Background: Orthostatic hypotension is a sustained reduction of systolic blood pressure of at least 20mmHg or diastolic blood pressure of at least 10mmHg within three minutes of standing from supine position. The burden of orthostatic hypotension on public health is substantial, with a prevalence of 7% to 55% in the elderly and is higher in those with risk factors especially hypertension, use of antihypertensive drugs and diabetes mellitus. The objective of this study was to determine the prevalence of orthostatic hypertension among the elderly hypertensives in Anambra State.Methods: This was a community based cross-sectional study among 400 elderly people aged 60 years and above who were recruited using a multistage sampling method. Data was collected using interviewer administered questionnaire. Mercury sphygmomanometer, weighing scale, stadiometer and glucometer were used to record blood pressure, body weight, height and random blood sugar respectively. Statistical analysis was done using Statistical Package for the Social Sciences version 21 and p value ≤ 0.05 was considered statistically significant.Results: Most of the participants had either systolic or diastolic hypertension (85.8%). The prevalence of orthostatic hypotension was 14.8% in the study population and 16.3% among those who were hypertensive.  Those with supine diastolic hypertension [OR: 1.699 (95% CI: 0.401-7.209)] were more likely than those without supine diastolic hypertension to have orthostatic hypotension. Conclusion: The prevalence of orthostatic hypotension among elderly hypertensives aged 60 years and above in Anambra State was found to be relatively high, implying a higher risk of orthostatic hypotension in elderly hypertensives than in normotensives. This finding will aid clinicians in better management of elderly patients which will lead to prevention of the various complications due orthostatic hypotension.

ABSTRACTBackground: Physical activity in the elderly is very influential in blood pressure, the greater the physical activity carried out in the blood pressure of the elderly also increases. The purpose of the study is to investigate the effect of physical activity on blood pressure in the elderly 60-74 years. The research method used in this study is a cross sectional study. The sample in this study was 32 elderly people in Taloarane Village. Data colecton method in this study used physical activity observation sheet. Data analysis used statistical analysis with wilcoxon test. The results of this study revealed p- value = 0,000 which is less than significant value 0,005. Obtained are blood pressure before physical activity has a smaller value than blood pressure after physical activity where the avarage blood pressure is increased from before doing activities that is 142,25 to 159,81. Conclusion there was an influence of physical activity on blood pressure in the elderly in the village of Taloarane. Tujuan penelitian diketahui pengaruh aktivitas fisik terhadap tekanan darah pada usia lanjut 60-74 tahun. Metode penelitian yang digunakan dalam penelitian ini adalah survey cross sectional. Sampel dalam penelitian ini adalah 32 orang lansia di Desa Taloarane. Penelitian menggunakan alat pengumpulan data berupa lembar observasi aktivitas fisik. Analisis data menggunakan uji wilcoxon. Hasil penelitian ini mengungkapkan p-value = 0,000 yang kurang dari nilai signifikan 0,005.  uji diperoleh yaitu tekanan darah sebelum aktivitas fisik memiliki nilai lebih kecil dari tekanan darah sesudah aktivitas  dimana didapatkan tekanan darah rata-rata meningkat dari sebelum melakukan aktivitas yaitu 142,25 menjadi 159,81.Kesimpulan  dari penelitian ini ada pengaruh aktivitas fisik dengan tekanan darah pada lansia didesa Taloarane. Hasil penelitian ini merekomendasikan untuk penelitian selanjutnya agar menambahkan jenis aktivitas fisik.Kata kunci : Aktivitas Fisik, Tekanan Darah, Lansia

