Toxicity Profile of Chemotherapeutic Agents among Cancer Patients Receiving Care in a Nigerian Tertiary Health Care Facility.

Background: Chemotherapeutic agents are among the mainstay of managing cancer patients. However, they are associated with various degrees of toxicity. Objectives: To evaluate the toxicity profile of chemotherapeutic agents among cancer patients receiving care in Usmanu Danfodiyo University Teaching Hospital, Sokoto, Nigeria. Method: Retrospective cross-sectional design and systematic random sampling were used in selecting the records of patients that met the eligibility criteria for the study. Five-year records (2014–2018) of Full Blood Count (FBC), Serum Electrolyte Urea and Creatinine (SrEUCr) and Liver Function Test (LFT) were evaluated for changes from baseline to the end of chemotherapy. The data were compared with standards and analysed using descriptive, t-test and correlation analyses at p<0.05. Results: The mean age of the 260 patients evaluated was 47.1±16.3 years. T-test analysis showed that the percentage changes in the patients’ parameters under FBC and SrEUCr tests were normal while the ones under LFT were abnormal. Patients on platinum-based combinations especially Cisplatin+Fluorouracil+Paclitaxel (87.5±87.4%) and Carboplatin+Paclitaxel (68.4±114.5%) had the highest percentage increase in their overall LFT results while those on Doxorubicin+Cyclophosphamide+Vincristine (4.8±18.7%) and Doxorubicin+ Cyclophosphamide+ Paclitaxel (12.3±27.9%) had the least. The number of chemotherapy cycles was weakly correlated with Hepatotoxicity (r=0.165, p=0.046). Conclusion: The patients had essentially normal FBC and SrEUCr results, however, LFT was abnormal due to the elevation of liver enzymes. Platinum-based combinations especially Cisplatin + Fluorouracil + Paclitaxel and Carboplatin + Paclitaxel had the highest elevation in liver enzymes while Doxorubicin+Cyclophosphamide+Vincristine and Doxorubicin+Cyclophosphamide+Paclitaxel had the least. These findings should be considered by clinicians in managing cancer patients to minimise their medication-related toxicities.

Кidney injury is a frequent and significant complication of cancer and cancer therapy. The kidneys are susceptible to injury from malignant infiltration, damage by metabolites of malignant cells, glomerular  injury, nephrotoxic drugs including chemotherapeutic agents. Also  bone marrow transplantation complications, infections with immune  suppression (including septicemia), tumor lysis syndrome should be  taken into account. Chemotherapeutic agents are a common cause  of acute kidney injury but can potentially lead to chronic kidney  disease development in cancer patients. This article summarizes risk  factors of acute kidney injury in cancer patients. Risk factors are  divided into two groups. The systemic are decrease of total  circulating blood volume, infiltration of kidney tissue by tumor cells,  dysproteinemia, electrolyte disturbances. The local (renal) risk  factors are microcirculation disturbances, drugs biotransformation  with formation of reactive oxygen intermediates, high concentration of nephrotoxic agents in proximal tubules and its  sensitivity to ischemia. Drug-related risk factors include: drugs  combination with cytotoxic effect high doses long term use necessity, direct cytotoxic effect of not only chemotherapeutic agents but also its metabolites, mean solubility forming intratubular  precipitates. Early diagnosis, timely prevention and treatment of  these complications provide significantly improve nononcologic results of treatment.

