Introduction: Natesto®, testosterone nasal gel (TNG), is a testosterone therapy (TTh) indicated for adult male hypogonadism.1 This study allowed titration decisions to be based on physicians’ assessment of patient symptoms. Methods: Hypogonadal males on active topical testosterone therapy (TThE) or naive to any form of testosterone therapy (TThN) were treated with 22 mg TNG daily (11 mg twice daily) for 90 days. Titration was determined by the physician at Day 90 wherein the dose was increased to 33 mg daily if symptoms were not properly managed. Total testosterone (TT) levels were collected at Day 90 and 120 and the quantitative Androgen Deficiency in the Aging Male (qADAM) symptom questionnaire was administered on Days 0, 30, 60, 90, and 120. Results: At study endpoint, 77.0% of all patients were in the normal TT range. Mean qADAM scores increased from 30.8 at baseline to 35.5 (6.6) at Day 90. Physician assessments resulted in 37% patients being up-titrated for an additional 30 days, however, qADAM scores did not change significantly at the higher dose. Conclusions: The majority of patients achieved the normal range of testosterone with TNG when physicians based their titration decision on an assessment of symptoms. Sexual function and energy-related symptoms were predictive of improvements resulting from treatment. These symptoms were the most relevant indicators for physicians in making decisions relating to titration.

Abstract : Nowadays there is an increasing prevalence of elder people all around the world, including Indonesia. With increasing age, several health problems occurred, such as the influence of older age to sexual desire which is commonly occurred in elder men. This is a descriptive-observational study, which is aimed to evaluate the influence of aging to sexual desire in male. 57 men aged 60-74 years on the area of Prof Dr. R. D. Kandou Hospital from November 2013 to January 2014 were involved in this research. Instruments that were used in this research are ADAM (Androgen Deficiency in Aging Male) questionnaire and AMS (Aging Male Symptoms) questionnaire to assess the sexual function in elder men. This study showed that among 57 respondents aged 60-74 years old, through ADAM and AMS questionnaire, 28 (49%) subjects had mild orgasm dysfunction, 27 (47,4%) subjects had moderate decrease in sexual coitus, 24 (42%) subjects had mild decrease in sexual desire, and 29 (51%) subjects had mild erection dysfunction. Older age influence the sexual function in men. Further  studies using larger samples and complete research procedures, including laboratory examination, are needed to assess the influence of older age to male sexual desire. Keywords : older age, male, sexual desire   Abstrak : Dewasa ini jumlah penduduk usia lanjut (60 tahun ke atas) di seluruh dunia, termasuk di Indonesia, mengalami peningkatan. Seiring dengan peningkatan usia, terjadi berbagai masalah kesehatan, termasuk di antaranya adalah pengaruh usia lanjut terhadap hasrat seksual yang banyak terjadi pada pria usia lanjut. Penelitian ini bertujuan untuk mengetahui pengaruh usia lanjut terhadap hasrat seksual pria. Desain penelitian adalah observasional deskriptif. Sebanyak 57 orang pria usia lanjut berusia 60-74 tahun yang berada di area RSUP Prof. Dr. R. D. Kandou Manado pada periode November 2013 hingga Januari 2014 menjadi subjek di dalam penelitian ini. Instrumen yang digunakan dalam penelitian ini adalah kuesioner ADAM (Androgen Deficiency in Aging Male) dan AMS (Aging Male Symptoms) untuk menilai fungsi seksual pada pria usia lanjut. Hasil penelitian menunjukkan bahwa sebanyak 57 responden dengan rentang usia 60-74 tahun melalui kuesioner ADAM dan AMS didapatkan 28 orang (49%) gangguan orgasme ringan, 27 orang (47,4%) penurunan frekuensi senggama sedang, 24 orang (42%) penurunan hasrat seksual ringan dan 29 orang (51%) gangguan ereksi ringan. Usia lanjut berpengaruh terhadap hasrat seksual pada pria. Studi lanjutan dengan jumlah sampel lebih banyak dan prosedur penelitian lebih lengkap, mencakup pemeriksaan laboratorium, diperlukan untuk meneliti lebih jauh mengenai pengaruh usia lanjut terhadap hasrat seksual pria. Background: Androgen deficiency in aging male (ADAM) is characterized by hypogonadism with symptoms such as reduced sexual desire, muscle mass loss, among others. The treatment with testosterone gel and the results around weight and other indicators are the research objects. Objective: Analysis of association between ADAM and obesity in the outpatient population studied and to verify the outcomes of the testosterone gel treatment. Materials & Methods: From a group of 126 outpatients of University Hospital Mário Palmério, 40 were selected with total testosterone lower than 300ng/dL, upon the signature of written informed consent form and the realization of laboratory tests. After new testosterone dosage and urologic evaluation, 6 patients were treated for an average time of 6 months. Initially the dosage was 50 mg/day, with medical consultations and laboratory tests to verify the effects and to adjust the dosage. The variables analyzed were BMI, abdominal circumference, weight, muscle mass, body and visceral fat, in addition to testosterone serum dosages, lipid profile, amongst others, considering the test t among the beginning and end of treatment, defining the significance level by p<0,05. Results: Significant increase of total testosterone levels (p<0,001) and a tendency of improvement in the free testosterone levels (p=0,061) and no significant reduction of BMI (p=0,4308), abdominal circumference (p=0,1695), weight (p=0,999), body fat (p=0,194) and muscle mass (p=0,632), while visceral fat increased (p=0,5265). Vitamin D had no significant increase (p=0,2422). Conclusion: The total testosterone level was increased after the testosterone gel treatment with statistical significance, however there must be new research with more subjects with ADAM to prove the benefits of this treatment.

Male hypogonadism is “a clinical syndrome that results from failure of the testis to produce physiological concentrations of testosterone and/or a normal number of spermatozoa due to pathology at one or more levels of the hypothalamic– pituitary–testicular axis”. The diagnostic protocol of male hypogonadism includes accurate medical history, physical exam, as well as hormone assays and instrumental evaluation. Basal hormonal evaluation of serum testosterone, LH, and FSH is important in the evaluation of diseases of the hypothalamus-pituitary-testis axis. Total testosterone levels < 8 nmol/l profoundly suggest the diagnosis of hypogonadism. An inadequate androgen status is moreover possible if the total testosterone levels are 8-12 nmol/L. In this “grey zone” the diagnosis of hypogonadism is debated and the appropriateness for treating these patients with testosterone should be fostered by symptoms, although often non-specific. Up to now, no markers of androgen tissue action can be used in clinical practice. The identification of markers of androgens action might be useful in supporting diagnosis, Testosterone Replacement Treatment (TRT) and clinical follow-up. The aim of this review is to analyze the main findings of recent studies in the field of discovering putative diagnostic markers of male hypogonadism in seminal plasma by proteomic techniques. The identified proteins might represent a “molecular androtest” useful as a seminal fingerprint of male hypogonadism, for the diagnosis of patients with moderate grades of testosterone reduction and in the follow-up of testosterone replacement treatment.
