The usual modes of spread of ovarian epithelial carcinomas are by exfoliation of cells into the peritoneal cavity and lymphatic dissemination to retroperitoneal lymph nodes. Primary presentation of ovarian carcinoma due to neurologic symptoms of spinal cord compression is extremely rare. We report an unusual case of epithelial ovarian carcinoma who primarily presented with paraparesis due to spinal extradural metastasis.
