Introduction - Myopia is the most common Refractive error of the eye seen in children and young adults and it has become more prevalent in recent years and is a cause of concern the world over. Myopia commonly referred to as near or shortsightedness. Some of the clinical features of Vataja Timira can be correlated with Myopia. Timira is counted under the Drishtigata Rogas [1] described by Sushruta. Timira involving first and second patala in terms of symptoms, anatomical structures involved and the pathogenesis of the disease. Timira begins with the blurring of vision gradually leading to the deterioration of eyesight. Shastrokta lakshanas of Vatika Timira such as bhramantiva sa pashyati, aavil darshan, vyaavidha darshan, aruna darshan [2]. Materials and Methods - A 20-year-old male, came to Shalakya Tantra OPD in National Institute of Ayurveda, deemed to be University Jaipur (R.J.), with chief complaints of defective vision for distance in both eyes, pain in both eyes and watering from both eyes since 4 months. He was treated with Nasya, Netra Pariseka, one sitting of Tarpana, Triphala guggulu, Patoladi eyedrops and Netramritam eyedrops. Result - At the end of the study, improvement in the visual acuity in both eyes, improvement in pain in both eyes and watering from both eyes have reduced. Conclusion -Thus, it can be concluded that Ayurvedic treatments are helpful to control Myopia. Keywords: Myopia, Nasya, Tarpana, Netra pariseka, Timira, Triphala Guggulu.

Amongst various differential diagnoses possible for intra-oral (palatal) mass, possibility of fibro-lipoma is extremely rare but documented in literature, and hence should be a consideration. Such patient requires thorough evaluation and step-wise approach to treat. This case study documents for a 30-year-old male patient presenting with such mass and chief complaints of post nasal drip, nasal obstruction (right side>left side) and intermittent headache for 3 years. After radiological and endoscopic examination, the mass was surgically removed in toto via dual route (trans-oral and endoscopic). Lipomas are uncommon tumors in the oral cavity, and palate being the rarest amongst them as is our case. It would be strongly advocated to keep lipoma in the differential diagnosis when evaluating a palatal mass. Also, it is worth mentioning and recommending that a careful radiological examination of large lesions arising from the palate is a must before approaching for surgery.

Introduction: Charcot’s triad was traditionally used to diagnose ascending cholangitis. However it is already proven that only minority of patients with ascending cholangitis who fulfill the triad of fever, jaundice and right hypochondriac pain. Aim: We would like to highlight the rarity of severe hyperbilirubinaemia secondary to benign cause as most of the incidence raised more suspicion for primary liver disease or malignancy. Case study: We presented a case report of a 58-year-old male patient with no comorbid who presented to us with right hypochondriac pain and obstructive jaundice with severe hyperbilirubinaemia (total bilirubin 1025 µmol/L), without fever or leukocytosis. Results and discussion: We presented a case report of a 58-year-old male patient with no comorbid who presented to us with right hypochondriac pain and obstructive jaundice with severe hyperbilirubinaemia (total bilirubin 1025 µmol/L), without fever or leukocytosis. Conclusions: Benign conditions such as common bile duct stones still can lead to severe hyperbilirubinaemia even though it is very rare. The usage of appropriate imaging is needed to exclude malignant causes.

Purpose. To study the effect and outcome of intravitreal Ziv-Aflibercept (IVZ) in treatment of Chronic Central Serous Chorioretinopathy (CSCR) related Choroidal Neovascular Membrane (CNVM). Methods. A case report of 48-year-old male patient treated with 1.25 mg/0.05 ml IVZ (total 3 doses at monthly intervals) in CSCR related CNVM. Pre- and posttreatment fundus fluorescein angiography (FFA) and Optical Coherence Tomography (OCT) were done to document response along with improvement in visual acuity. Patients. Single eye of a 48-year-old male patient. Results. Regression of CNVM was noted with improvement of macular contour and thickness on OCT and cessation of leakage on FFA. Visual acuity improved from 3/60, <N36 to 6/12, N12. Discussion. Anti-VEGF injections have shown benefit in treatment of CNVM. There is very little information about benefit of IVZ in CSCR related CNVM. Conclusion. IVZ is effective in regression of CSCR related CNVM and is associated with better macular anatomy and improved visual function.

