This is a rare case report of acute pancreatitis secondary to a massive incarcerated paraoesophageal hernia. The pathogenesis resulted from obstruction of the distal pancreatic duct after displacement of the pancreatic head and body into the thorax as part of a Type IV paraoesophageal hernia. Although this condition is rare, the patient made steady progress following laparotomy and open repair of hernia. She made a good recovery after prompt therapy, therefore, this report can be a guide to the diagnosis and treatment of similar conditions.

Association of hypercalcemia with SLE is very rare. Since initial report in 1991 only hand full cases have been described mainly in adult patients sporadically. Three pattern observed in this rare presentation. Commonly it is associated with serositis and lymphadenopathy or lymphedema known as hypercalcemia-lymphadenopathy SLE {HL-SLE) or hypercalcemia- lymphedema syndrome. Second group do not show this particular association. Concomitant primary hyperparathyroidism is responsible for third variety. This report describes a case of hypercalcemia associated with SLE from Bangladesh. ABSTRACTAnomalous pancreaticobiliary ductal union (APBDU) can cause recurrent acute pancreatitis. We describe the case of a 16-year-old boy with recurrent acute pancreatitis. The discussion provides a review of recent literature, supporting use of various diagnostic modalities and surgery as choice of treatment.

Neonatal hypertriglyceridemia is a very rare condition. Diagnosis in neonatal period is very difficult and is usually diagnosed when acute pancreatitis sets in. Early diagnosis is important as it can prevent the complications associated with the condition that is acute pancreatitis and pancreatic necrosis. Here we present a case of neonatal hypertriglyceridemia who presented to us with respiratory distress but was diagnosed early due to the presence of highly viscous and milky blood. This holds importance as early treatment can reduce the complications and morbidity associated with familial hypertriglyceridemia.
