The purpose of this chapter is to review the literature concerning the psychopharmacology of personality disorders. We first review the double-blind placebo-controlled pharmacologic studies of all the personality disorders, except for borderline personality disorder (BPD). Attention is then focused on BPD. While there have been many open-labeled studies, there are fewer than 30 randomized-controlled trials even for this personality disorder. The section on borderline personality disorder will conclude with guidance as to how to think about the management of the actual pharmacologic treatment of this disorder, including matters such as dosage, lethality, augmentation, and avoiding polypharmacy. We conclude with a discussion of the obstacles hampering the development of a more substantial database that could provide guidance and direction.

Psychotherapy 2019; 56: 260–273 Die Borderline-Persönlichkeitsstörung (BPS) ist eine häufige Erkrankung im klinisch-stationären Setting und ist u. a. durch Probleme im zwischenmenschlichen Bereich gekennzeichnet. Zur Behandlung werden i. d. R. hauptsächlich psychotherapeutische Methoden angewandt. Die hier vorgestellte Metaanalyse weist insbesondere der Gruppenpsychotherapie eine hohe Wirksamkeit nach.

This paper reviews recent literature on the psychopharmacologic management of borderline personality disorder (BPD) patients and discusses an approach to drug therapy. Five randomized controlled trials have shown positive, but non-specific effects of antipsychotic drugs on the symptoms suffered by BPD patients. There were too few data on other types of drugs to draw any conclusion. We propose that BPD patients be treated on the basis of being in a state or episode of co-existing Axis I disorder.
