This single case study of a bilingual child with a severe speech sound disorder examined whether language transference occurs when treatment is provided in L1 or L2 and whether monolingual or bilingual intervention is most effective. A 6-year 5-month old sequential Portuguese/English bilingual female was seen for 3 separate intervention phases lasting about 2 months each. Four phonological processes common to both languages were targeted in either monolingual or bilingual conditions in each phase. The child’s speech was examined with formal tests and analyses of spontaneous speech after each phase. Results indicated that transference occurred in both directions for some processes (L1 to L2 and L2 to L1), but in general L1 had to be specifically targeted for significant improvement to occur. For both languages, the bilingual condition was most effective. A follow-up assessment one year after Portuguese treatment was interrupted revealed that the child continued to make progress in English only (the only language treated for the entire year) and her Portuguese phonology still showed the same errors as in the previous year. This case study provides further evidence that bilingual intervention is the preferred choice for bilingual children. Implications for language choice and future studies are discussed.

Behaviour change is often a desired outcome for carers and professionals who work with people with learning disabilities and challenging behaviours. Managing these behaviours in the short term is an important step towards this goal. This single case study presents the use of a low arousal approach in managing challenging behaviours in a young man labelled with severe challenging behaviour. This strategy focuses on the interaction of the carers with the client and how their approach has an important impact on the behaviour of the client. The study documents the incidents of challenging behaviour and shows a decline in their frequency from baseline over a one-year period. These gains were maintained at five-month follow-up. The implications for services of these findings are discussed.

Objective: The aim of the study was to evaluate the effectiveness of minimal pair intervention in a school-aged child with consistent phonological disorder, using a multiple baseline single case study design. Method: The participant was a boy aged 8;11 who presented with a consistent phonological disorder. A multiple baseline single case study design was used to measure the effect of six sessions of minimal pair intervention for treating consistent phonological disorder. There was a two-week baseline period prior to the intervention phase. The outcome measures included measures of the severity of the speech sound disorder and speech intelligibility, which were taken pre-, immediately postintervention, and five months post-intervention. Other measures included the monitoring of the accuracy of production of the treated targets /ʃ/ and un-treated targets /ʤ/, /ʧ/, and /ɹ/ during intervention. Main results: There was an increase in accuracy in the production of treated and untreated target sounds post-intervention. There was a reduction in the severity of the speech sound disorder and an improvement in speech intelligibility post-intervention. These changes were maintained five months post-intervention. Conclusion: Minimal pair intervention, when provided over six sessions, can result in positive changes in the severity of speech sound disorder, accuracy of the target phonemes and speech intelligibility in a schoolaged child with consistent phonological disorder.

Purpose This study investigated the extent to which speech-language pathologists (SLPs) facilitate parents' completion of homework activities for children with speech sound disorder (SSD). In addition, this study explored factors related to more consistent communication about homework completion and strategies considered particularly effective for supporting this element of parental involvement. Method Licensed SLPs serving at least one child with SSD were invited to participate in an online survey. Questions relevant to this study gathered information regarding (a) frequency of communication about homework distribution and follow-up, (b) demographic and workplace characteristics, and (c) an open-ended question about the specific strategies used to support parental involvement and completion of homework activities. Results Descriptive results indicated considerable variability with respect to how frequently SLPs engaged in communication about homework completion, but that school-based SLPs were significantly less likely to engage in this type of follow-up. Strategies considered effective, however, were similar across therapy contexts. Conclusion These results suggest potentially important differences between school-based services and therapy in other contexts with respect to this particular aspect of service provision for children with SSD.

Polydipsia is a disorder that has received little attention in the research literature. Treatment has been mainly confined to medical or pharmacological intervention. Few studies have reported the use of contingency management techniques and none have sought to encourage self-management. This study shows how such a procedure brought about a significant change in rates of water drinking in a thirty-one year old man with a mild learning disability.

This case study illustrates the reduction of severe stuttering by an individually tailored treatment programme. Interventions are derived from a tripartite analysis (Lang, 1971) and include EMG biofeedback, regulated breathing, exposure in vivo to stressful communication situations and cognitive techniques to reduce relapse risk. The role of dysfunctional response system interactions in stuttering is emphasized. Treatment resulted in a marked reduction of stuttering and associated facial contortions during videotaped conversations with strangers and oral reading. Improvement was maintained at one-year follow-up.

