Background: Several findings show how as the intestinal microbiota regulates some important metabolic and physiological body functions, as production of toxic or useful substances, induction of inflammatory processes and interaction with immune system. Alterations of the intestinal microbiota can occur by changes in composition (dysbiosis), function, or microbiota-host interactions and they can be directly correlated with several diseases as bacterial vaginosis (BV). Objectives: the aim of this study is to understand if there is a correlation between acute intestinal dysbiosis condition and the failure in IVF freeze embryo transfer(FET)cycles, and if a prebiotic and probiotic treatment can improve the success rate. Search strategy: we investigate about the incidence of pregnancy rate in acute-severe dysbiotic patients and prebiotics–probiotics treated patients undergoing to a FET cycle. Selection criteria: 53 patient with acute or severe intestinal dysbiosis undergoing IVF cycle was recruited and randomized in two group. Group A (n=29) (control group) and Group B (n=24) have to transfer two freeze embryos each. Group B was treated with prebiotic and probiotic for at least two months to reduce the dysbiosis condition from acute-severe to mild-moderate level. Main results: After FET in the group A there have been 5 ongoing pregnancy and 3 born baby, in the group B (treated group) there was been 15 pregnancy(p<0,05) and 12 born baby (p<0,05) Conclusion: Our result show that a treatment whit probiotic and prebiotic is able to increase pregnancy rate in IVF cycles of infertile women affected by acute or severe intestinal dysbiosis, by modulation of some crucial mechanisms involved in embryo implantation.

Embryo implantation begins when the blastocyst both assumes a fixed position in the uterus and establishes a more intimate relationship with the endometrium. Successful implantation depends upon hormonal synchronization and development and the receptivity of the endometrium. CD44 is a cell surface molecule that has been implicated in the initial attachment of the embryo. The aim of this work was to study the hormonal levels of P4 in three groups of animals which have a normal pregnancy or an induced reduction in the number of implants. Twelve adult New Zealand does (n = 12) were naturally inseminated with a buck of proven fertility. Blood samples were obtained daily during 20 days of pregnancy. Hormonal determinations were performed by enzyme immunoassay. Animals were divided into three groups: group A (n = 4): control animals; group B (n = 4): endometriosis was surgically induced in the right horn a month before the animal was mated; and group C (n = 4): animals received an injection of 20 micrograms of anti-CD44 in the right horn via mid-ventral laparotomy on Day 6.5 post-coitum (0.5 mL each, from the ovarian end to the cervix). Each animal served as her own control with the left uterine horn receiving 0.5 mL of saline. Statistical analysis was performed using ANOVA.The number of corpora lutea was similar in all treatments. No statistical differences were found comparing CLs in the right/left ovary. In group A, a mean of 3.4 ± 0.47 (mean ± SE) implants was found in the right horn while the mean in the left side was 4.6 ± 0.81. In group B, a marked reduction in implantation sites was found, with 1.8 ± 0.60 and 4.66 ± 0.84 on the right and left horns, respectively. With anti-CD44 injected into the uterine horn (Group C), a mean of 0.12 implant was present in the right uterine horn compared with 3.6 implants on the left side (P < 0.001). Progesterone levels from Days 1 to 10 are shown in the following table (mean ± SE). Comparisons in day values are not statistically significant P > 0.05. After Day 10 the levels of progesterone were similar in all groups. The results showed an increase of progesterone levels in group B; this could be due to endometriosis and not to the number of implants. The results in the CD44 group reveal that progesterone profiles were similar to those in the control group, and we can conclude that the reduced number of implants found in group C did not affect the progesterone levels. <b><i>Background:</i></b> The recent studies have shown that sildenafil citrate can enhance estrogen-induced proliferation of the endometrium in infertile women. <b><i>Objective:</i></b> This study was aimed to investigate whether sildenafil citrate could affect pregnancy outcomes in infertile women receiving frozen-thawed embryo transfer (FET) after resection of intrauterine adhesions (IUAs). <b><i>Materials and Methods:</i></b> A total of 310 subjects who met the inclusion criteria were recruited and divided into the control group (group A) and the sildenafil citrate group (or the SC group, group B). The 2 groups were, respectively, divided into 2 subgroups based on the severity of reformed adhesions: (1) group A1 (with mild IUAs) and group A2 (with moderate to severe IUAs) and (2) group B1 (with mild IUAs) and group B2 (with moderate to severe IUAs). Therapeutic effects of sildenafil citrate on the cases were evaluated after resection of IUAs during FET cycles. Endometrial thickness, endometrial pattern, and pregnancy outcomes were evaluated and compared between the 2 groups. <b><i>Results:</i></b> There was no significant difference in the number of embryos transferred between the 2 groups. The endometrial thickness in group B (0.80 [0.68–0.90] cm) was significantly higher than that in group A (0.73 [0.35–0.80] cm). Besides, the biochemical pregnancy rate, clinical pregnancy rate, and live birth rate (LBR) were 71.60, 50.83, and 39.17% in group B, which were significantly higher than those in group A, namely, 57.36, 34.73, and 23.68%, respectively (<i>p</i> &#x3c; 0.05). The univariate analysis and multivariate logistic regression showed that the LBR in either subgroups of group B after vaginal sildenafil treatment was significantly higher than that in the corresponding control group (<i>p</i> &#x3c; 0.05). <b><i>Conclusions:</i></b> It was observed that the administration of sildenafil citrate during FET could effectively improve the poor endometrial conditions after FET following the resection of IUAs.

