Abstract Background Some evidence has been found that malignant breast tumors have lower electrical impedance than surrounding normal tissues. Electrical impedance could be used as an indicator for breast cancer detection. Methods Is a prospective, cross-sectional epidemiological observational study of serial screening. The women were invited to participate and signed their consent letter. The purpose of our study was to analyze the sensitivity and specificity of electrical impedance mammography (EIM) and its implementation for the differential diagnosis of pathological lesions of the breast, either alone or in combination with mammography / ultrasound, in 1200 women between 25 to 70 years old. Results We found a sensitivity of 85 % and a specificity of 96 %, the positive predictive value was 12% and 99% negative predictive value. Seven biopsy confirmed cancers. Significant correlation between electrical conductivity index and Body mass index (BMI) (p<0.05) and patient's age was observed (p=0.01). We also observed that the average of the conductivity distribution is increasing according to the age group (p <0.01). We used chi-squared test to assess for interactions between percent density mammary and BMI (normal <25 kg/m2 (n= 310), overweight 25-29.9 kg/m2 (n= 418) and obese ≥ 30 (n=437) (p<0.05). The patients with a diagnosis of mammary carcinoma had a BMI of 35.51 kg / m2. Conclusions Our study showed that the sensitivity and specificity of Mono Frequency Electrical Impedance Mammography (EIM) has 85% and 96% respectively. These results suggest that breast electroimpedance can be used in women with obesity and dense breasts since these factors generate an effect that leads to a high rate of cancers at intervals, especially in young women. This report describes an evaluation of »observer variation« in history taking and examination of patients with abdominal pain. After an initial survey in which the degree of observer variation amongst the present authors fully confirmed previous rather gloomy forecasts, a system of »agreed definitions« was produced, and further studies showed a rapid and considerable fall in the degree of observer variation between the data recorded by the same authors. Finally, experience with a computer-based diagnostic system using the same system of agreed definitions showed the maximum diagnostic error rate due to faulty acquisition of data to be low (4.7°/o in a series of 552 cases). It is suggested as a result of these studies that — at least in respect of abdominal pain — errors in data acquisition by the clinician need not be the prime cause of faulty diagnoses.

An internationally compatible medical thesaurus in German, the TdM, is presented. The principle of its structure is based on a documentation language including a complete hierarchic classification as well as a very variable faceted one. The structure items of the thesaurus are marked by abbreviations constructed mnemotechnically by letter corresponding to the hierarchic classification, the digits to the facet division. The organisation of the facets is done in such a way that they can be regarded as additional aspects within the arrangement of the hierarchic classification. The aspect of the meaning of a diagnosis is marked by a string, every item consisting of several strings. The number of strings is unlimited in general. The codification structures of the ICD/E, KDS and SNOP are included in every diagnosis. The thesaurus has a total volume of about 22,000 diagnoses.

The relevance of the study under consideration is due to the need to increase the efficiency of students independent work in solving computational problems. A theoretical rationale is proposed and the practical implementation of an automated training and diagnostic system for the formation of skills to solve problems according to the “white box” model is described. The leading idea of the study is the construction of mental schemes for a given topic, which allow to visualize the dynamics of changes in the learner’s level of ability to solve computational problems. The methods of accounting for forgetting educational information and methods of personalized selection of tasks are substantiated.
