Introduction: The instructive climate in any organization is an important concern to debate, as it assumes a powerful part in overseeing understudies' scholastic accomplishment. Objective: The target of my examination was to determine students’ perception of the educational environment at Khawaja Mohammad Safdar Medical College, Sialkot. Methods: A descriptive, cross-sectional, poll-based study was led in which undergraduate students of all five years were analyzed.50-item DREEM Questionnaire was used, and scoring was based on 5 points Likert scale. Data were analyzed by using SPSS v 25. The obtained score was expressed as the mean ±SD. Results: A total mean score is 114/200 (57%), which is more affirmative than negative. The mean score for Students’ perception of Learning is 28/48, Students’ perception of Teachers 25/32, Students’ academic self perception20/28, Perceptions of atmosphere 27/48, and Students’ social self-perception is 16/28. There were four problem areas; item no.1 (I am encouraged to participate in teaching sessions) (1.78), item no.8(course organizers ridicule their students)(1.42), item no.35(I find experience disappointing)(1.76), and item no.3(there is a sound support system for students who get tense)(1.25) all with means ≤2. Conclusion: The overall educational climate was more valuable than the negative; however, few problem areas need to be further explored, and measures are taken to make the environment more favorable for learning. Climate Study

Climate is the soul and spirit of medical college and students’ experiences of the climate are a useful basis for modifying and improving the quality of educational environment. The present study was undertaken to identify how the students perceive the educational environment in a newly restructured curriculum using the Dundee Ready Education Environment Measure (DREEM). The inventory was administered to 1903 medical students (3rd, 4th and 5th year) of MBBS course adopting purposive sampling. The mean DREEM total score and mean score for five essential domains were found significantly different in different phase of medical students. DREEM total score were found to be highest for 3rd year students (119/200) and lowest for 5th year students (105/200) but all the three groups of students perceive the environment as positive. 3rd year students also scored high than 4th and 5th year students in five domains. All the three groups of students identify problems in environment and give opinion that there are many issues need to improve with special attention to perception of atmosphere and social self perception. The results are the first data obtained from the students about the educational environment and give important feedback to curriculum planners and change managers of the faculty for necessary improvement. OBJECTIVE Craniopharyngiomas can be difficult to remove completely based on their intimate relationship with surrounding visual and endocrine structures. Reoperations are not uncommon but have been associated with higher rates of complications and lower extents of resection. So radiation is often offered as an alternative to reoperation. The endonasal endoscopic transsphenoidal approach has been used in recent years for craniopharyngiomas previously removed with craniotomy. The impact of this approach on reoperations has not been widely investigated. METHODS The authors reviewed a prospectively acquired database of endonasal endoscopic resections of craniopharyngiomas over 11 years at Weill Cornell Medical College, NewYork-Presbyterian Hospital, performed by the senior authors. Reoperations were separated from first operations. Pre- and postoperative visual and endocrine function, tumor size, body mass index (BMI), quality of life (QOL), extent of resection (EOR), impact of prior radiation, and complications were compared between groups. EOR was divided into gross-total resection (GTR, 100%), near-total resection (NTR, > 95%), and subtotal resection (STR, < 95%). Univariate and multivariate analyses were performed. RESULTS Of the total 57 endonasal surgical procedures, 22 (39%) were reoperations. First-time operations and reoperations did not differ in tumor volume, radiological configuration, or patients' BMI. Hypopituitarism and diabetes insipidus (DI) were more common before reoperations (82% and 55%, respectively) compared with first operations (60% and 8.6%, respectively; p < 0.001). For the 46 patients in whom GTR was intended, rates of GTR and GTR+NTR were not significantly different between first operations (90% and 97%, respectively) and reoperations (80% and 100%, respectively). For reoperations, prior radiation and larger tumor volume had lower rates of GTR. Vision improved equally in first operations (80%) compared with reoperations (73%). New anterior pituitary deficits were more common in first operations compared with reoperations (51% vs 23%, respectively; p = 0.08), while new DI was more common in reoperations compared with first-time operations (80% vs 47%, respectively; p = 0.08). Nonendocrine complications occurred in 2 (3.6%) first-time operations and no reoperations. Tumor regrowth occurred in 6 patients (11%) over a median follow-up of 46 months and was not different between first versus reoperations, but was associated with STR (33%) compared with GTR+NTR (4%; p = 0.02) and with not receiving radiation after STR (67% vs 22%; p = 0.08). The overall BMI increased significantly from 28.7 to 34.8 kg/m2 over 10 years. Six months after surgery, there was a significant improvement in QOL, which was similar between first-time operations and reoperations, and negatively correlated with STR. CONCLUSIONS Endonasal endoscopic transsphenoidal reoperation results in similar EOR, visual outcome, and improvement in QOL as first-time operations, with no significant increase in complications. EOR is more impacted by tumor volume and prior radiation. Reoperations should be offered to patients with recurrent craniopharyngiomas and may be preferable to radiation in patients in whom GTR or NTR can be achieved.