Jaundice is a yellow pigmentation of the skin, the conjunctival membrane over the sclera and other mucous membrane caused by hyperbilirubinemia (increase level of bilirubin in blood). Today’s lifestyle with unhygienic and poor dietary habit and alcoholic habits, etc. which are responsible factor to promote hepatic damage which clinically reflect as kamala In this case study 25 years male patient having kamala who was suffering from pain in abdomen, weakness, anorexia, burning maturation and fever on and off, the patient was treated with shodhan chikitsa (Virechan with panchatikta ghrita) followed by shaman chikitsa. Patient got significant result as per the values of bilirubin with symptomatic relief in complaints within 30 days, Kamala can be successfully managed by shodhan and shaman chikitsa , the effect of ayurvedic treatment was assessed in relation to improvement in overall clinical sign and symptoms and biochemical investigations. Further study will be needed as per different assessment criteria.

This case report describes a 66 years old male patient with pellucid marginal degeneration (PMD). The patient had fallen out of a regular follow-up scheme and presented with a poorly fitting corneal rigid lens that he had worn for almost five years. Correction had failed to improve vision in the left eye, so the patient wears a contact lens in his right eye only. Scleral lenses vault over the cornea, the tear layer between the lens and the cornea masks corneal irregularities and are a good correction option for corneal ectasias. This patient was fitted with an Onefit scleral lens that provided good visual acuity and comfort. Lens handling was no obstacle to this patient. A heart surgery postponed the first follow-up examination. Follow-up examination 8 months after the refit resulted in no changes of lens parameters as the fitting was still acceptable and the patient was happy. Attention to the inferior limbal clearance will be the focus at the next follow-up in 6 months’ time.

Background Ocular and visual problems are frequent consequences of an acquired brain injury (ABI). The literature suggests that many patients demonstrate a myopic shift in refractive error after a traumatic brain injury (TBI), a form of an ABI. Three reported courses are suggested. Most commonly the myopia resolves under cycloplegic conditions, secondly the shift is transient, and least commonly the myopia increases over time. This case report, although not due to a TBI, is an example of the least common subgroup. Case Report The patient, a 63 year-old male, presented with a history (15 months prior to initial evaluation) of an excised right frontal temporal lobe tumour. The history included dexamethasone 4mg QD pre- and post-surgery which caused blood glucose levels to fluctuate between 5.72-22.2 mmol/l; a stroke occured postsurgery causing hemiparesis on his left side; and shunt surgery to decrease swelling to the area of excision and the optic nerves. Medications included: Cartia, lisinopril, Toprol XL, prednisone, Reglan, and Temodar. The patient was receiving chemotherapy to treat remnants of the tumour. His last eye exam in 2004 indicated a refractive error of -2.50-1.25X080 in his right eye and -3.000.75X080 in his left eye with visual acuity of 6/7.5 in both the right and left eye. He presented to our clinic with an aided visual acuity of 8/100 (6/110) in both the right and left eye and a left homonymous hemianopsia on finger counting fields. Cycloplegic retinoscopy revealed -6.003.00X090 in his right eye and -6.002.00X090 in his left eye. Dilated fundus exam showed mild swelling of both optic nerves. The patient reported blood glucose levels of 103-140. At a one-month follow-up, his best corrected visual acuity at a distance improved to 6/60 in both the right and left eye through the new prescription. His cycloplegic retinoscopy did not show any change in myopia or astigmatism and the disk edema was resolved. Conclusion This case illustrates an unusual presentation of myopic shift post ABI. The patient’s condition continued to deteriorate eventually taking his life. It remains unclear whether the myopia and astigmatism continued to increase or remained stable. Although, the cause of this change is not fully understood and needs further investigation, providing appropriate optical correction did improve the patient’s quality-of-life.

Adenomatoid tumor is an uncommon benign mesothelial neoplasm, usually localized in the epididymis. It is the most common paratesticular tumor of middle-aged patients (average age of clinical presentation: 36 years). However, these tumors in pediatric and pubertal patients are extremely rare. Due to their rarity, we present a case of adenomatoid tumor of the tail of the epididymis in a 16-year-old patient. After systematic research of the current literature, we did not find another case report of epididymal adenomatoid tumor in a male patient aged 16 years old or less. This notice and our concern, as well, about the patient’s surveillance protocol during the postoperative period were the motive for this case study.

Nadi Vrana is a term used to describe the sinus. Sinus is a blind track lined by granulation tissue from an epithelial surface.it can be treated mainly by Ksharasutra therapy and by other methods which include varti, Lepana etc. In Susrutha Samhitha Chikitsasthana first chapter Dwivarineeya chikitsa; it is mentioned that the Vrana Varthi 1can be used in Nadi Vrana where the wound is having minute opening, deep and going into the Mamsadhatu (muscles). In the present case study, a 44-year-old male patient visited the OPD with Grade 1 inter sphincteric sinus in the perianal region and the patient was selected for application of the Ghontaphaladi Varthi. Daily application of Ghontaphaladi Varthi has shown significant changes in the healing of the track and a decrease in the severity of the associated symptoms like pain and discharge.
