The aim of the study is to show that long standing forgotten intrauterine device contributes to infertility, reporting three cases presented at Central Hospital Warri, Nigeria, a government tertiary health center. Three cases of forgotten intrauterine contraceptive device (IUCD) contributing to infertility were seen. Two were inserted for contraceptive reasons while one was inserted while being managed for uterine synechae. Health care providers should ensure proper documentation of all procedures carried out, adequate counseling which should include taking an informed consent and also ensuring both short and long term follow up of their clients. Also all patients being evaluated for infertility and clients with past history of intrauterine device must have a speculum examination and ultrasound scan carried out.

Background: Nevus lipomatosus superficialis (NLS) is a unique developmental anomaly or nevoid form of lipoma characterized by the ectopic presence of mature adipocytes in reticular dermis. The condition is rare; apart frrom isolated case reports, there are no large case series dealing with the clinicoepidemiologic characteristics and posttreatment long-term follow-up in patients with NLS and little published information on treatment outcome. Objective: To study the clinicoepidemiologic characteristics and long-term posttreatment follow-up in patients with NLS. Methods: This was an 11-year retrospective study analyzing disease characteristics and treatment outcome in eight patients with NLS. Results: There were eight (six males, two females) patients with NLS, of whom three were children. The classic variant of NLS was the predominant presentation. One patient demonstrated a combination of both classic and solitary variants. Most patients, especially those with solitary variants, were commonly misdiagnosed before presenting to us. Four patients, including two with the solitary variant, one each with the classic and the combined type, underwent surgical resection without any recurrence over 8 years of follow-up. Conclusions: The rare nature of the disorder, which is commonly misdiagnosed, and the absence of long-term follow-up data prompted us to share our experience about NLS to increase its awareness among health care providers.

Abstract Pulmonary sequelae related to tuberculosis (TB) are among the major causes of bronchiectasis in Eastern Europe. The role of bacterial colonisation in the pathogenesis of bronchiectasis has been continuously studied over the last decades, less understood remains the impact of fungal infection, alone or in association with bacterial. Although the data on the development of chronic pulmonary aspergillosis (CPA) secondary to TB are scarce, recent evidence suggests a higher prevalence of CPA in patients with a past history of pulmonary TB than it was previously estimated. We present a case of natural evolution of CPA, with a radiological follow-up, in a patient with post-tuberculous bronchiectasis.

Background: Postpartum period is an ideal time to counsel and begin contraception as women are strongly motivated at this time, also being convenient for both patients and health-care providers. Copper T 380A is a safe, highly effective method of contraception and has become the method of choice of many women for attaining their reproductive goals.Methods: It was a prospective study conducted from February 2012 and November 2013 in the Department of Obstetrics and Gynecology, JNMCH, AMU, Aligarh, U.P.Results: 386 clients were counseled for immediate postpartum IUCD insertion (GROUP-I), out of which 34.2% clients accepted for insertion but 24.3% actually got it inserted. 337 clients were counseled for extended IUCD insertion (GROUP-II) out of these 61.1% clients accepted but only 16.5% actually got it inserted. 10.63%, 6.02% and 5.19% clients in GROUP-I and 16.22%, 13.11%, 11.54% GROUP-II went lost to follow up at 6 weeks, 3 months and 6 months respectively. Continuation rates in GROUP-I after 6 months was 73.4% in GROUP-I and 59.5% in GROUP-II. Among the clients who continued, 92.7% were satisfied in GROUP-I and 81.8% in GROUP-II.Conclusions: Immediate postpartum IUCD has better acceptability, continuation and satisfaction profile when compared to extended insertion.

Abstract E-cigarettes have surged in popularity over the last few years, particularly among youth and young adults. These battery-powered devices aerosolize e-liquids, comprised of propylene glycol (PG) and vegetable glycerin (VG), typically with nicotine, flavors and stabilizers/humectants. Although the use of combustible cigarettes is associated with several adverse health effects including multiple pulmonary and cardiovascular diseases, the effects of e-cigarettes on both short- and long-term health have only begun to be investigated. Given the recent increase in the popularity of e-cigarettes, there is an urgent need for studies to address their potential adverse health effects, particularly as many researchers have suggested that e-cigarettes may pose less of a health risk than traditional combustible cigarettes and should be used as nicotine replacements. This report is prepared for clinicians, researchers, and other health care providers to provide the current state of knowledge on how e-cigarette use might affect cardiopulmonary health, along with research gaps to be addressed in future studies.

