Over one million artificial heart valve transplantations are performed each year due to valvular stenosis or regurgitation. Among them, bioprosthetic heart valves (BHVs) are increasingly being used because of the absence of the need for lifelong anticoagulation. Almost all of the commercial BHVs are treated with Glutaraldehyde (GLUT). As GLUT-treated BHVs are prone to calcification and structural degradation, their durability is greatly reduced with a service life of only 12–15 years. The physiological structure and mechanical properties of the porcine aortic valve (PAV) are closer to that of a human heart valve, so in this study, PAV is used as the model to explore the comprehensive properties of the prepared BHVs by radical polymerization crosslinking method. We found that PAV treated by radical polymerization crosslinking method showed similar ECM stability and biaxial mechanical properties with GLUT-treated PAV. However, radical polymerization crosslinked PAV exhibited better cytocompatibility and endothelialization potential in vitro cell experiment as better anticalcification potential and reduced immune response than GLUT-treated PAV through subcutaneous animal experiments in rats. To conclude, a novel crosslinking method of non-glutaraldehyde fixation of xenogeneic tissues for the preparation of BHVs is expected.

Abstract The implantation of bioprosthetic heart valves (BHVs) is increasingly becoming the treatment of choice in patients requiring heart valve replacement surgery. Unlike mechanical heart valves, BHVs are less thrombogenic and exhibit superior hemodynamic properties. However, BHVs are prone to structural valve degeneration (SVD), an unavoidable condition limiting graft durability. Mechanisms underlying SVD are incompletely understood, and early concepts suggesting the purely degenerative nature of this process are now considered oversimplified. Recent studies implicate the host immune response as a major modality of SVD pathogenesis, manifested by a combination of processes phenocopying the long‐term transplant rejection, atherosclerosis, and calcification of native aortic valves. In this review, we summarize and critically analyze relevant studies on (1) SVD triggers and pathogenesis, (2) current approaches to protect BHVs from calcification, (3) obtaining low immunogenic BHV tissue from genetically modified animals, and (4) potential strategies for SVD prevention in the clinical setting.

Изучены образцы створок клапанов сердца, полученные в ходе кардиохирургического вмешательства от 26 пациентов с установленным диагнозом «инфекционный эндокардит» (ИЭ), и от 12 пациентов без данной патологии (контрольная группа). Оценка уровня экспрессии генов врожденного иммунного ответа TLR1, TLR2, TLR4 и TLR6 проводилась методом количественной полимеразной цепной реакции. Установлено, что экспрессия гена TLR2 у пациентов с ИЭ практически не отличалась от контрольной группы, в то время как уровень мРНК генов TLR1, TLR4 и TLR6 в группе пациентов был значительно ниже, чем в контроле. We studied samples of heart valves obtained during cardiac surgery from 26 patients with infective endocarditis (IE) and 12 patients without this pathology (control group). The expression of innate immune response genes (TLR1, TLR2, TLR4 and TLR6) was evaluated by quantitative polymerase chain reaction. It was found that the TLR2 expression in patients with IE was similar to the control group, while the mRNA level of the TLR1, TLR4 and TLR6 genes in the patients was significantly lower compared to the control.