Physical fitness is ability to perform the activity or activities without feeling tired excessively. Physical fitness associated with a person`s organs to carry out its tasks properly every day without experiencing significant fatigue. In a child’s mental retardation with a les mobile lifestyle that will affect his physical fitness. The purpose of this study to determine which type of gymnastic exercise better to the physical fitness on the students in Sekolah Dasar Luar Biasa Kuncup Bunga Denpasar.This research used experimental method using Randomized study Pre and Post Test Group Design. The subjects in this study were students in Sekolah Dasar Luar Biasa Kuncup Bunga Denpasar. There were 18 students which is divided into two groups: group-1 is senam penguins and group-2 is senam ria anak Indonesia. Data were obtained after a six-week training analyzed by paired t-test and unpaired t-test.The results of physical fitness test before training with Senam Penguins was 23.39 ± 1.69 and the test results after the training was 27.87 ± 3.28. the physical fitness test in group-2 before training with Senam Ria anak Indonesia was 22.69 ± 1.76 and the results after training was 24.64 ± 2.11. Paired t-test in both group before and after training showed significant differences after training in group-1 and group-2 with a value of p > 0.05 with an increase in physical fitness test results of (19.15) in group-1 and (8.64) in group-2. The percentage increase physical fitness test results of group-1 with senam penguins is higher than those in group-2 with senam ria anak Indonesia.So we can conclude that the training of senam penguin in Sekolah Dasar Luar Biasa Kuncup Bunga Denpasar improved the physical fitness test results higher than senam ria anak Indonesia.

A new paradigm in cancer chemotherapy derives from the interaction between chemotherapeutics, including irinotecan and 5-fluorouracil (5-FU), and the immune system. The patient’s immune response can modulate chemotherapy effectiveness, and, on the other hand, chemotherapeutic agents can foster tumor cell immunogenicity. On these grounds, the analysis of the cancer patients’ immunogenetic characteristics and their effect on survival after chemotherapy represent a new frontier. This study aims to identify genetic determinants in the immuno-related pathways predictive of overall survival (OS) after FOLFIRI (irinotecan, 5-FU, leucovorin) therapy. Two independent cohorts comprising a total of 335 patients with metastatic colorectal cancer (mCRC) homogeneously treated with first-line FOLFIRI were included in the study. The prognostic effect of 192 tagging genetic polymorphisms in 34 immune-related genes was evaluated using the bead array technology. The IL15RA rs7910212-C allele was associated with worse OS in both discovery (HR: 1.57, p = 0.0327, Bootstrap p-value = 0.0280) and replication (HR:1.71, p = 0.0411) cohorts. Conversely, SMAD3 rs7179840-C allele was associated with better OS in both discovery (HR:0.65, p = 0.0202, Bootstrap p-value = 0.0203) and replication (HR:0.61, p = 0.0216) cohorts. A genetic prognostic score was generated integrating IL15RA-rs7910212 and SMAD3-rs7179840 markers with inflammation-related prognostic polymorphisms we previously identified in the same study population (i.e., PXR [NR1I2]-rs1054190, VDR-rs7299460). The calculated genetic score successfully discriminated patients with different survival probabilities (p < 0.0001 log-rank test). These findings provide new insight on the prognostic value of genetic determinants, such as IL15RA and SMAD3 markers, and could offer a new decision tool to improve the clinical management of patients with mCRC receiving FOLFIRI.

The introduction of cisplatin in cancer treatment represents an important achievement in the oncologic field. Many types of cancers are now treated with this drug, and in testicular cancer patients major results are reached. Since 1965, other compounds were disovered and among them carboplatin and oxaliplatin are the main Cisplatin analogues showing similar clinical efficacy with a safer toxicity profile. Lipoplatin is a new liposomal cisplatin formulation which seems to have these characteristics. Lipoplatin was shown to be effective in NSCLC both in phase 2 and phase 3 trials, with the same response rate of Cisplatin, a comparable overall survival but less toxicity. A new protocol aiming to elucidate the double capacity of Lipoplatin to act as a chemotherapeutic and angiogenetic agent in triple-negative breast cancer patients is upcoming.

