Contracture of the elbow represents a disabling condition that can impair a person's quality of life. Regardless of the event that causes an elbow contracture, the conservative or surgical treatment is usually considered technically difficult and associated with complications. When the conservative treatment fails to restore an acceptable range of motion in the elbow, open techniques have been shown to be successful options. More recently the use of arthroscopy has become more popular for several reasons. These reasons include better visualization of intra-articular structures, less tissue trauma from open incisions, and potentially the ability to begin early postoperative motion. The purpose of this paper is to review the indications, complications, and results of arthroscopic management of a stiff elbow.

Aim. Comparative analysis of the long-term results of surgical treatment of patients with chronic pancreatitis. Methods. The study included patients with complicated clinical forms of chronic pancreatitis including: ductal pancreatic hypertension, main pancreatic duct concrements, marked fibrous restructuring of the pancreatic parenchyma and presence of calcifications, presence of biliary hypertension, portal hypertension syndrome, duodenostasis syndrome, chronic abdominal pain syndrome, retention and post-necrotic cysts. Patients were divided into 3 groups: group 1 included patients with chronic pancreatitis receiving conservative treatment (n=32), group 2 - patients who underwent the Frey pancreatic resection (n=24) and group 3 - Beger pancreatic resection (n=9). The assessment was performed according to the most spread specific questionnaire in gastroenterology GSRS (Gastrointestinal Symptom Rating Scale). Results. The quality of life of patients who underwent both Frey and Beger pancreatic resection was significantly higher than the quality of life of patients in whom chronic pancreatitis was treated conservatively. Quality of life of patients who underwent Frey or Beger pancreatic resection did not differ significantly. Malabsorption syndrome has a significant effect on the quality of life of patients who underwent surgical treatment of chronic pancreatitis. The GSRS questionnaire demonstrated low sensitivity in detecting malabsorption syndrome. Conclusion. Quality of life of patients who underwent surgical treatment of chronic pancreatitis is statistically significantly higher than in patients receiving conservative treatment; correction of existing quality of life questionnaires is required in order to identify malabsorption syndrome.

Background: Various methods of physical therapy are widely used in the rehabilitation of patients with cancer (magnetotherapy, pneumocompression, electrotherapy, laser therapy). The goal of such rehabilitation is to minimize side effects and complications after radical treatment. However, a staged approach to the appointment of physiotherapeutic factors in the early period after surgical treatment of breast cancer has not been developed enough. Aim: To develop a staged approach to the appointment of physiotherapeutic factors at different times after surgical treatment of breast cancer. Design: a randomized-placebo-controlled study. Location: the clinic named after professor Yu. N. Kasatkin of the Russian Medical Academy of Continuing Professional Education of the Ministry of Health of the Russian Federation, Moscow. Population: Examination and treatment of 78 patients aged 30-70 after surgery for breast cancer were performed. The main group of 39 patients underwent a 2-stage course of medical rehabilitation: the first course on days 2-4 after surgery and the second course after 1-1.5 months against the background of adjuvated radiation therapy. The control group of 39 patients underwent physiotherapy placebo procedures. Methods: All the patients underwent medical rehabilitation: individual exercise therapy, balance-therapy, sessions with a medical psychologist, course exposure to an alternating low-frequency electrostatic field and general magnetic therapy. Results: two-stage physical rehabilitation improves the quality of life, decreases swelling, increases the range of motion, decreases pain, decreases the number of postoperative complications, and shortens the duration of lymphorrhea. Conclusions: It is advisable to include general magnetic therapy procedures and exposure to an alternating low-frequency electrostatic field in combination with physiotherapy exercises, balance-therapy and individual lessons from a medical psychologist in rehabilitation courses. Clinical Rehabilitation Impact: Prescribing two courses of medical rehabilitation after breast cancer surgeries in the early stages (on days 2-4) and 1-1.5 months after surgery contributed to a decrease in pain and postoperative edema, and an increase in the range of motion in the shoulder joint, preventing severe lymphostasis and improving the quality of life.

Background: Chronic scrotal content pain (CSCP) is a syndrome that seriously affects the quality of life in men. But in most men, there is no obvious structural abnormality and there may not be any identifiable pain on palpation, even medications and surgeries are not 100% effective and maybe cause other problems. In addition to medications and surgical treatment, any other effective method needs to be a therapeutic option, too. Objective: To discuss a 36-year-old patient with chronic scrotal content pain for 3 years, with a view to introduce a potentially effective treatment for Chronic scrotal content pain. Conclusions: Many pain syndromes can be treated with removal of the original stimulus. For the patients we treat, moxibustion is effective for the treatment of Chronic scrotal content pain caused by the cooling on the portion. Therefore, an effective conservative treatment may be a worthwhile option to try.

ABSTRACT Around 6% of the elderly population over 65 years of age are affected by adult spinal deformity (ASD). The increasing prevalence of ASD with aging has prompted discussion regarding the use of various techniques for its treatment, such as surgery and conservative treatment. The objective of this study is to investigate whether surgical treatment demonstrates significant benefits as compared to conservative treatment. A literature review was conducted, focusing on the most relevant papers on the topic published in the last five years. Surgical treatment, which costs an average of US$ 99,114 per procedure, enables almost instant improvement of the pain and disability of ASD. The rate of perioperative complications in ASD is approximately 7.5%, and the average improvement in back pain is 6.2 times higher in the surgical approach than in conservative treatment. In addition, the use of modern operative techniques, such as minimally invasive surgery (MIS), reduces the complications and greatly improves patients’ quality of life, compared to open surgery. Therefore, the expected benefits of surgical treatment meets the patient’s expectations by eliminating the pain caused by ASD. Although surgical treatment has a higher cost and a greater risk of complications, the use of minimally invasive techniques give the ASD patient a better quality of life. Background: Pain in the lower back is an urgent health issue in industrialized countries. The dominant cause of pain in the lumbosacral spine is the degeneration of the inter vertebral discs (I VD). The total prosthetics I VD is a modern method of surgical treatment of I VD degenerative diseases, an alternative method of rigid stabilization.Aim: To analyze the results of surgical inter vention (I VD prosthesis M6-L implantation) in patients with I VD degeneration of lumbar spine at the 6, 12, 24, and 36 months after the surger y.Materials and methods: The study included 156 patients (92 men, 64 women) aged 23 to 45 years who under went a single-level discectomy with implantation of an artificial IVD prosthesis M6-L  at the bases of three neurosurgical centers. For dynamic assessment, clinical parameters (pain intensity from the visual analogue pain scale (VAS), quality of life according to the Oswestry index, subjective satisfaction with the result of surgical treatment on the Macnab scale) and instrumental data (amplitude of movements in the operated spinal-motor segment, degree of heterotopic ossification according to McAfeeSuchomel classification) were applied at 6, 12, 24, and 36 months after surger y.Results: The mean value of the quality of life for the Oswestr y index before surger y was 40.2±6.9%, after ― 12.3±6.1% (t-test, p0.001). The mean value of the level of pain according to VAS before surger y was 6.9±1.6 cm; after surger y ― 1.3±1.2 cm (t-test, p0.001). The average range of motion in the operated segment at baseline was 36.8±2.6°, within 36 months after the operation increased up to 41.2±2.9°. During the entire period of obser vation the initial (13.4%) or moderate (10.2%) signs of heterotopic ossification were revealed. Conclusions: The use of prosthetic I VD M6-L can significantly reduce the level of pain, improve the life quality, and maintain the physiological range of motion in the operated spinal motion segment with a low level of adverse outcomes.
