In standard cataract surgery, one of the major goals is to reach target refraction. Based on keratometry measurements, axial length and anterior chamber depth, most of the intraocular lens calculation formulae are suitable to achieve this aim. Further evaluation of corneal refractive parameters like anterior and posterior corneal surface by Scheimpflug devices led to a significant enhancement of precision in astigmatic and post-refractive surgery cases.

Important physiological feature of the anatomically «short» eye is the disproportionately large volume of the lens relative to the volume of the eye. A gradual and continuous increase in the volume of the lens contributes to the anterior displacement of the irido-lenticular diaphragm, causing the functional block of the anterior chamber angle. This leads to persistent disorders of the hydrodynamics of the eye, up to the development of an acute attack of angle-closure glaucoma. Purpose. To study incidence of increased risk formation of primary angle-closure glaucoma (PACG) and its acute attack in young patients with moderate and high hyperopia (Hm). Material and methods. Clinical material was presented by 58 patients (116 eyes) with indicators of axial length of the eyes less than 23 mm. The main group consisted of 33 patients (66 eyes) with moderate and high Hm. The comparison group was represented by 25 patients (50 eyes) with PACG. Results. The study made it possible to identify group of increased risk of the formation of PACG, which included 19 people (58%), from selected general population of young patients with moderate and high Hm. Conclusion. Comparative analysis of morphometric parameters of structures of the anterior segment of the anatomically «short» eyes in young patients with moderate and high Hm, on the one hand, and in patients with PACG, on the other, revealed statistically significant differences between them in the anterior chamber depth in the central zone, the thickness of the lens, the anterior chamber volume, the peripheral anterior chamber depth, of the anterior chamber angle. <p>Anterior segment eye parameters are essential factors in diagnosis, screening and management of abnormal ocular conditions. Based on the previous studies, they might differ from one race or population to another. Sistan-and-Baluchestan province, the southeast of Iran, has special weather conditions and race, plus lack of research on these diagnostic factors. Hence, the objective of the present study was to assess anterior segment parameters using pentacam in this area. 800 eyes of subjects which had been referred to the Al-Zahra eye hospital of Zahedan, the capital city of the province, for corneal refractive surgery from October 2014 to March 2015 participated in this research. 95% confidence limits for mean of central corneal thickness, anterior chamber depth and volume were (536.02, 541.20), (3.13, 3.18) and (187.63, 192.58) respectively. Multiple linear regression models showed a lower mean central corneal thickness, and maximum/minimum of keratometry, for males than females, adjusting for age and spherical equivalent. Inversely, anterior chamber depth, and volume were more in males. In order to diagnosis and treating ocular diseases which have effect on retinal thickness, precisely specification of predictive factors is highly needed.</p>

AIM: To compare the anterior chamber volume and corneal volume using Pentacam in people with early keratoconus, subclinical keratoconus, and healthy subjects. METHODS: This epidemiologic study was performed on 63 patients who were candidates for refractive surgery. We classified our patients into three groups as normal, subclinical keratoconus, and early keratoconus according to the Amsler-Krumeich criteria. We collected demographic information, including age and sex, and obtained a full medical history of the patients. Complete visual examination was performed for all patients. Then, using Pentacam, corneal volume and anterior chamber volume were measured among three groups. RESULTS: Corneal volume in the control group was significantly higher than the subclinical keratoconus and early keratoconus groups (P<0.05), but the anterior chamber volume was not significantly different between the groups (P>0.05). CONCLUSION: Measuring the corneal volume can help to distinguish the eyes with subclinical keratoconus and early keratoconus from normal eyes. The chamber volume might not differ remarkably among people with subclinical keratoconus, early keratoconus, and those with normal eyes.

Iris pigment epithelial (IPE) cysts are a subset of iris cysts that arise from the IPE. They are spontaneously erupting epithelial-lined cavities that are found in various anatomic locations of the iris, including the iris pupillary margin, midzone, periphery, and free floating in the vitreous or anterior chamber. We report the case of an asymptomatic 13-year-old boy with an incidental finding of a dislodged anterior chamber cyst diagnosed on routine examination. Modern multimodal image analysis of the cyst including anterior segment optical coherence tomography and ultrasound biomicroscopy (UBM) was utilized to characterize the microstructural anatomy of the lesion. The patient was managed conservatively without complications. Cysts of the IPE typically do not affect vision or ocular health and can be monitored and observed after ascertaining no associated malignancy. Initial diagnostic investigation can include UBM and anterior segment optical coherence tomography. Intervention should be reserved only for cases where the cyst growth leads to obstruction of the visual axis and/or other secondary complications.