Background: To assess the competency of a student different tools are used. Since its introduction in 1975 by Dr. Harden and his team, OSCE has gained tremendous strides to assess the clinical competencies. Since 1975 onward OSCE has been very successfully used to assess the clinical competencies of medical student globally. OSCE is an assessment tool in which student is observed for performance of different tasks at specified stations. In the current study perception of medical students about OSCE examination was done which shall give room for positive criticism and further improvement of the system where ever required. Objective: To expedite view of final year MBBS students of Azra Naheed College about OSCE Material and Method Study design: Quantitative, cross sectional study. Settings: Azra Naheed College, Lahore. Duration: Six months i.e. 1st July2020 to 31st December 2020 Data Collection procedure: After an informed consent and appropriate briefing, the questionnaire was distributed among the final year medical students of Azra Naheed Medical College. Questionnaire developed by Russell et al was used. Results: Out of 148 students who participated in the study, 66(45%) students were females and 82(55%) were male. Majority of the students were satisfied with the quality of the exam. Consensus about the quality of exam was that, 29.7% were aware about the nature of the exam, 52.7% were satisfied that the syllabus taught was asked in the exam, 58.1% were satisfied about the time allocation for each station. Majority i.e. 60% considered OSCE an exam of practical nature which is not biased by gender or ethnicity. More than 50% of the students were satisfied with the standard of the exam. At the same time more than 50% students considered essay exam the easiest format of assessment. However, OSCE was considered to be fairest form of assessment 73%. 68.9% perceived that learning is enhanced by MCQs rather than other formats of assessment. Conclusion: To conclude this study, it is very much clear that the perception of students about OSCE as an assessment tool was very encouraging, as it not only provided them the opportunity to highlight their weaknesses but also helped them to perform well in the exam, manage time during exam and to overcome them stress which influenced their results. Prostate cancer is the number one cancer by incidence in males of increasing age & second by mortality throughout the world. Many known etiological factors are associated with increased risk of prostatic carcinoma including male hormone & its active metabolites, genetic predisposition (BRCA2, inherited polymorphism of some transcription factors), racial difference, environmental factors, food habits etc, Recently an association of importance has been given to molecular level changes; inflammatory cytokines, trace elements like zinc with prostate cancer. Diagnostic modalities & treatment facilities are important for better management & giving improved quality of life to the patients. The present review article highlights some of these findings on prostate cancer namely genetic factors, Highfatty diet, Hormone, medication, sexual activity, inflammatory cytokines, trace element zinc, PSA. Background: Recurrent aphthous Ulceration (RAU) is characterized by necrotizing ulcers of the oral mucosa that persist, remit, and recur for variable periods of time. Despite the benign nature of the disease, RAU frequently affects quality of life as a result of long lasting and recurrent episodes of burning pain.Aim and objective: the aim of the study was to determine the efficacy of colchicine in treatment of recurrent oral aphthous ulcer.Methods: A clinical trial was conducted among 30 samples in the department of dermatology and venereology of Bangabandhu Sheikh Mujib Medical University and Dhaka Medical College Hospital, Dhaka. Study was performed among those who fulfilled the inclusion criteria. Data was collected from 1st September 2007 to 29th February. 2008 and enrolled data was analyzed by using statistical SPSS win 13.Results: Among 30 patients, 24 (80%) were male and 6(20%) were female. Mean age of the patients was 26.20 (±6.65) years. 26(87.7%) patients found ulcer in oral mucosa and 4(13.3%)in tongue, but previous family history was present in only 20% cases out of 30 patients. All patients experienced a marked decrease in symptoms during colchicine treatment in where; mean aphthae count was 3.27 (±1.05) before treatment and 0.43 (±0.68) during treatment. Pain score also reduced during treatment from 6.80 (±2.12) to 1.13 (±.1.11) which was statistically highly significant.Conclusions: Colchicine may be a promising modality of treatment in the therapeutic armamentarium for RAU.DOI: http://dx.doi.org/10.3329/bmj.v39i3.9948 BMJ 2010; 39(3)

Background: Maternal childbirth expectations play an important role in determining a woman’s response to her childbirth experience. Women need to be helped to develop realistic and positive expectations and identify the factors that influence these expectations.Objective: The aim of this study was to compare woman’s expectations and experiences of childbirth.Methods: This descriptive-comparative study was carried out in Tabriz Alzahra Hospital from 2006 to 2007. For this purpose, a total of 600 primiparas and multiparas women who were candidates for vaginal delivery, were randomly selected and interviewed. The data were collected by questionnaire.Results: Comparison of the means of mothers expectation and experience of labor and birth between the two groups demonstrated a statistically significant difference (p<0.05). The findings indicated a number of differences exist between primiparas and multiparas women in relation to expectations and experiences of birth when compare two groups. The majority of women had negative expectations and experiences of childbirth.Conclusion: The evaluation and understanding of birth expectations and experiences as positive and negative is priority of maternity system. Antenatal educators need to ensure that pregnant women are appropriately prepared for what might actually happen to limit this expectation-experience gap. Background: Primary documentation of a patient is crucial for making effective healthcare decision and improvements in the quality of care. The objective of this study was to assess the quality of current documentation practice in tertiary care hospitals. Materials and methods: This was an assessment of medical documentation practice of one year from the period of January 2010 to December 2010 in Chitwan Medical College, Teaching Hospital. Total 184 patients' discharge files were enrolled and reviewed. Documentation was reviewed in its quality such as completeness, Coherent, consistency and Legibility.Results: In overall pooled analysis, High omission rate was observed in final diagnosis, results (cure, improved, referral and death), hospital stay, and final case summary. Although, satisfactory performance was observed in complete set of forms (72.2%); Patient consent for treatment &release authorization forms (78.2%) and treatment chart (60.8%), the overall pooled performance in ten components showed50% performance gap. Study demonstrated that documentation and its legibility, coherent and consistency in all departments needs substantial improvements in the institution.JNGMC Vol. 12 No. 2 December 2014, Page: 11-16