Abstract OBJECTIVE Atrial fibrillation (AF) and hypertension are prevalent chronic disease conditions in the elderly population. In the present cross-sectional study, we investigated the association between blood pressure (BP) and AF in an elderly Chinese population. METHOD Our elderly (≥65 years) subjects were residents recruited from 6 communities in Shanghai from 2006 to 2017. Atrial fibrillation was systematically screened by rest 12-lead electrocardiogram (ECG) or by a handheld single-lead ECG. BP status was defined according to the European hypertension guidelines as optimal, normal, or high-normal BP, and stage 1, 2, or 3 hypertension. RESULT In the 6,966 participants (women 56.0%, mean age: 72.3 years), the prevalence of AF was 3.3%, and the prevalence of hypertension was 58.7% (83.7% treated). In all participants, the association with prevalent AF was negative for systolic BP (odds ratio [OR] per 10-mm Hg increase 0.79, 95% confidence interval [CI]: 0.71–0.88, P < 0.0001) but positive for diastolic BP (OR per 5-mm Hg increase 1.11, 95% CI: 1.02–1.22, P = 0.02). In untreated participants (n = 3,544), the association with prevalent AF was U-shaped for both systolic and diastolic BP, with the nadir at high-normal BP and a significantly higher risk of prevalent AF in optimal systolic BP (OR: 3.11, 95% CI: 1.65–5.85, P = 0.004) and stage 2 or 3 diastolic hypertension relative to the nadir (OR: 8.04, 95% CI: 2.28–28.3, P = 0.001). CONCLUSION In the elderly population, BP shows a complicated relationship with prevalent AF, with high-normal BP at the lowest risk and optimal systolic BP and stage 2 or 3 diastolic hypertension at increased risks.

Hypertension is among the most common non-communicable and lifestyle disease in our country that affects adult population of both the genders from all socio-economic backgrounds and urban and rural population. In-spite of this, early diagnosis and appropriate treatment are suboptimal. Adherence of prescribed treatment has been studied in patients of hypertension in this study. : An observational and cross-sectional study was conducted in the Department of Pharmacology, Jawahar Lal Nehru Medical College and Hospital, Bhagalpur, Bihar. Prior to the initiation of the study, clearance was obtained from the Institutional Ethics Committee. Study period was between January 2021 and June 2021.A predesigned pretested interview schedule was used to collect the data from the 247 study participants. This schedule contained information related to socio-demographic variables, comorbidity, a format to assess the compliance to antihypertensive drugs prescribed and any adverse event. : A significant improvement in maintain optimal in blood pressure was observed in patients treated with one pill per day as compared to patients prescribed with two and three pills per day. Compliance was significantly better in patients in combination therapy as compared to monotherapy. : Low dose combination therapy has been stated to be more effective than high dose monotherapy in controlling blood pressure. It shows better compliance and lesser incidence of side-effects.

Aims: To measure the waist circumference of Sudanese adults in Khartoum Locality and its relationship to blood pressure and lifestyle  during celebration of international day of hypertension in May 2016 . Study Design: It was a descriptive cross-sectional study. Place of the Celebration: Khartoum Locality at Alsahaa Alkhadraa (The Green Park). Methodology: A total of 364 adult participants, 196 men and 168 women were interviewed using structured questionnaire. Blood pressure (BP) was measured considering hypertension as ≥ 140 mmHg and ≥ 90 mmHg for systole and diastole BP respectively. Waist circumference was measured using an anthropometric measuring tape at cut-off point of 94 cm and 80 cm for men and women respectively. Data was managed by SPSS version 20 and Chi-square test at 95% CL was used to test the association between waist circumference, blood pressure and life style characteristics. Results: Age distribution of the study population showed 48.2% females and 45.4% males in the middle age group (38-57 years). Two thirds of the study population were hypertensive, 62.8% of males and 64.3% of females. The mean waist circumference of men was 97.82 cm + 16.7, mean Systolic BP was 127 + 22 and mean Diastolic BP was 85 + 15. The mean waist circumference of women was 99.31 + 16.2, mean Systolic was 128 + 24 and mean Diastolic BP was 84 +17. Abnormal waist circumference was found in 61.2% of males and 86.9% of females. Fifty nine (30.1%) of the males and 86 (51.2%) of the females with abnormal waist circumference were hypertensive. The association between abnormal waist circumference and high blood pressure was significant among both sexes, P value = 0.001. Physical exercise and fat and salt foods were not significantly associated waist circumference in both men and women. Conclusion: Two thirds of women and men in the celebrating areas were hypertensive.  Half of women and one third of men were significantly hypertensive and having abnormal waist circumference. Doing physical exercise, avoiding fat and salt foods was insignificantly associated with normal waist circumference. Large survey with representative sample is needed to estimate the real Sudanese waist circumference.

