We present a 38 year old male with a rare case of plasma cell granuloma arising from facial nerve which posed a diagnostic and therapeutic challenge. He has no other comorbidities except for a history of left ear modified radical mastoidectomy performed in 2002 for cholesteatoma after which he recovered uneventfully. This time he presented with mass in the ear canal with ear block and otorrhea for 1 month duration. Initial biopsy revealed granulation tissue. CT scan revealed bony destruction with soft tissue mass. Diagnosis of recurrence of cholesteatoma was made for which he underwent MRM under general anesthesia. Intraoperatively, there was soft tissue mass filling the mastoid and the middle ear and destruction of all ossicles. The mass was arising from facial nerve and there was bony dehiscence of facial nerve. The mass sent for histopathology revealed plasma cell granuloma. He recovered uneventfully with intact facial nerve and hearing level of 40 dB .Ear canal polyp should not be avulsed as it may arise from facial nerve and any mass should be subjected to histopathological examination and should not be left as granulation tissue. This case posed both diagnostic and therapeutic challenge and supported the hypothesis of that plasma cell granuloma are psudotumour and with proper excision and post-operative steroids, there will be complete remission.

ABSTRACT Choanal polyp is the term used for benign, solitary soft tissue mass which extend toward the junction of nasal cavity and nasopharynx, i.e. choana. We report a rare case of choanal polyp arising from the middle turbinate and its successful treatment by endoscopic surgery. The complete resection of choanal polyp at the site of origin using an endoscope is usually sufficient and carries good prognosis without recurrence. How to cite this article Gupta M, Singh S. A rare case of an epitheloid haemangioendothelioma presenting as an aggressive soft tissue mass in the wrist of an infant is presented. The management of the lesion presents particular difficulties because of its diffuse involvement of surrounding muscles, tendons and neurovascular bundle and the importance of preserving hand function. The epitheloid haemangioendothelioma rarely occurs extravascularly with less than 15 such cases reported in the literature.

Measurements of crown-heel length, head circumference, and birth weight were made on a large number of newborn infants. Birth weight by itself was frequently not a valid measure of fetal growth impairment. By including measurements of body length and head size along with birth weight, four distinct patterns of fetal growth impairment were identified. The four patterns included infants who had abnormally short body lengths for dates, infants who had evidence of disproportionate growth between body length and head circumference, infants who accumulated excessive amounts of soft-tissue mass, and infants who accumulated too little soft-tissue mass. Criteria for diagnosing each pattern were obtained prospectively and have been presented with allowances made for the effects of race, sex, fetal age, and parity on each pattern. The separate identification of the four patterns provided a more precise description of fetal growth impairment than could be obtained from birth weight and calculated gestational age.