Research in the field of psycho-oncology in South Africa is increasing, and there is a need for validated measures that assess factors associated with cancer, such as social support. The Berlin Social Support Scales are a battery of instruments that measure various types and functions of social support. The measure was originally developed for use among adult cancer patients, and their partners but has also been used among other clinical populations and healthy adults. We investigated the psychometric properties of the English version of the perceived and received sub-scales, Berlin Social Support Scales. Our sample included South African women ( N = 201) who were diagnosed with breast cancer and receiving treatment at a public health care facility. We administered several measures, including a demographic questionnaire, the Berlin Social Support subscales, the Duke-UNC Functional Social Support Questionnaire, and The Functional Assessment of Cancer Therapy to participants. Validity and reliability analyses were conducted. Factor analysis resulted in the retention of 17 items that clustered on two factors, namely received support and perceived support. The 17-item version of the Berlin Social Support Scale demonstrated strong reliability and validity in the sample. The two subscales are quick to administer, easy to interpret, and are a reliable measure of social support among breast cancer patients in South Africa.

This study aims to determine the effect of meditation practice and internal imagery on the concentration and shooting accuracy of the 10-meter category in the Perbakin Bantul branch. The research method was experimental with a two-group pretest-posttests design. The population investigated 16 athletes using the purposive sampling technique so that the sample became 12 athletes. The research instrument used the Grid Concentration Exercise to measure concentration and measurement of firing accuracy using ISSF standard target targets. The data analysis technique used was paired t-test and independent t-test. The results showed an effect of training between autogenic meditation and internal images on concentration and shooting accuracy. When viewed from the percentage increase, the meditation practice method showed better results than internal imagery, while internal imagery training showed better results in increasing concentration.

Gastrointestinal cancers (GI) are one of the most abundant types of cancers among the world population, though statistical data indicate that in eastern Asia these types of cancer occur 4 times more often than in Western Europe. Absence of treatment of bacterial infections, obesity, and lack of vegetable food in a diet can be the case of GI cancer. All pathologies are inevitably connected to the changes in cell cycle, abnormal protein amount and their dysfunction. Serum proteins are widely used as an additional source of information about body condition, also changes in protein composition can point out the mechanism of disease development and effectiveness of treatment. In the presented work we studied protein composition of GI cancer patients in different stages of cancer development, after and before chemotherapy and compared these data to protein composition of healthy control group of voluntaries. Treatment of patients was performed according the guidelines appropriate for the GI cancer. Association of the effectiveness of treatment at the different stages of chemotherapeutic courses and changes of protein composition of blood serum has been assessed. Proteins composition was studies by SDS-PAGE electrophoresis and densitometry analysis. Experimentally gained molecular and statistical information exposed the most vulnerable groups of proteins affected by chemotherapeutic agents indicating targets for searching new biomarkers for treatment effectiveness. Research involving human patients performed in accordance with the requirements of the Council of Europe Convention on Human Rights and Biomedicine, Biomedical Research, as well as the UNESCO Declaration of Bioethics and Human Rights.

Background: A peripheral intravenous catheterization is performed for injecting therapeutic agents into the blood stream. However, it is not easily done in most cancer patients due to the abnormalities of blood vessels because of the repetition of intravenous insertion as well as toxicity of the chemotherapeutic agents. Objective: To study the proper venipuncture sites for chemotherapy in cancer patients. Materials and Methods: Using the Delphi technique, nine of the ten experts, with more than seven years of experience, volunteered to respond to the Likert rating questionnaires. Results: All participants accomplished the study without procedure-related problems. Items concerning peripheral intravenous assessment on the dorsum of hand, forearm, antecubital fossa, and upper arm on the first, second, and third round were 58.3%, 58.3%, 58.3% and 48.3%; 71.6%, 71.6%, 73.3%, and 60.0%; 78.3%, 75.0%, 76.6%, and 65.0%, respectively. Discussion: Veins located on the dorsum of hand were preferred in the first rank due to the facility to identify as well as to care, clean and control infection. This was followed by antecubital veins for its larger size, small risks of thrombophlebitis, and less irritation during chemotherapy, and veins at the forearm for its facilitating self-care management, as well as preventing dislodgement and occlusion. Conclusion: The dorsum of hand followed by the antecubital fossa, forearm and upper arm of the non-dominant hand were the favorable sites of venipuncture for all cancer patients.
