Osteoarthritis of the knee is a common clinical degenerative disease, which occurs in middle-aged and elderly people. Acupuncture therapy has a significant effect in the treatment of knee osteoarthritis, and is gradually accepted by more and more patients. Observing the treatment of knee osteoarthritis by different acupuncture and moxibustion therapies, we can find that acupuncture treatment of knee osteoarthritis has the advantages of relieving knee joint pain, improving knee joint function, absorbing knee joint inflammation, and promoting the recovery of damaged soft tissue. The treatment of osteoarthritis provides a more effective treatment plan.

Objective: To explore the effects of sodium hyaluronate combined with Jintiange on knee joint function after arthroscopic debridement. Methods: Totally 122 knee osteoarthritis patients underwent arthroscopic debridement were randomized into control and experimental groups. The interleukin-1β, interleukin-6 and tumor necrosis factor-α serum levels were tested via Elisa before and after treatment. The Lysholm knee joint function score, effective rate of treatment, interleukin-1β, interleukin-6 and tumor necrosis factor-α expression, visual analogue scale of pain, incidence of adverse reactions and patient satisfaction were compared. Results: Compared with the control group, the effective rate of treatment and the score of Lysholm knee joint function in the other were higher (both P <0.001), and the serum interleukin-1β, interleukin-6, tumor necrosis factor-α expression and visual analogue scale score were lower (all P <0.001). The incidence of adverse reactions revealed no marked difference (P >0.05), but the satisfaction scores of the experimental group were higher (P <0.01). Conclusion: sodium hyaluronate combined with Jintiange is helpful for knee joint function recovery after arthroscopic knee osteoarthritis, which effectively decreases the level of inflammatory factors and increase patients’ satisfaction.

The aim of this study was to assess a novel approach to treating severe knee osteoarthritis by targeting synovial membrane, superficial articular cartilage, synovial fluid, and subchondral bone by combining intra-articular injections and intraosseous infiltrations of platelet rich plasma. We explored a new strategy consisting of intraosseous infiltrations of platelet rich plasma into the subchondral bone in combination with the conventional intra-articular injection in order to tackle several knee joint tissues simultaneously. We assessed the clinical outcomes through osteoarthritis outcome score (KOOS) and the inflammatory response by quantifying mesenchymal stem cells in synovial fluid. There was a significant pain reduction in the KOOS from baseline(61.55±14.11)to week 24(74.60±19.19), after treatment(p=0.008), in the secondary outcomes (symptoms,p=0.004; ADL,p=0.022; sport/rec.,p=0.017; QOL,p=0.012), as well as VAS score (p<0.001) and Lequesne Index(p=0.008). The presence of mesenchymal stem cells in synovial fluid and colony-forming cells one week after treatment decreased substantially from7.98±8.21 MSC/μL to4.04±5.36 MSC/μL(p=0.019)and from601.75±312.30to139.19±123.61  (p=0.012), respectively. Intra-articular injections combined with intraosseous infiltrations of platelet rich plasma reduce pain and mesenchymal stem cells in synovial fluid, besides significantly improving knee joint function in patients with severe knee osteoarthritis. This trial is registered on EudraCT with the number2013-003982-32.

Objective: To observe therapeutic effect of sodium aescinate Tablets on knee osteoarthritis complicated with synovitis. Methods: 86 patients with knee osteoarthritis combined with synovitis treated in the Honghui Hospital Affiliated to Xi’an Jiaotong University from January 2018 to December 2019 were randomly divided into control group and intervention group, with 43 cases in each group. The control group was given routine treatment, and the intervention group was given sodium aescinate Tablets orally on the basis of routine treatment, twice a day, 2 tablets each time. The curative effect was evaluated after 2 weeks of treatment. The knee pain and joint function of the two groups were evaluated by Visual Analogue Scale (VAS) and Western Ontario and McMaster Universities Arthritis Index (WOMAC), and the clinical effects of the two groups were compared. Results: Before treatment, there was no significant difference in general information, VAS and WOMAC scores between the two groups (P > 0.05). After treatment, the VAS and WOMAC scores of the two groups were both lower than those before treatment, and the difference was statistically significant (P < 0.05). The vas and WOMAC scores of the intervention group were lower than those of the control group (VAS, 1.87 ± 0.79 vs 3.38 ± 0.81. In this study, sodium aescinate tablets were applied to the treatment of KOA combined with synovitis. The results showed that the intervention group was better than the control group in alleviating knee pain, improving joint function and improving clinical efficacy, and the difference was statistically significant. The total effective rate of the intervention group was higher than that of the control group (100% vs 86.05%), and the difference was statistically significant (P < 0.05). Conclusion: Sodium aescinate tablet in the treatment of knee osteoarthritis combined with synovitis can effectively alleviate knee pain, improve joint function and improve clinical efficacy.

In clinical practice viscosupplementation with hyaluronic acid (HA) is common for the treatment of degenerative osteoarthritis (OA). Both molecular weight and concentration of HA have significant impact on its rheological properties, which in turn affects its therapeutic effects. The objective of this study is to evaluate the effectiveness of a double HA preparation for the treatment of knee osteoarthritis with respect to pain reduction, joint function improvement and concomitant medication consumption reduction. One thousand and fourteen patients (521 males and 693 females) with a mean age of 62.4 years old, suffering from OA of the knee, were enrolled into this study. All patients received two intra-articular injections one week apart and a third injection one month after the second one. Concomitant medication was recorded and evaluated at follow up visits. Evaluation was performed at baseline, day 30 and day 180, on several parameters: knee pain by visual analog scale (VAS) 0-10 cm, Lequesne Index, and consumption of concomitant medications including non-steroidal anti-inflammatory drugs, analgesics and chondoprotective supplementations. A statistically significant reduction in pain VAS score was recorded at D30 (38.01±17.68; P<0.01) before the third injection, and D180 (25.91±15.33; P<0.01) check-points comparing to baseline (67.12±15.99). Similarly, remarkable reduction in Lequesne Index was shown at D30 (5.91±4.01; P<0.01) in 1214 patients before the third injection, and D180 (3.59±3.45; P<0.01) (with 938 patients) when compared to the baseline (11.60±5.13). Patients also consumed less concomitant medications after the treatment course. The beneficial effects were maintained for up to six months. Intra-articular injection of a double HA preparation of low molecular weight and high molecular weight of different concentrations was well tolerated, and generated satisfactory results in terms of pain control, joint function improvement and concomitant medication reduction for the management of knee OA.