Background: Polycystic ovary syndrome is the commonest endocrinopathy in anovulatory infertility in young women. It is estimated that infertility affects 10 to 14% of the Indian population of which approximately 25-30% part occupied by PCOS. Methods: This prospective study enrolled 180 infertile women with PCOS, age 21-35 yrs who have taken 1 cycle of clomiphene citrate 100 mg, endometrial thickness <7 mm inspite of follicles greater than 18 mm. Half of them treated with clomiphene citrate with estradiol valerate and remaining half with letrozole. Results: In Group-A treated with clomiphene citrate with estradiol valerate 13 patients (16.3%) conceived and in Group-B treated with letrozole 26 patients (32.5%) conceived. Conclusion: Pregnancy rate is higher in group which treatment with letrozole in comparison with clomiphene citrate plus estradiol valerate. Cellular adhesion molecules are thought to be responsible in embryo attachment in human and animals. Blastocyst attachment to the lining of the mammalian uterus during early implantation involves the initial apposition of the trophoblast to the uterine epithelial surface. A number of cell surface molecules have been implicated in the initial attachment reaction between trophectoderm and maternal surface epithelium, these include heparin-binding epidermal growth factor (HB-EGF), trophinin, CD44, integrins and extra cellular matrix molecules such as osteopontin (OPN) each of which exhibit elevated levels of expression at the time of implantation. CD44 is a membrane glycoprotein that is present in a variety of isoforms and is considered to be the major cell surface receptor for hyaluronan. The aim of these work was to study the effect of CD44 on embryo implantation. The rabbit is potentially an excellent model for study of implantation because it is an obligate ovulator, and therefore pregnancy can be precisely timed. Adult New Zealand rabbit females (n=12) were naturally inseminated with a buck of proven fertility. The day of coitus was counted as ‘Day 0’ of pregnancy. To determine the effect of CD44 at the time of implantation, the animals were divided in three groups: group A (four animals): received 100μg of anti-CD44/mL in an intraperitoneal injection on day 4.5 of pregnancy; group B (four animals), via mid-ventral laparotomies on day 6.5 p.c animals received an injection of 20μg of anti-CD44 on the right horn (0.5mL each, from the ovarian end to the cervix). All these treatments were performed in the right horn. Each animal served as her own control with the left uterine horn receiving 0.5mL of saline. Group C (four animals): This was the control group; two animals received an injection of 2mL of saline and the other two received 0.5mL of saline in each uterine horn. Finally, each animal was sacrificed on Day 10 of the pregnancy, the uterus was removed, and the number of implants were counted in each uterine horn. In the group A: a mean of 4.5±0.47 s.e.m implants were found in the right and on the left side. In group B, a profound reduction in implantation sites was found with the anti-CD44 injected into the uterine horn, a mean of 0 implants in the right uterine horn compared with 3.5 implants on the left side P&lt;0.0001. Group C showed 4.25 implants on the right horn and 5.5 on the left horn; no statistical differences were found. Conclusion: the intraperitoneal injection of the anti-CD44 slightly reduced the number of embryos implanted but this reduction was not statistically significant. Anti-CD44 injected intrauterine affected seriously the embryo survival at the time of implantation.

