Articular cartilage is a specific type of connective tissue composed of hydrated proteoglycans within a matrix of collagen fibrils. In the elderly population, it shows degenerative changes that may results in osteoarthritis. The more severe form of osteoarthritis occasionally demonstrates bone formation within the cartilage, which is designated as a bony protuberance, however, such lesions are rare in children. This report presents the case of a 10-year-old boy with a bony protuberance within the articular cartilage of the knee. The patient initially complained of knee pain and he subsequently developed flexion contracture. Radiological and arthroscopic examinations revealed a bony protuberance in the articular cartilage and degenerative changes of the cartilage above it. He was successfully treated by the removal of the bony protuberance and osteochondral grafting. The bony protuberance may have caused cartilage degradation since the thickness of the cartilage above it was thinner than that around the lesion. The bony protuberance within the articular cartilage formed in the younger population may be a possible cause of osteoarthritis. This case is a noteworthy with regard to the pathogenesis of osteoarthritis.

Degenerative aortic valve disease (DAVD) is the most common heart valve pathology and is especially prevalent in the elderly population. Studies have shown that stenosis, the most severe form of DAVD, increases in prevalence from 0.7% in people between 18 and 44 years of age to over 13% of people over 75 years of age. Furthermore, early symptoms of DAVD have been detected in 29% of patients over 65 years of age. These symptoms are associated with a 50% increase in cardiovascular related morbidity and a similar increase in the risk of myocardial infarction [1]. Currently, aortic valve (AV) replacement surgery is the only method available to correct DAVD, and while these procedures have become less invasive, replacement valves are still rather expensive and do not function as well as native valves. Therefore, the development of a therapeutic that could prevent DAVD would greatly benefit many patients.

Eye injuries have been shown to be greater in the elderly population as a possible cause of this greater incidence. Three separate impact scenarios simulating a foam particle (30 m/s), a streering wheel (15 m/s), and an air bag (67 m/s), were applied to a finite element eye model in order to reveal the effects of aging on the eye when subjected to blunt trauma. The lens stiffness of the model was varied according to human lens stiffness values determined for each age group. The computational eye model demonstrated that increased risk was associated with the increasing stiffness of the lens, producing up to a 120% larger stress in the ciliary body.

Dysphagia (subjective difficulty in swallowing) is a common clinical problem, particularly in the elderly population. Ideally, patients should be assessed promptly in the appropriate outpatient setting, but frequently they will be admitted on an acute medical take due to ‘total’ dysphagia, dehydration, debilitation or possible aspiration. A careful history and examination can usually elucidate the site of the lesion and its possible cause, and thus direct initial management. This article will concentrate on the role of the acute physician in the assessment of patients with dysphagia within the first 48 hours of presentation.

Abstract. This study examines the psychometric properties of the IQCODE and its applicability in the Iranian elderly population. A group of 95 elderly patients with at least 4 years of formal education who fulfilled the criteria of DSM-IV-TR for dementia were examined by the MMSE and the AMTs. The Farsi version of the IQCODE was subsequently administered to their primary caregivers. Results showed a significant correlation ( p = .01) between the score of the questionnaire and the results of the MMSE ( r = −0.647) and AMTs ( r = −0.641). A high internal reliability of the questionnaire was confirmed by Cronbach’s alpha coefficient (α = 0.927) and test-retest reliability by correlation coefficient ( r = 0.81). This study found that the IQCODE has acceptable psychometric properties and can be used for evaluating the cognitive state in the elderly population of Iran.

Sacral insufficiency fractures (SIFs) are a cause of debilitating low back pain that is often difficult to diagnosis and manage. The diagnosis of SIF is often delayed due to inaccurately attributing symptoms to spondylosis, which is a commonly present in the elderly population where SIFs are most prevalent. Historically, treatment consisted of medical management and open reduction internal fixation reserved for severe cases. However, percutaneous sacroplasty has emerged as a minimally invasive treatment option which provides early pain relief without significant complications. The objective of this article is to raise awareness of SIFs and percutaneous sacroplasty as an effective and safe treatment method.