Abstract Objective: to analyze blood pressure control in elderly hypertensives accompanied by a Basic Health Unit of the Federal District, determining the sociodemographic profile and the associated risk factors. Method: this is a cross-sectional study with 133 elderly hypertensive patients. Variables related to sociodemographic factors, life habits, clinical factors and adherence to drug therapy were evaluated through the application of semistructured instruments and validated scales. Results with p <0.05 were considered significant. Results: the elderly presented controlled blood pressure (56.4%) predominantly. The majority of the participants were female and women also had a higher rate of uncontrolled blood pressure (86.2%). Older age was associated with higher blood pressure values (p = 0.031). Alcoholism (p = 0.020) and mean body mass index of 33.0 (p <0.000) were factors associated with hypertension adherence to therapy had more controlled values of systolic and diastolic blood pressure. Conclusion and implications for the practice: it was verified that there is a strong association between the risk factors discussed and the lack of control of the blood pressure of elderly hypertensive, especially with regard to advanced age, alcoholism, obesity and overweight, and it is necessary to reorient the planning and strategies of promotion of health and prevention of diseases directed at the elderly hypertensive in the scope of primary health care.

Peripheral arterial disease (PAD) increases with age and ankle-brachial index (ABI) ≤ 0.9 is a noninvasive marker of PAD. The purpose of this study was to identify risk factors related to a low ABI in the elderly using two different methods of ABI calculation (traditional and modified definition using lower instead of higher ankle pressure). A cross-sectional study was carried out with 65 hypertensive patients aged 65 years or older. PAD was present in 18% of individuals by current ABI definition and in 32% by modified method. Diabetes, cardiovascular diseases, metabolic syndrome, higher levels of systolic blood pressure and pulse pressure, elevated risk by Framingham Risk Score (FRS), and a higher number of total and antihypertensive drugs in use were associated with low ABI by both definitions. Smoking and LDL-cholesterol were associated with low ABI only by the modified definition. Low ABI by the modified definition detected 9 new cases of PAD but cardiovascular risk had not been considered high in 3 patients when calculated by FRS. In conclusion, given that a simple modification of ABI calculation would be able to identify more patients at high risk, it should be considered for cardiovascular risk prediction in all elderly hypertensive outpatients.

Hypertension affects approximately 26% of the adult population and it is a leading cause of death in up to 13.5% worldwide. Hypertension is a disease with the highest prevalence in Indonesia, so good treatment and prevention are needed. Several studies suggest that there is a correlation between hypertension risk factors and the disturbance of sleep quality. Therefore, it is necessary to develop preventive and promotive efforts to obtain optimum blood pressure in patients with hypertension to avoid complications or even death. This study aims to find out the correlation between sleep quality and the value of blood pressure in the elderly ?60 years old in Batu City. A cross-sectional study was conducted in Batu City in 2020. Stratified random sampling was performed to select the respondents. A validated PSQI questionnaire and sphygmomanometer were used to assess sleep quality and blood pressure, respectively. A Chi-Square test was used to test the hypothesis. There were 391 respondents involved in this study. Most of the respondents have poor sleep quality, 205 respondents (52,43%) of which 41 respondents (20%) have normal blood pressure, and 164 respondents (80%) have high blood pressure. The statistical analysis shows a significant correlation (p=0,000) between sleep quality and blood pressure. In conclusion, hypertension is associated with poor sleep quality in the geriatric population in Batu City.