Background: The current study aims to estimate additional values of laparoscopic intervention for diagnosis and treatment of concomitant pelvic pathologies among infertile women with clomiphene-resistant polycystic ovarian syndrome (PCOS) subjected to laparoscopic ovarian drilling (LOD) in comparison to non-PCOS infertile women subjected to diagnostic/therapeutic laparoscopy.Methods: A prospective cross sectional observational study was carried out in the Endoscopic unit of a tertiary care university hospital.  The study included 232 infertile women planned for laparoscopy were divided into a study group A (116 cases) with clomiphene-resistant PCOS and a control group B (116 cases) without PCOS. Each group was further subdivided into two subgroups according to the presence and absence of risk factors (RF) for adhesion formation. Diagnostic/operative laparoscopy was done. The main study outcome was the prevalence of any pelvic abnormalities seen during laparoscopy.Results: Both groups showed insignificant difference regarding socio-demographic history and basic data. Laparoscopy detected pelvic pathologies in 44 cases (37.9%) and 86 cases (74.1%) in both groups respectively. In group A, we diagnosed pelvic pathologies in 29 (32.6%) and 15 (55.6%) cases with and without RF respectively while in group B they were diagnosed in 76 (84.4%) and 10 (38.5%) cases with and without RF respectively. If compared to women with unexplained infertility, PCO patients without risk factors have an insignificant but higher prevalence of pelvic abnormalities. All concomitant pelvic pathologies in both groups were treated on a one-stop (see and treat) basis.Conclusions: Detection and proper management of associated pelvic pathologies at laparoscopy is a valuable additional advantage of LOD particularly in women with positive risk factors. LOD plus see and treat associated pathologies is a time saving and prompt management approach for women with PCO–associated infertility.

The study was carried out to know the effect of probiotics (Bio-Top®, Curd) supplementation on intestinal microorganisms in rats (Rattus norvegicus) during the period of 01 March to 20 April 2011. A total of twelve (12), one month old rats were randomly divided into 3 experimental groups: A, B and C, with 04 rats in each group. Group C was fed with commercial rat pellet (20 gm/rat) without supplementation of probiotics (control), group A was fed with commercial ration (20 gm/rat) with addition natural probiotic (Curd) and group B was fed with commercial ration (20 gm/rat) with the addition of commercial probiotic (Bio-Top®) feed for 7 weeks. Feed and water were supplied ad libitum. The result showed that intestinal bacterial population changed in probiotics treated groups compared to control group. Bacillus spp. was isolated from Bio-Top®, Curd and Probiotic treated rats. Whereas E. coli was present in control rats, indicating environmental contamination which is hazardous for public health.  The study suggests that probiotics supplementation helps in maintaining normal haematobiochemical values and beneficial gut microbes.DOI: http://dx.doi.org/10.3329/bjvm.v10i1-2.15654

The quest for excellence in mental and physical health is not new. We find various references and formulations in Ayurvedic classics meant for promoting mental and physical health of a child. Suvarna Prashan is one of the formulations explained in age old Ayurvedic classic Kashyap Samhita. This formulation is very widely used now days as a memory and immune booster for children. But there is very little systematic documented study which can be used to evaluate the efficacy of the formulation. Suvarna Bhasma was prepared in Ayurved Rasayani Pharmacy, Pune. Madhu and Ghrita were collected from KLE Ayurveda Pharmacy, Belgaum. Suvarna Bindu Prashan was prepared in KLE Ayurved pharmacy, Belgaum. It contains Suvarna Bhasma, Ghrita and Madhu. Twenty apparently healthy male and female children with age group of three to four years were ready to sign inform consent form were selected into two groups each. Subjects in Group A received Suvarna Bindu Prashan where as Group B (Control group) did not receive any treatment. Both the groups were observed for six months. Children in Suvarna Bindu Prashan group showed significant reduction in the scores of eating habits, behavior, mood, temperament and scores of event of illness. However there was no significant difference in the score of sleeping habit. There was significant increase in IQ percentage.