A case study is presented of a young woman with an unusual phobia, a fear of babies. Barabasz's (1977) technique of systematic desensitization using psycho-physiological measures was chosen as the main treatment strategy. Difficulties arose as the client was unable to visualise scenes involving babies. Nor could she look at photographs of babies long enough for the hierarchy to be ordered using a psycho-physiological measure (skin conductance). A set of photographs was eventually used for the hierarchy, but it was ordered in terms of the length of time the client could look at the various photographs. Systematic desensitization was carried out using the set of photographs instead of imaginary scenes, together with some in vivo exposure in the latter stages of treatment. At termination the client could approach babies without discomfort. Improvement was maintained at one year follow-up.

Amavata is a chronic, progressive and crippling disorder caused due to generation of ama and its association with vitiated vata dosha and deposition in shleshma sthana (joints). Clinically resembling with Rheumatoid Arthirtis, it poses a challenge for the physician owing to its chronicity, morbidity and complications. The treasure of Ayurveda therapeutics has laid out detailed treatment line for amavata. A 13years old male patient reported to this hospital with pain and stiffness of metacarpophalangeal joints of right hand followed by pain in corresponding joints of other hand 1 year back. This was succeeded by pain and mild swelling on bilateral wrist, ankle and elbow joints. Based on clinical examination and blood investigations, diagnosis of amavata was made and Ayurvedic treatment protocol was advised with baluka sweda (sudation) as external application, rasnasaptak kashayam and dashmoolharitaki avaleha for oral intake for 30 days. The patient was asked for follow up every 15 days up to total of 45 days. Assessment was done subjectively based on clinical symptoms and blood investigations as objective parameters. There was substantially significant improvement and the patient felt relieved of the pain and inflammation of the joints after the treatment. This case study reveals the potential of Ayurvedic treatment protocol in management of amavata and may form a basis for further detailed study of the subject.

Abstract Background Speech sound disorder is a communication disorder in which children have persistent difficulty saying words or sounds correctly. It refers to any difficulty or combination of difficulties with perception, motor production, or phonological representation of speech sounds and speech segments. Aim of the Work to construct an Arabic auditory bombardment therapy program and measure its effectiveness in treatment of functional speech sound disorder. Subjects and Methods This study was applied on 60 participants divided into 2 groups (30 for each group) with age ranging from 3-5 years diagnosed with functional speech sound disorder with or without language disorders, attending at the Phoniatrics outpatient clinic in Ain Shams University Hospitals. The test for identification of phonological processes was applied on 60 patients with speech sound disorder selected to participate in this study. These were divided in to 2 groups (Group (1) received only the conventional therapy while group (2) received auditory bombardment in addition to the conventional therapy for 3 months) and the test was repeated again after therapy. Results Group (2) showed high significant difference (improvement) in consonant assimilation, voicing change, final consonant deletion, palatal fronting, gliding, lateralization and glottal replacement while group (1) showed high significant difference (improvement) in syllable deletion and partial cluster reduction. Conclusion The present study showed that application of auditory bombardment therapy program in addition to conventional therapy has high significant improvement than conventional therapy alone.

‘Skipped generation’ families, consisting of grandparents and grandchildren with parents absent from the home, are frequently served in social work agencies. These families have unique multigenerational patterns and family structures that are important for service providers to recognize. This paper uses a multigenerational systems perspective to highlight the diversity among grandparent-headed households. Twenty families who were previously part of a larger study of stress, well-being, and life satisfaction among caregiving grandparents constituted a follow-up case study involving videotaped family interviews one year after the first study. Three families representing the range of diversity among the twenty are described with accompanying genograms. Differences in structure, interactional processes, and links with prior generations are identified in each case. These examples reveal the strengths and vulnerabilities, as well as the diversity, of grandparent-headed families.

PURPOSE: To describe the tongue shape for /s/ and /∫/ sounds in three different groups of children with and without speech sound disorder. METHODS: The six participants were divided into three groups: Group 1 - two typically developing children, Group 2 - two children with speech sound disorder presenting any other phonological processes but not the ones involving the production of the /∫/ and Group 3 - two children with speech sound disorder presenting any phonological processes associated to the presence of the phonological process of palatal fronting (these two children produced /∫/ as /s/) aged between 5 and 8 years old, all speakers of Brazilian Portuguese. The data were the words /'∫avi/ (key) and /'sapu/ (frog). Tongue contour was individually traced for the five productions of each target word. RESULTS: The analysis of the tongue contour pointed to evidences that both /s/ and /∫/ were produced using distinct tongue contours for G1 and G2. The production of these two groups was more stable than G3. The tongue contour for /s/ and /∫/ from the children in G3 was similar, indicating that their production was undifferentiated. CONCLUSION: The use of the ultrasound applied to the speech analysis was effective to confirm the perceptual analysis of the sound made by the speech-language pathologist.
