IntroductionLockdown measures have a profound effect on many aspects of daily life relevant for diabetes self-management. We assessed whether lockdown measures, in the context of the COVID-19 pandemic, differentially affect perceived stress, body weight, exercise and related this to glycemic control in people with type 1 and type 2 diabetes.Research design and methodsWe performed a short-term observational cohort study at the Leiden University Medical Center. People with type 1 and type 2 diabetes ≥18 years were eligible to participate. Participants filled out online questionnaires, sent in blood for hemoglobin A1c (HbA1c) analysis and shared data of their flash or continuous glucose sensors. HbA1c during the lockdown was compared with the last known HbA1c before the lockdown.ResultsIn total, 435 people were included (type 1 diabetes n=280, type 2 diabetes n=155). An increase in perceived stress and anxiety, weight gain and less exercise was observed in both groups. There was improvement in glycemic control in the group with the highest HbA1c tertile (type 1 diabetes: −0.39% (−4.3 mmol/mol) (p<0.0001 and type 2 diabetes: −0.62% (−6.8 mmol/mol) (p=0.0036). Perceived stress was associated with difficulty with glycemic control (p<0.0001).ConclusionsAn increase in perceived stress and anxiety, weight gain and less exercise but no deterioration of glycemic control occurs in both people with relatively well-controlled type 1 and type 2 diabetes during short-term lockdown measures. As perceived stress showed to be associated with glycemic control, this provides opportunities for healthcare professionals to put more emphasis on psychological aspects during diabetes care consultations.

Hereditary tyrosinemia type 1 (HT1) is an inherited condition in which the body is unable to break down the amino acid tyrosine due to mutations in the fumarylacetoacetate hydrolase (FAH) gene, coding for the final enzyme of the tyrosine degradation pathway. As a consequence, HT1 patients accumulate toxic tyrosine derivatives causing severe liver damage. Since its introduction, the drug nitisinone (NTBC) has offered a life-saving treatment that inhibits the upstream enzyme 4-hydroxyphenylpyruvate dioxygenase (HPD), thereby preventing production of downstream toxic metabolites. However, HT1 patients under NTBC therapy remain unable to degrade tyrosine. To control the disease and side-effects of the drug, HT1 patients need to take NTBC as an adjunct to a lifelong tyrosine and phenylalanine restricted diet. As a consequence of this strict therapeutic regime, drug compliance issues can arise with significant influence on patient health. In this study, we investigated the molecular impact of short-term NTBC therapy discontinuation on liver tissue of Fah-deficient mice. We found that after seven days of NTBC withdrawal, molecular pathways related to oxidative stress, glutathione metabolism, and liver regeneration were mostly affected. More specifically, NRF2-mediated oxidative stress response and several toxicological gene classes related to reactive oxygen species metabolism were significantly modulated. We observed that the expression of several key glutathione metabolism related genes including Slc7a11 and Ggt1 was highly increased after short-term NTBC therapy deprivation. This stress response was associated with the transcriptional activation of several markers of liver progenitor cells including Atf3, Cyr61, Ddr1, Epcam, Elovl7, and Glis3, indicating a concreted activation of liver regeneration early after NTBC withdrawal.

Purpose The purpose of this study is to examine the short-term efficacy (3 months) of early diabetes-specific READY-Girls preconception counseling (RGPC) on more general risk-taking behaviors, condom use, and sexually transmitted infections (STIs) among adolescent females with type 1 diabetes. Methods Secondary analysis was performed with data pooled from 2 independent randomized controlled trials to evaluate the short-term impact of RGPC. The pooled sample had 136 participants (mean age, 16.9 years; range, 13-19 years) and compared those who received the RGPC (n = 76) with a control group who received standard care (n = 60). Both groups self-reported on demographic characteristics, risk-taking behaviors (eg, substance use and unsafe sex), birth control, and STIs. Results No effect of RGPC emerged on risk-taking behaviors, condom use, and STIs. Only 25% (n = 36) of the adolescents were sexually active at baseline, and 29% (n = 39) were sexually active at 3 months. Their overall mean age of sexual debut was 15.4 years, with more than half reporting an episode of unprotected sex. Condoms were the most frequent type of birth control used by both groups at both time points. By 3 months, only 4 participants had been diagnosed with an STI. Over time, subjects in both groups became more sexually active and used more condoms. Conclusion RGPC did not appear to directly affect general risk-taking behaviors or STIs, since it focuses on diabetes and reproductive health issues. Condom use did increase over time in both groups. More information on risk-taking behaviors and STIs should be included in diabetes-specific preconception counseling programs, including RGPC.

In the process of sufficiently considering fuel economy of plug-in hybrid electric vehicle (PHEV), the working time of engine will be reduced accordingly. The increased frequency that the three-way catalytic converter (TWCC) works in abnormal operating temperature will lead to the increasing of emissions. This paper proposes the equivalent consumption minimization strategy (ECMS) to ensure the catalyst temperature of PHEV can work in highly efficient areas, and the influence of catalyst temperature on fuel economy and emissions is considered. The simulation results show that the fixed equivalent factor of ECMS has great limitations for the underutilized battery power and the poor fuel economy. In order to further reduce fuel consumption and keep the emission unchanged, an equivalent factor map based on initial state of charge (SOC) and vehicle mileage is established by the genetic algorithm. Furthermore, an Adaptive changing equivalent factor is achieved by using the following strategy of SOC trajectory. Ultimately, adaptive equivalent consumption minimization strategy (A-ECMS) considering catalyst temperature is proposed. The simulation results show that compared with ordinary ECMS, HC, CO, and NOX are reduced by 14.6%, 20.3%, and 25.8%, respectively, which effectively reduces emissions. But the fuel consumption is increased by only 2.3%. To show that the proposed method can be used in actual driving conditions, it is tested on the World Light Vehicle Test Procedure (WLTC).

In this paper, the HDM car-following model, the IIDM car-following model and the IDM car-following model with a constant-acceleration heuristic is utilized to explore the effects of ACC/CACC on the fuel consumption and emissionsat the signalized intersection. Two simulation experiments are studied: (i) one with free road ahead and (ii) the second with a red light 300 m downstream at the second intersection. The numerical results show that CACC vehicle is the best vehicle type among the three vehicle types from the perspective of vehicle’s cumulative fuel consumptions and cumulative exhaust emissions. The results of this paper also suggest a very high environmental benefit of ACC/CACC at little or no cost in infrastructure.
