Hypnosis does not always require suggestions of relaxation in order enter into this state. It can also be induced through suggestions of activation and cognitive alertness. This procedure and the hypnotic state caused by it has been called active-alert hypnosis (AAH). In this chapter, we describe a strategy to increase the probability to achieve goals using an AAH technique in which we ask the patient to move his arms in an alternate way, while imagining that he has a pair of dumbbells of several kilograms in each hand, in order to produce a hypnotic age progression phenomenon, in which the patient is oriented to a positive future and mobilizing hope, and could see himself achieving his goals, creating “memories of the future.” We report several clinical cases in which this hypnotic strategy was used.

Chronic obstructive pulmonary disease (COPD) is one of the most important problems of modern medicine associated with a high mortality rate, high costs of treatment and relief of exacerbations of COPD. The main objectives of COPD treatment are symptom control, reduce the frequency of exacerbations and hospitalizations, and reduced risk of exacerbation in the future. The recommendations of the GOLD initiative propose a treatment approach based on the assessment of exacerbation rates external respiratory function indicators (spirometric classification of GOLD), the severity of symptoms assessed on the CAT test and mMRC. When choosing therapy, the physician must first of all take into account the effectiveness, safety of the drug, adherence to treatment in order to achieve the therapeutic goals of treating patients with COPD. The change in therapeutic approaches in COPD treatment is associated with the accumulation of knowledge in physiology, clinical pharmacology, and the isolation of new clinical phenotypes of COPD. Currently, the main classes of drugs for the treatment of COPD are long-acting beta-agonists (LABA), longacting anticholinergics (LAMA), and inhaled glucocorticosteroids (ICS). The evolution of therapeutic approaches in COPD treatment has led to the creation of new fixed inhalation combinations of the main groups of drugs for COPD treatment. The therapeutic strategies recommended by GOLD and the Russian Federal Guidelines determine the long-term goals of COPD treatment – the impact on the risk of exacerbations in the future. The presented clinical observation of a patient with severe COPD demonstrates the effectiveness of a triple fixed combination vilanterol/umeclidinium/fluticasone furoate 55/22/92 μg as a basic therapy. The  chosen treatment strategy not only reduces the  severity of  the  symptoms of  the  disease, but also reduces the  risk of exacerbations in the future.

Paraneoplastic syndromes are the rare states caused by a change in the immune system's response to neoplasms. These include non-metastatic systemic effects that accompaniy malignant diseases. Diagnosis of these syndromes has some difficulties. In this article clinical cases of patients with paraneoplastic syndromes are analyzed and differential diagnosis is performed. Careful analysis of each clinical case is extremely important for the accumulation of experience, improvement of diagnostics and avoiding mistakes in the future.

SUMMARYA study of diphtheria in Iran was undertaken during the summer of 1969. Diphtheria was found to be more common among the younger people and to reach its peak 3 months after the start of the school term, during the coldest period of the year. Owing to the liberal use of antibiotics a number of modified clinical cases were observed. Of the strains isolated, 89·7% were gravis, 1·3% intermedius and 9 % mitis. Phage typing of these strains showed that the mitis and intermedius strains could only be typed by adapted phages and the majority of gravis strains were phage type XIV, which is the epidemic type seen in other countries.As the mass immunization campaign reduces the incidence of diphtheria in Iran, the epidemiological pattern of the disease will change as seen in the U.K. and U.S.A. It is, therefore, suggested that in the future further studies of diphtheria in Iran be undertaken in order to provide us with information about the changing epidemiological pattern of the disease.

This chapter describes the four domains of Managing Cancer and Living Meaningfully (CALM), which represent the general content themes that patients are given the opportunity to address over the course of CALM therapy. These domains—namely, symptom management and communication with healthcare providers; changes in self and relations with close others; spirituality or sense of meaning and purpose; and thinking about the future, hope, and mortality—are discussed here in detail. The general therapeutic goals of exploration in each domain are outlined, with suggested probes to initiate discussion. Case examples, which include potential therapeutic responses and non-CALM responses, are provided to enhance understanding of therapeutic intent within each domain.
