 34 CoNS, 24 (70.6%) and 10 (29.4%) were identified as MRCoNS and MSCoNS, respectively. The CoNS isolates showed relatively high levels of resistance towards amoxicillin/clavulanic acid (19/34, 55.9%), tetracycline (14/34, 41.2%) and penicillin (13/34, 38.2%). In conclusion, the presence of CoNS, especially MRCoNS, and the detection of multiple drug resistant (MDR) species with a high prevalence were regarded as being important since they might limit and have negative effects on the therapeutic treatment options of staphylococcal infections in animals, and might have both public and veterinary concerns.

Background. Anemia is a widespread public health problem associated with increased risk of morbidity and mortality. Infants, under-5-year-old children, and pregnant women have greater susceptibility to anemia. The magnitude and associated risk factors for anemia vary in different settings. The study aimed to assess the magnitude, severity, and associated factors of anemia at Hawassa University Teaching and Referral hospital, Hawassa, southern Ethiopia. Methods. In a hospital-based cross-sectional study, a total of 422 under-five children were included. Sociodemographic data and other predisposing factors were collected by structured questionnaire. Venous blood samples were collected and analyzed for hemoglobin determination using a Cell-Dyn 1800 automated analyzer. Stool samples were collected and processed using direct wet mount and formol-ether concentration method to detect intestinal parasites. Data were entered and analyzed using SPSS version 20 statistical packages. Binary and multiple logistic regressions were computed to assess factors associated with anemia. p value less than 0.05 was taken as statistically significant. Result. The overall prevalence of anemia was found to be 41.7%. The mean hemoglobin level was 10.59 g/dl. Anemia was of mild, moderate, and severe type in 6.6%, 19%, and 16.1% of the children, respectively. Children in the age group 6–23 months (AOR = 2.04 (95% CI: 1.13, 3.69)), and mothers having no formal education (AOR = 1.73 (95% CI: 0.99, 3.02)) were identified as associated factors for anemia. Conclusion. The prevalence of anemia among the study subjects was 41.7% indicative of the fact that anemia is an important public health problem. It was associated with the child’s age, residence, mother’s education level, and intestinal parasite (Ascaris lumbricoides). It clearly indicates that there should be well integrated public health interventions to improve the health status that needs to be prioritized to prevent anemia among children under five years of age.