Abstract Objective To investigate the impact of microecological preparation combined with modified low-carbon diet on the glucolipid metabolism and cardiovascular complication in obese patients. Methods From August 2017 to July 2020, 66 obese patients were recruited, and administrated with an modified low-carbon diet with (group A) or without (Group B) microecology preparation and a balanced diet in control group (group C) for 6 months. Meanwhile, 20 volunteers administrated with a balanced diet were recruited as the healthy control group (group D). Results After 6-month intervention, obese subjects in group A and B showed significant improvement of body and liver fat mass, reduction of serum lipid levels, intestinal barrier function markers, insulin resistance index (IRI), high blood pressure (HBP) and carotid intima thickness, as compared with subjects in group C. More importantly, subjects in group A had better improvement of vascular endothelial elasticity and intimal thickness than subjects in group B. However, these intervention had no effect on carotid atherosclerotic plaque. Conclusion Administration of microecological preparation combined with modified low-carbon diet had better improvement of intestinal barrier function, glucose and lipid metabolism, and cardiovascular complications than low-carbon diet in obese patients, but the effect of a simple low-carb diet on carotid atherosclerotic plaque need to be further addressed.

Abstract Background This study explored the comparison of the thermal insulation effect of incubator to infusion thermometer in laparoscopic hysterectomy. Methods We assigned 75 patients enrolled in the study randomly to three groups: Group A: Used warming blanket; group B: Used warming blanket and infusion thermometer; group C: Used warming blanket and incubator. The nasopharyngeal temperature at different time points during the operation served as the primary outcome. Results The nasopharyngeal temperature of the infusion heating group was significantly higher than that of the incubator group 60 min from the beginning of surgery (T3): 36.10 ± 0.20 vs 35.81 ± 0.20 (P<0.001)90 min from the beginning of surgery (T4): 36.35 ± 0.20 vs 35.85 ± 0.17 (P<0.001). Besides, the nasopharyngeal temperature of the incubator group was significantly higher compared to that of the control group 60 min from the beginning of surgery (T3): 35.81 ± 0.20 vs 35.62 ± 0.18 (P<0.001); 90 min from the beginning of surgery (T4): 35.85 ± 0.17 vs 35.60 ± 0.17 (P<0.001). Regarding the wake-up time, that of the control group was significantly higher compared to the infusion heating group: 24 ± 4 vs 21 ± 4 (P = 0.004) and the incubator group: 24 ± 4 vs 22 ± 4 (P = 0.035). Conclusion Warming blanket (38 °C) combined infusion thermometer (37 °C) provides better perioperative thermal insulation. Hospitals without an infusion thermometer can opt for an incubator as a substitute. Trial registration This trial was registered with ChiCTR2000039162, 20 October 2020.

Objectives: To determine the effect of mechanical loading of bone on the stability and histomorphometric variables of the osseointegration of dental implants using an experimental test in an animal model. Materials and Methods: A total of 4 human implants were placed in both tibiae of 10 New Zealand rabbits (n = 40). A 6-week osseointegration was considered, and the rabbits were randomly assigned to two groups: Group A (Test group) included 5 rabbits that ran on a treadmill for 20 min daily during the osseointegration period; Group B (Controls) included the other 5 that were housed conventionally. The monitored variables were related to the primary and secondary stability of the dental implants (implant stability quotient—ISQ), vertical bone growth, bone to implant contact (BIC), area of regenerated bone and the percentage of immature matrix. Results: The results of the study show a greater vertical bone growth (Group A 1.26 ± 0.48 mm, Group B 0.32 ± 0.47 mm, p < 0.001), higher ISQ values (Group A 11.25 ± 6.10 ISQ, 15.73%; Group B 5.80 ± 5.97 ISQ, 7.99%, p = 0.006) and a higher BIC (Group A 19.37%, Group B 23.60%, p = 0.0058) for implants in the test group, with statistically significant differences. A higher percentage of immature bone matrix was observed for implants in the control group (20.68 ± 9.53) than those in the test group (15.38 ± 8.84) (p = 0.108). A larger area of regenerated bone was also observed for the test implants (Group A 280.50 ± 125.40 mm2, Group B 228.00 ± 141.40 mm2), but it was not statistically significant (p = 0.121). Conclusions: The mechanical loading of bone improves the stability and the histomorphometric variables of the osseointegration of dental implants.
