The present paper aims to assess the effect of lotrafilcon A contact lenses (CL) <em>in situ</em> on intraocular pressure (IOP) measurement performed with three portable tonometers (ICare, Tonopen and Perkins). This cross-sectional study included thirty young healthy subjects. Intraocular pressure measurements without CL were performed first, followed by IOP measurements with CL for twenty minutes. ICare IOP measurements obtained with lotrafilcon A CL overestimated IOP values without CL by 1 mmHg (P&lt;0.001). However, both techniques displayed close level of agreement (95% LoA, -4.17 to +1.63 mmHg). Also, differences between both methods tended not to increase (P=0.9). No significant differences were observed between IOP measurements without and with CL for Perkins (paired-t test, P=0.23) and Tonopen XL (pairedt test, P=0.17). In conclusion, adequate IOP measurements through zero power lotrafilcon A silicone hydrogel CL can be obtained with all three tonometers in healthy eyes, although, in case of ICare, the practitioner must be aware of the 1 mmHg overestimation with CL.

Objective. To determine whether accommodation induced by reading alters intraocular pressure (IOP) in healthy, young, emmetropic adults and to document the duration and magnitude of this effect. Design. Cross-sectional study. Participants. Fifteen healthy, emmetropic young adults. Methods. Subjects performed 20 minutes of near work (reading at 33 cm) followed by 20 minutes of far work (reading at 520 cm) while IOP was measured using an iCare tonometer at baseline and every 5 minutes thereafter. Statistical analysis was performed using repeated measures ANOVA. Main Outcome Measures. Intraocular pressure. Results. IOP decreased significantly compared to baseline IOP after 10 minutes of near work (average change of −1.60 ± 2.2 (SD) mm Hg, p<0.05). IOP remained lower than baseline IOP throughout all subsequent near and far work. The difference in IOP at the end of experimentation compared to baseline IOP was −1.87 ± 1.81 mm Hg (p<0.05). The minimum IOP reached during experimentation compared to baseline was on average −3.8 ± 2.2 (SD) mm Hg (range: 0 to −8.0 mm Hg). 13 of 15 subjects (87%) and 9 of 15 subjects (60%) had at least one IOP measurement of at least 2 mm Hg and 4 mm Hg less than their baseline IOPs, respectively. Conclusions. Near work decreases IOP in healthy emmetropes, and this effect is sustained for at least 20 minutes after discontinuing prolonged near work. Providers may need to consider this effect when measuring IOP in clinical practice.

AbstractThis population-based cross-sectional study was performed to determine the mean corneal endothelial cell density (ECD), coefficient of variation (CV), and hexagonality (HEX), and their associations with myopia in Japanese adults living in Chikusei city. Of 7109 participants with available data, 5713 (2331 male and 3382 female) participants were eligible for analysis. After assessing the relationship between participant characteristics and spherical equivalent refraction (SER), the association of SER with the abnormal value of ECD (< 2000 cells/mm), CV (≥ 0.40), and HEX (≤ 50%) were determined using the logistic regression models adjusting for potential confounders (age, intraocular pressure, keratometric power, height, and antihypertensive drug use). In male participants, there was no statistically significant relationships between SER and endothelial parameters. In female participants, compared to emmetropia, SER ≤ − 6 D had significantly higher odds ratio (OR) of having the abnormal value of CV (OR = 2.07, 95% confidence interval [CI] 1.39–3.10) and HEX (OR = 2.04, 95% CI 1.29–3.23), adjusted for potential confounders, indicating that the high myopia was associated with the abnormal values of CV and HEX. Further adjustment for contact lenses wear partly attenuated these associations. Association between the SER and ECD was not detected.

Introduction: Posterior capsular opacification is a common complication after cataract surgery. Neodynium Yttrium Aluminium Garnet laser capsulotomy is still the preferred treatment for posterior capsular opacification. This study was done to determine the use of Brimonidine eye drop in preventing the rise of intraocular pressure post-Neodynium Yttrium Aluminium Garnet laser capsulotomy. Methods: A descriptive cross-sectional study was conducted in Lumbini eye institute and research center, Bhairahawa, Nepal, in 200 eyes with posterior capsular opacification using Brimonidine from Feb 1, 2019, to July 30, 2019. The Institutional Review Committee approved the study with approval number 0237. A convenient sampling method was used. Pre-capsulotomy best-corrected visual acuity, slit-lamp examination of the anterior segment, and dilated fundus examination were done. Intraocular pressure was measured with Goldmann Applanation Tonometer. Post capsulotomy patients were evaluated after one hour, two hours, and two weeks for intraocular pressure and any complications. The statistical analysis was done using Statistical Package of Social Sciences version 20.0 statistical analysis software. The descriptive statistical analysis of the study was done after the collection of the data. Results: Mean age of patients at presentation was 61.61±SD 1.09. The mean intraocular pressure following Neodynium Yttrium aluminum garnet laser capsulotomy using brimonidine at 1 hour was 12.73±3.3 mmHg.and two hours was 11.98±3.2 mmHg. The mean energy per pulse was 2.3±SD 0.3 mJ. The mean duration of posterior capsular opacification from cataract surgery was 22.28 weeks. Conclusions: Neodynium Yttrium Aluminium Garnet laser capsulotomy had lower intraocular pressure after the Brimonidine eye drop procedure. The maximum mean reduction in intraocular pressure was observed after two hours.
