Approximately ten cases have been documented of people falling out of the side exit doors of moving passenger trains throughout the US. Most of these individuals were elderly passengers and the events usually happened late at night. Amtrak has classified most of these accidents as individuals with mental disorders or suicidal tendencies. Legal actions at the present time have been dismissed by the courts due primarily to the lack of any witnesses to the events. An investigation was conducted of a passenger car allegedly involved in one of these accidents. A number of recommendations were made including: 1) a systems engineering study of the problem, 2) an interlock system which would prevent the side exit door locking handle from being activated while the train is in motion, 3) an “All Green Board” at the engineer's station to indicate that all doors on the train are locked before starting the train, 4) a protective translucent cover installed over the regular door handle to indicate the criticality of opening this door, 5) a new warning sign on the exit doors which follows the ANSI standard, and 6) a training program, related to this dangerous situation, for all Amtrak operational personnel.

During the LHC Long Shutdown 2, the ALICE experiment has undergone numerous upgrades to cope with the large amount of data expected. Among all new elements integrated into ALICE, the experiment counts with a new Inner Tracking System (ITS), with innovative pixel sensors that will substantially improve the performance of the system. The new detector is powered up through a complex Low Voltage (LV) distribution, increasing the power dissipated by the detector and requiring the installation of a large number of temperature measurement points. In 2020, a new safety system has been developed to distribute the ITS LV interlock system and to monitor the new temperature values. The safety system is based on a Siemens S7-1500 PLC device. The control application governing the PLC has been configured through the UNICOS-CPC infrastructure made at CERN for the standardisation of industrial applications. UNICOS-CPC enables both the automatisation of control tasks governing the PLC and the interface to the WinCC OA based SCADA system. This paper provides a complete description of the setup of this safety system.

A novel translating bed total body irradiation treatment delivery technique that employs dynamically shaped beams is presented. The patient is translated along the floor on a moving bed through a stationary radiation beam and the shape of the radiation beam is changed dynamically as the patient is moved through it, enabling compensation for local variations in patient thickness and tissue density. We demonstrate that the use of dynamically shaped beams results in greatly improved dose homogeneity compared with standard techniques, which use a single static beam shape. Along a representative dose profile through the lungs of a mock-human body, the maximum range of dose deviation from the average is 5.6% (from +2.7% to −2.9%) for the dynamic beam technique compared with 12.8% (from +3.6% to −9.2%) for the static beam technique. A novel, dual-interlock system that prevents bed motion when the radiation beam is stopped and stops the radiation beam when the bed motor is stopped has also been developed. The dual-interlock not only enhances the safety of the treatment but also ensures accuracy in the delivery of the treatment.
