The causes of hematuria, both gross and microscopic, in the pediatric patient are numerous and confusing. The pediatrician should develop an organized, logical sequence of diagnostic steps, the keystones of which are (1) a careful review of the history, (2) physical findings, and (3) simple, inexpensive, minimally invasive laboratory and radiologic tests. If serious pathology is suspected, early referral to a nephrologist or urologist is urged. In these instances, renal biopsy or invasive urologic evaluation is probably inevitable.
