One of the major causes of root canal treatment failure is inability to find and manage an extra canal (s). For achieving successful endodontic therapy, knowledge of root canal morphology and variations in the root is important. The purpose of this case report is to report a rare case of maxillary canine with two root canals. It describes the treatment of a permanent maxillary left canine having an additional canal (Vertucci’s Type II canal configuration) present in a 48 year old female patient. Clinical examination revealed a maxillary canine with a previously initiated access. Radiographic examination revealed a previously initiated access in left maxillary canine, having an additional canal. Maxillary first premolar is a bi-rooted tooth with several variations in the root canal morphology. Usually it has 2 canals, however at times there could be an additional third canal present which could easily be missed by the clinician owing to lack of awareness and knowledge about the canal variations. Awareness of internal and external anatomy of tooth and also about its variations are essential for successful endodontic therapy. Prevalence of three canals in maxillary premolar is very low. This case report describes the endodontic management of permanent maxillary premolars with three root canals

Human mandibular premolars gained popularity as they have complex root and root canal morphology. Multiple roots and canals are more common in mandibular first premolars when compared to mandibular second premolars. The occurrence of three root canals and separate foramina in mandibular second premolars is not common. In this case report, endodontic management of a three-rooted mandibular second premolar is discussed.

A thorough knowledge and understanding of the root canal morphology including the variations is important for the successful outcome of endodontic treatment. The success of an endodontic treatment depends on the eradication of microbes from the root-canal system and prevention of re-infection [1] . The variations in mandibular first molar involves the number of roots, the number of root canals, and morphology. Radix entomolaris and the radix paramolaris are the additional root located lingually and buccally respectively [2] . This case report discusses endodontic treatment of a mandibular first molar with a radix entomolaris and pulp stone.

Thorough knowledge of root canal morphology is essential for the endodontic therapy. There are rare variations in canal number and configuration in maxillary molars, which could affect treatment outcome. This paper presents the endodontic management of a maxillary first molar with two palatal canals in one root (Vertucci type IV) and a single buccal canal. In this paper cone-beam computed tomography was made to asses this morphology. This paper is intended to reinforce clinician’s awareness of the rare morphology of root canals.

It is of interest to compile available information on the root canal morphology of primary maxillary molars from known literature. The literature resources used to collect data include Medline/PubMed, The Cochrane Central Register of Clinical Trials, SIGLE and Science Direct. Data consists of type of population, number of teeth per study, number of root canals, canal length and type of root canal configuration. We used data from a total of 13 studies (951 primary maxillary molars). Maxillary molars (1st and 2nd) are dominant for two roots variant. The first molar the mean root length ranges from 7.9mm – 8.1mm. The second molar ranges from 7.2mm-8.5mm. Type I (explain in a phrase) canal morphology is the common variant in both the molars. Data shows that Root Canal morphology shows variations with the diagnostic aid (example micro CT) used and in different ethnic populations.

The proper knowledge of the root canal system anatomy is crucial to perform successful endodontic treatments. Maxillary lateral incisors usually have a single root. However, aberrant canal configurations of this tooth have been reported in the literature. The clinician should be aware of the difficulties arising from this more complex root canal anatomy and should be able to manage them. A rare case of a maxillary lateral incisor with three root canals is presented in this article; the challenge of diagnosis and treatment in similar situations is also discussed.
