Introduction: Pathology of the rotator cuff is the cause of most common problems at the shoulder joint and accurate diagnosis is essential for appropriate management. High-resolution real-time ultrasonography and MRI are successful imaging modalities for both rotator cuff and non–rotator cuff disorders and have important implications in the management of rotator cuff pathologies. This study was conducted to Compare the results obtained after the evaluation of rotator cuff injuries of shoulder joint by high resolution Ultrasonography and Magnetic Resonance Imaging. Methods: 50 patients with suspected rotator cuff injuries of the shoulder joint were included in this study over a period of 2years. Patients had presented with various complaints like prolonged pain in the shoulder joint with stiffness, restriction and difficulty in movement of the shoulder joint, traumatic injuries and recurrent dislocations of the shoulder joints. Results: Mean age was 41.6years ( range 15-80 ).Maximum patients were between 41-50 years (24%). 29 were males and 21 were females. In our study diagnostic accuracy of ultrasound in rotator cuff tears was 57. 14% for full thickness tears and 58.33 for partial thickness tears. The diagnostic accuracy of ultrasound in detection of complete tear of the supraspinatus muscle was 57.14% while that of MRI was 100%. Conclusion: High resolution Ultrasound being a dynamic study can be used as the first-line investigation for rotator cuff tear. Full-thickness rotator cuff tears can be identified using ultrasound and MRI with comparable accuracy. Objective: The objective of this study was to outline a novel unique concept of secondary impingement of the muscles, myotendons, and tendons of the rotator cuff from hypertrophy as a result of strength training exercises. Methods: In this retrospective observational study, 58 patients were referred for an magnetic resonance imaging (MRI) by the orthopedic surgeon to the radiology department over a period of 1½ years. All patients gave a history of strength training exercises and presented with clinical features of rotator cuff impingement. Results: We identified features of hypertrophy of rotator cuff muscles, myotendons, and tendons in 12 of these 58 patients. This was the only abnormality on MRI. The hypertrophy of rotator cuff muscles and tendon bulk completely filling the subacromial space to the point of overfilling and resulting in secondary compressive features. Conclusion: Rotator cuff impingement is a common phenomenon that can occur with various inlet and outlet pathological conditions. However, rotator cuff impingement may also result from muscle and tendon hypertrophy from strength training regimens. Hypertrophy of the rotator cuff can result in overfilling of the subacromial space, leading to secondary impingement, which we have termed as “pseudo-impingement.”

AbstractMuscle and tendon injuries about the shoulder and upper arm are commonly evaluated and characterized with magnetic resonance imaging. This article reviews our experience with abnormalities of the rotator cuff tendons and muscles as well as the deltoid muscle in the shoulder. We discuss and illustrate abnormalities of the upper arm such as strain injuries of the pectoralis major, strain injuries of the teres major and latissimus dorsi, and contusion and crush injuries of the biceps and brachialis muscles in the upper arm.

<p class="abstract"><strong>Background:</strong> Shoulder pathologies can cause significant pain, discomfort, and affect the activity of daily living. The aim of this study was to compare the efficacy of clinical examination, ultrasound, magnetic resonance imaging (MRI) with shoulder arthroscopy in diagnosing various shoulder pathologies, considering shoulder arthroscopy as the gold standard tool.</p><p class="abstract"><strong>Methods:</strong> This was a prospective, comparative study conducted over 35 patients, between 18-75 years of age presenting with chronic shoulder pain or instability of more than 2 months duration. All patients were examined clinically, followed by high resolution ultrasound, MRI, arthroscopy of the affected shoulder.<strong></strong></p><p class="abstract"><strong>Results:</strong> The sensitivity and specificity of ultrasonography (USG) for diagnosing full thickness tear was 100% each and for MRI was 88% and 100% respectively. For subacromial impingement USG had sensitivity of 66.67%, specificity of 94.12%, positive predictive value of 50% and negative predictive value of 88.89%. For rotator cuff tear USG had sensitivity of 92.86%, specificity of 50%, positive predictive value of 81.25% and negative predictive value of 75% considering shoulder arthroscopy as gold standard.</p><p class="abstract"><strong>Conclusions:</strong> USG and MRI both are sensitive techniques for diagnosing of rotator cuff pathologies. USG has high accuracy in diagnosing partial thickness tears as compare to MRI. MRI proved to be superior in estimation of site and extent of tear. Considering shoulder arthroscopy as gold standard, it can be reserved for patients with suspicious of USG/MRI findings or those who may need surgical intervention simultaneously.</p>