AbstrakLatar belakang: Sampai saat ini, hipertensi masih merupakan tantangan besar di Indonesia. Kepatuhan pasien dalam menggunakan obat berpengaruh terhadap keberhasilan terapi pengobatan. Adanya ketidakpatuhan pasien hipertensi dalam minum obat dapat memberikan efek negative yang sangat besar, seperti munculnya komplikasi. Tujuan: Mengetahui hubungan kepatuhan minum obat dengan tekanan darah pada penderita hipertensi di Puskesmas Karangjambu Kabupaten Purbalingga. Metode: Desain penelitian ini adalah cross sectional study. Responden dalam penelitian ini adalah pasien hipertensi di Puskesmas Karangjambu Kbupaten Purbalingga. Sampel diambil menggunakan teknik accidental sampling dengan jumlah 86 responden. Data dikumpulkan pada bulan Mei 2021 menggunakan kuesioner 8 – Item Morisky Medication Aderence Scale (MMAS – 8) dan pengukuran tekanan darah secara langsung. Hasil: Penelitian ini menunjukkan bahwa Sebagian besar responden adlaah perempuan (86%), sebgain besar berada pada kategori tingkat kepatuhan sedang (47%) dan pada kategori hipertensi tahap 2 (63%). Uji stastistik didapatkan nilai p value 0.901 menunjukkan bahwa tidak ada hubungan kepatuhan minum obat dengan tekanan darah pada penderita hipertensi. Kesimpulan: Tidak ada hubungan kepatuhan minum obat dengan tekanan darah pada penderita hipertensi AbstractBackground: In Indonesia, hypertension is still a major problem. Adherence to treatment is critical for hypertension patients' long-term health and well-being. Adherence are required for hypertension therapy to be effective, and altering these patients' behavior has the greatest potential for improving hypertensive control. Objectives: To explore the relationship between medication adherence and blood pressure in hypertension patients. Methods: We used the cross-sectional study. This research sample was hypertension patients at Karangjambu Health Centre Purbalingga Regency. We included 89 respondents by accidental sampling technique. We collected the data by questionnaire. The Gamma & Sommers’d test was performed to analyze the data. Results: This research showed that that the majority of respondents were women (86%), the moderate category had the highest level of adherence (47%) and the most respondents suffer from hypertension stage 2 (63%). The statistical test result of 0.910 suggested that there was no relationship between medication adherence and blood pressure in hypertensive patients. Conclusions: There was also no significant between medication adherence and blood pressure.

INTRODUCTION: blood pressure System (SNS) has been considered as the ultimate integrator of the systems’ physiology on Blood Pressure (BP) control. Posture is also regulated by SNS. Systems which regulate BP also act on postural control. OBJECTIVE: To test the hypothesis of an association between postural misalignments (PM) and BP fall in hypertensive individuals. METHODS: Exploratory study using a sample of 40 hypertensive individuals, who regularly use antihypertensive drugs. All of them underwent Ambulatory Blood Pressure Monitoring (ABPM) and posture assessment, through Postural Assessment Software (PAS). To test association between posture angles and BP variables, the student’s t-test and Mann-Whitney tests were used, at a 5% level of significance. This study is registered at clinical trials, under the number NCT02401516. RESULTS: For Systolic Blood Pressure (SBP), anterior trunk shift presented smaller awake/asleep variation (14.7%vs25.3%, p=0.01), and flexing ankle for higher BP loads: 21.9%vs7.8% for total load (p=0.02), 21.8%vs9% for load during the period awake (p=0.04) and 21.9%vs7.9% for load during the period asleep (p=0.02). For Diastolic Blood Pressure (DBP), posterior trunk shift presented higher pressure load (24.0%vs16.2%, p=0.04), and anterior trunk shift presented smaller awake/asleep variation (14.4%vs25.5%, p=0.01) and flexing hip presented higher BP load (29.4%vs18.3%, p=0.02). From posture scores, the PM presented smaller awake/asleep variation for SBP (13.7%vs22.8%, p=0.03) and DBP (11.5%vs23.5%, p=0.01). CONCLUSION: PM can be associated with pressure fall. Three or more alterations in posture angles are associated with smaller awake/asleep BP variation.