Background: Post-partum period is defined to be an ideal time for family planning counselling. Being a second largest populated country, India has a high rate of unintended pregnancy especially in post- partum women. There is a need for accessible, reliable contraception like post-partum intra uterine contraceptive device (PPIUCD) in our country. The present study is an attempt to evaluate the safety and efficacy of intra- uterine devices in post-partum women.Methods: The study population included all the women who delivered vaginally who were counselled for PPIUCD insertion in prenatal period, early labour and within 48 hours of delivery and were willing to participate in the study. Participants were interviewed prior to discharge after receiving a PPIUCD and then six, ten, fourteen weeks and 6 months or earlier when necessary. Institutional ethical approval and informed consent were taken.Results: A total of 1625 deliveries took place out of which only 888 were inserted PPIUCD. So, the total acceptance rate was 54.6%. Majority of cases were between the age group of 21-25 (46.4%) and most of them were primipara (39.8%). 34.8% came for routine follow up in OPD at 6 weeks, 14% at 6 months and 6.1% at 1 year.Conclusions: The IUCD insertion has been proved to be a safe and effective method of contraceptive with very few side effects and no major complications. To increase the knowledge and skills among health care providers, it is necessary to arrange training on PPIUCD.

Violence against health care providers is getting more awareness nowadays. These are usually deliberate actions committed by patients or family members of them resulting in short and long term physical or psychological debilitating harm in the staff members. The causes of the violent acts are usually rooted in patient-related factors, although some characteristics of the professionals and of the workplace may also play some role. The present article presents different definitions of violence and possible reasons for violence against health care providers based on relevant international and national literature. The paper discusses the different forms and frequency of violence, furthermore, details about the effects, consequences and some options for prevention in health care settings are also included. Orv. The Children’s Oncology Group Long-Term Follow-Up Guidelines for Survivors of Childhood, Adolescent, and Young Adult Cancers are risk-based, exposure-related clinical practice guidelines intended to promote earlier detection of and intervention for complications that may potentially arise as a result of treatment for pediatric malignancies. Developed through the collaborative efforts of the Children’s Oncology Group Late Effects Committee, Nursing Discipline, and Patient Advocacy Committee, these guidelines represent a statement of consensus from a multidisciplinary panel of experts in the late effects of pediatric cancer treatment. The guidelines are both evidence-based (utilizing established associations between therapeutic exposures and late effects to identify high-risk categories) and grounded in the collective clinical experience of experts (matching the magnitude of risk with the intensity of screening recommendations). They are intended for use beginning 2 or more years following the completion of cancer therapy; however, they are not intended to provide guidance for follow-up of the survivor’s primary disease. A complementary set of patient education materials (“Health Links”) was developed to enhance follow-up care and broaden the application of the guidelines. The information provided in these guidelines is important for health care providers in the fields of pediatrics, oncology, internal medicine, family practice, and gynecology, as well as subspecialists in many fields. Implementation of these guidelines is intended to increase awareness of potential late effects and to standardize and enhance follow-up care provided to survivors of pediatric cancer throughout the lifespan. Background: Diabetes is one among chronic diseases requiring long term medication and its prevalence is increasing globally. Adherence to prescribed treatment among diabetes patients is very important for good glycemic control. Poor glycemic control following poor adherence is associated with increasing risk of complications, disease progression, morbidity and mortality with increasing costs of care. Hence, the present study was undertaken to assess the adherence of diabetic patients with their prescribed medications.Methods: The study involved 200 diabetic patients on oral anti-diabetic medication visiting out-patient diabetic clinic in a teaching hospital, Bangalore, who met required criteria. Information was collected using predesigned questionnaires by personally interviewing selected patients.Results: A total of 122 patients (61 %) were non-adherent with medication; 36 (18%) and 42 (21%) of patients were moderately adherent and adherent to treatment respectively. Among non adherent patients, males were more (72%) compared to females (50%), which was statistically significant (P<0.05). Non adherence was more among employed (69%) patients compared to unemployed patients (54%), this was significant statistically (p >0.05). Non adherence level was very high among males with history of smoking (80%).Conclusions: It was observed that the proportion of diabetic patients who are non-adherent to their prescribed medications was high. It is recommended that patients should be counselled repeatedly by health care providers on the importance of complying with prescribed drug regimen for better compliance and treatment outcome.

The primary objective of this study was to assess the novel fixation method of a frameless copper-releasing intrauterine device inserted following placental delivery during cesarean section and analyze its impact in reducing device displacement and expulsion during and after uterine involution. We hypothesized that the dual-anchoring technique could reduce the risk of intrauterine device displacement and expulsion during and after the uterine involution. The study was conducted at the Gazi University Medicine Faculty Hospital in Ankara, Turkey. Twenty-one pregnant women were enrolled. Insertion was performed following placental removal. To confirm the proper placement and good retention of the device, the distance between the fundal serosa (S) and device anchor knot (A) was measured (S–A) during follow-ups, by ultrasound. There were significant differences in the S–A, as observed by ultrasound at discharge and at 6 weeks post-delivery, which is consistent with the tissue contractions associated with uterine involution. Notwithstanding the uterine involution, no device displacements or expulsions occurred, which indicated a good retention of the frameless device. This innovative retention method of the frameless intrauterine device ensures a well-tolerated, long-term contraception, allowing for immediate contraception and proper pregnancy spacing for cesarean scar healing, and overcomes the issue of expulsion encountered with conventional intrauterine systems.