Background: Dementia is a multifactorial disease due to genetic and environmental influences. Lack of intake of vitamins B6, B12, folic acid and blood pressure are one of the environmental factors that cause dementia.Objective: To analyze the relationship between the adequacy of vitamins B6, B12, folic acid and blood pressure with dementia in the elderlyMethod: This research was a cross sectional study, which was carried out in Tulungagung Regency. The sampling technique was cluster random sampling and a sample size of 100 elderly from 7 elderly Posyandu in Tulungagung Regency, with independent variables such as adequacy of vitamin B6, B12, folic acid and blood pressure, while the dependent variable was dementia. Data were analyzed with chi square testResults: Respondents in this study were generally aged 60-74 years (70.0%), women (48.0%), were educated in elementary / junior high school (60.0%) and did not work (69.0%). Most respondents have insufficient vitamin B6, B12 and folic acid in the less category, with percentages of 57.0%, 54% and 64%, respectively. There was an association between the adequacy of vitamins B6, B12 and folic acid with dementia in the elderly (OR=2.302; 95% CI=1.011-5.241; p=0.045; OR=2.397; 95% CI=1.060-5.422; p=0.034; OR=2.516; 95% CI=1.015-6.238; p=0.043). Blood pressure is associated with dementia in the elderly (OR=3.000; 95% CI=1.288-6.988; p=0.010). The average consumption of vitamin B6, B12 and folic acid in the normal group was higher than the dementia group, with a difference of 0.16 ± 0.04 mg, 0.54 ± 0.05 mcg and 65.89 ± 4.95 respectively mcgConclusionABSTRAKLatar Belakang: Demensia merupakan penyakit multifaktorial karena pengaruh genetik dan lingkungan. Minimnya asupan vitamin B6, B12, asam folat dan tekanan darah menjadi salah satu faktor lingkungan yang menyebabkan demensia.Tujuan:  Tujuan penelitian ini adalah menganalisis hubungan antara kecukupan vitamin B6, B12, asam folat dan tekanan darah dengan demensia pada lanjut usiaMetode: Penelitian ini berupa penelitian cross sectional, yang dilakukan di Kabupaten Tulungagung. Teknik sampling dengan cluster random sampling dan besar sampel 100 lansia dari 7 posyandu lansia di Kabupaten Tulungagung, dengan variabel bebas berupa kecukupan vitamin B6, B12, asam folat dan tekanan darah, sedangkan variabel terikatnya adalah demensia. Data dianalisis dengan uji chi squareHasil: Responden penelitian ini umumnya berusia 60-74 tahun (70,0 %), wanita (48,0 %), berpendidikan terakhir SD/SMP (60,0 %) dan tidak bekerja (69,0 %). Kebanyakan responden memiliki kecukupan vitamin B6, B12 dan asam folat dalam kategori kurang, dengan persentase masing-masing  57,0 %, 54 % dan 64 %. Ditemukan hubungan antara kecukupan vitamin B6, B12 dan asam folat dengan demensia pada lanjut usia (OR=2,302, 95% CI=1,011-5,241, p=0,045; OR=2,397, 95% CI=1,060-5,422, p=0,034; OR=2,516,95% CI=1,015-6,238, p=0,043). Tekanan darah berhubungan dengan demensia pada lanjut usia (OR=3,000,95% CI=1,288-6,988,p=0,010). Rata-rata konsumsi vitamin B6, B12 dan asam folat kelompok normal lebih tinggi dibandingkan kelompok demensia, dengan selisih masing-masing sebesar 0,16±0,04 mg, 0,54±0,05 mcg dan 65,89 ±4,95 mcg.Kesimpulan: Terdapat hubungan antara kecukupan vitamin B6, B12, asam folat dan tekanan darah dengan demensia pada lanjut usia.
